Bugrep creation

This document describes how to create a Bugrep. The Bug report program is used to save a current system status composed of a specific group of files, (as SYSLOG, Error log, and more) into /var/tmp/Problems directory. And later should be forwarded to R&D for evaluation. As soon as a problem occurs in the system a Bugrep has to be created by the F.E. or by the operator.Note: Avoid Log out until the Bugrep is created. If log out Log in was performed after the problem was observed, then most of the relevant data is lost.

1. Press on Service.2. Select Bugrep.3. A dialog box opens.4. At Problem Name, write a short description of the

problem.5. At Submitter, write your name.6. Mark the 3 small squares to the left of Save Irs

state Save Core filesCompress report

7. At Problem Description, fill out a detailed description of the

Problem including actions taken. It may be as detailed as Needed.

8. The next line is the path of the Bugrep file : var/tmp/Problems

9. Press on Save report for saving, The saving will take several

Minutes.10. Open root and login then go to the var/tmp/Problems

Directory and convert the file to a tar file so you can ftp it v

and then e-mail it to Tech support personnel. Example:

var/tmp/Problems tar –cvf (give it a name here).tar (put the file to be compressed here) the press the enter key. A file called by the name you gave it with a .tar extension will be created. FTP this to your laptop and attach it to an e-mail to send.

11. A typical file is: var/tmp/Problems/20000106- 0120.9053.aaa.bbb.3205/

12. tar –cvf Hospital.tar 20000106- 0120.9053.aaa.bbb.3205/ (spaces between r in tar and – cvf, space between f in cvf and Hospital, space between r in .tar and 2000)

13. Hospital .tar will be created14. Note: Once you had FTP the Bugrep, clean the

accumulated reports by using the clean up Icon.

15. Bugrep size may be as large as 20-30 Mb, depend how long

time Elapsed since the last Log in.

To Save bugrep to EOD perform the following:1. Save bugrep2. Return to bugrep window again .3. Click on Export4. Choose the bugrep file from the list.5. Click on Export and choose the EOD option.

WCS, Schrieber Chiller and Gantry Cooling Notes By Darius Sampey

The WCS attempts to keep water temperature to gantry 18.5 C. When monitoring the WCS control card with a laptop using hyperterm you can see what the temperature sensors are seeing. All temperatures are C/10. Tprim is temp. of water coming from chiller or facilities chilled water. Tact is the temp. in loop to gantry after being cooled in WCS exchanger. The WCS control card adjusts the mixer valve in the WCS to control the amount of gantry water that will pass through the exchanger to keep it at 18.5 C. In=full cooling, Out=full bypass or no cooling of water. If Tprim is below 18.5 C the controller should adjust the WCS mixer valve so that Tact is 18.5 C. This mixer moves almost constantly bases on heat being exchanged, and demands of gantry. If Tprim is above 18.5 C the valve will go full in and pass all water through the exchanger attempting to get to 18.5 C. If this occurs Tact can only get down to or close to Tprim. If Tprim goes above 27.4 C the pump motor will shut off. This condition can occur if say you have a Schreiber chiller and the compressor shuts off, but pump keeps running. The water in Tprim will gradually warm to over 27.4 C and pump in WCS will shut off. You would need to get water cooled again and power scanner down then back up to reset everything. System will run fine and not overheat even if Tprim is around 22-23 C.

The gantry mixer valve is controlled by the MCU. It decides how much of this cool water from the WCS it will allow to circulate into gantry. Full out=full bypass and no cool water into gantry, full in=open and all water goes into gantry. There are 2 temp sensors in the DMS cover. They are the small ones

17

Page 18: Mx8000 Reference Hand Book 01-18-06

mounted on little pcb’s with 3 dipswitches on them(settings are different for each). There is one in each corner of the cover. One sends temp. feedback to MCU so it knows where to set the gantry mixer valve to keep gantry temp. constant. The other one monitors temp. for the purpose of shutting down gantry systems in case of overheating.

Schrieber chillers come with 2 cables for hooking up the remote control panel. Both must be used. The large cable has about 12-14 multistrand insulated wires in it. The other is smaller like a phone cable with 4 solid core insulated copper wires. This small cable must be used when hooking up the remote panel. It is used for temp. sensor feedback and control in the chiller. You can tell where they go in chiller and control panel as there are solid core wires hooked to the terminal strips in each. This cable is used in place of and not in addition to the multistrand wires. If you use the multistrand wires the temp. of the water from the chiller will fluctuate by as much as 20 deg. The control panel readouts will stay stable, but if you monitor Tprim it will not stay consistent day to day. Also if placed next to a wall the side with coils must be 24 inches or more from the wall (according to manufacturers manual) to allow for adequate cooling of compressor. Compressor will overheat if side cover or service panel is left off.

Chiller Notes: The water pump in the WCS must run ccw, when facing the rear of the pump. Also the charging Hoses must be ½” inside diameter.

Water connections: Inside the WCS, the hose with the red stripe is connected To the connection with the RED handle. Also the hoses Must be terminated in the WCS. The Gantry side has the Factory ends.Temperature Sensors: R1 and R2 resistance should read 1,800 ohms at 55 degrees. You can measure them on X10 connector on the WCS control board. R1 is the water going to the Gantry and R2 is the supply water from the chiller.

Thermal couple cable: RWS wire and cable inc 770-469-3515 has type K thermal couple cable.

18

Page 19: Mx8000 Reference Hand Book 01-18-06

Shriber Chiller Water Flow: The Shriber chiller water pump usually runs at 20-25gpm. With the water temp set for 56 degrees, the flow needs to be decreased to 8-10gpm. Slowing down the flow by closing off the shut off valves can damage the pump in the Shriber. The proper way to slow down the flow is to open up the bypass valve in the shriber. This valve is located usually just above the water pump in the Shriber chiller. The valve has a red handle and when in full bypass the handle is vertical or inline with the water pipe. This will reduce the flow rate to around 8-10 gpm. The valve is set at the factory in the horizontal position to allow full flow.

How to monitor the WCS with a laptop

1. Connect a 9 pin RS 232 cable between the Laptop and the WCS control PCB.

1. Set up the laptop with a communication program(Hyper Terminal) to direct connect.

2. Once a connection is made type in STA 1 enter.3. The status of the WCS will be shown on the laptop screen.

WCS Leds meaning:

Monitoring and Bypassing the WCS

The WCS temperatures can be monitored using an RS232 cable and a laptop. Connect the laptop to X2 on the WCS pcb. Using HyperTerminal, set up a connection using your COM port 1, 9600 Baud, N, 8, 1 with XON/XOFF. Press S1 on the WCS pcb (reset). You should see a table display on the laptop with a > sign. type in sta 1 this will display the status every 5 seconds (pick a different value for your needs).

The values are as follows (Note temperatures are in tenths of degrees Celsius);

Time Run TimeTset Thermostat setting, the desired temperature of the system should be 18.5 degrees Celsius This is in tenths of degreesTact Actual gantry water temperature from sensor R1, should be around 18.5 degrees CelsiusPrim Primary input water temperature from chiller supply from sensor R2, should be 2-14 degrees Celsiusimem ?perc ?New Ht Exchanger valve requested position, range of 0-1200

Non-proper directional rotation will cause the motor to overheat and quit. Note: if the Prim is below 17c the yellow error light will come on in the WCS control board. You need to slow the flow rate or increase the input water temp.Note: always verify that there is water flow on the inlet and that it is in the right direction The colder the water the slower the flow must be.

Bypassing the ChillerThere are two ways to cool the system in case of emergency and cooling is required. The first and suggested way is to connect a water source such as a faucet to the chilled water input of the WCS by whatever means is available. Most northern community water supplies are cold enough to allow proper operation with a minimal amount of tap water. This is the preferred method since it allows the system to regulate cooling. The second way to cool the system is not preferred because it requires bypassing the systems internal regulation of temperature. If the chiller supply fails it is possible to temporarily bypass the chiller and run the cooling from another water source of cool water such as a faucet. To do this connect up hoses to the WCS in the normal manner of filling the unit. Turn off the Red Handled valve and turn on the valves for each of the hose input lines. Disconnect the two valves (one in the WCS and the one on the right side of the gantry) and manually turn them both fully clockwise. Turn off the motor for the pump by turning off F3 in the WCS. Keep the water running and monitor the system temperature using the method above. Regulate the water flow and temperature as best you can to keep around 18.5 degrees Celsius. This should get you by while your chiller is being repaired.

RAW DATA

How to identify a Raw data file and transfer it to and from the EOD for

Troubleshooting purposes.

1. Save the raw in the IRS by selecting under Service Parameters “Raw for Diagnostic only” and perform a scan.

2. At processing option chose a viewer.

3. At the viewer chose IRS. (Note refresh does not work switch between local and IRS top update IRS files).

4. From the Raw Data Parameters write down the date and the exact time e.g. hour, minute, seconds

5. Open an xterm window password: pkserv99

6. Type cd /usr/diamond.root/IRSbin/Scsi_shell <enter>

7. Type ./sls

8. You will see a list of Raw data files. Search for the desired raw data according to the exact date and time.

Note: The time you see at the file may differ by 1 to 2 seconds from the time you wrote previously, This is normal. If it is more than that It may be another Raw data.

9. A typical file looks like this

1.2.840.113704.1.1762561350.7483.947134403.51111111111111111This is the file name

10. copy the file to the local disk using the command. ./scph filename

This Document contains propriety information. Copying Disclosure to others or other use is prohibitive without the express written authority of Philips Medical Legal Dept.

23

Page 24: Mx8000 Reference Hand Book 01-18-06

The copied file is in the director /usr/diamond.root/IRSbin/Scsi_shell

NOTE: you may find it easier to mark and drag the file with the left and middle mouse keys respectively.

At the above directory type ls –1 to verify that the file is copied to this Directory.

13. From this directory the RAW may be copied to an EOD.

Raw transfer from Mx8000 to the EOD

1. Insert a write enabled EOD. Note (must be already formatted)

2. Mount the EOD by opening up a root window

3. Type: mount –v –o rw /dev/dsk/dks1d3s7 /EOD

4. Copy the Raw to EOD by typing:

5. cp –p /usr/diamond.root/IRSbin/Scsi_shell/<file name> /EOD

6. Verify that the data was transferred by typing: cd /EOD

7. ls –1 should show you the file you just transferred

8. Unmount the EOD by typing: umount –v /EOD

9. Since the axial raw is 13 MB, it is better to erase this raw after the transfer. Make sure that you are in the /usr/diamond.root/IRSbin/Scsi_shell directory by typing: pwd, if this is correct use the command rm <file name> to remove the file.

Note: Now that you have Raw Data in the EOD it can be transferred to another system for troubleshooting purposes.

RAW DATA transfer from the EOD to the Mx8000

4. Mount the EOD by opening up a root window and typing: mount -v –o rw /dev/dsk/dks1d3s7 /EOD

2. Copy the Raw from the EOD by typing: (Can be done from root)

cp –p /EOD/<file name> /usr/diamond.root/IRSbin/Scsi_shell

This Document contains propriety information. Copying Disclosure to others or other use is prohibitive without the express written authority of Philips Medical Legal Dept.

24

Page 25: Mx8000 Reference Hand Book 01-18-06

3 Use scps to copy the Raw back to the IRS Type: ./scps <file

name>Note: you must reboot (Note: must be done from the service terminal)

4. Now you should be able to see the new Raw from the IRS viewer and

Perform an off line reconstruction.

Laser Adjustment

To align the cover markers, you have to attach a piece of solder on the middle of the system phantom on the right and left sides. Scan the solder and open grid on the image, make sure that the phantom is centered. Adjust the solder so it will be exactly on the middle grid lines on the x axis (needs a few scans).Turn on the laser marker. The laser should fall on the solder, if not open the cover and adjust the markers. There are two bolts that holds the lasers to the cover. Open them and adjust the markers.

Calibrations

Another way to run HCORUse slice “B” only first. Run the automatic HCOR test on slice “B” only using a 4X5 QA scan on HF water or FF water. Then go to this directory. usr/diamond.root/cfg/siteThen type: jot IRSprep_siteManually bring the other slices to match slice “B” The file looks like this.0A1B2C3D

Mx P-Plane and Calibration Notes

When having to replace a P-Plane motor encoder and/or having to calibrate the P-Plane, here are some notes that should help the calibration complete successfully.

26

Page 27: Mx8000 Reference Hand Book 01-18-06

Before doing anything you need to make sure that all the mechanical connections to the motor and encoder are tight. An additional way to check the motor coupling is to go into service mode on the Delphi and manually drive the p-plane collimator to each limit. When the p-plane collimator stops moving, ensure that the motor is not still moving. Sometimes this requires some extreme torques on the motor couplings.

First go to your MCU utilities under the service tab and read the T9B file and write it down, these values are an important reference. Readings from one site were:

POSITION MM Encoder Reading

0 30 04371 15 107A2 6 17FE3 2 1B6B

Using the calibration jigs you will be reading the Delphi directly with your laptop hooked up to it. As you read these values, you must ensure two things. First, that the values are all positive, meaning that they do not start with FFXX. And secondly they should be as close as possible to your original values. To get these values to change you will have to change the index position by rotating the encoder in small increments between calibration attempts.

The physical positions on the p-plane as you go from fully open to fully closed are: -Outermost physical limit-Opto Limit Switch is made for open limit-Slightly in is the index pulse of the encoder-Then your p-plane positions

If Slice “D” is lower than Slice “A” Then you have to lower the numbers in the tables.

The amount by which to change the ZCO initialization parameter is by trial and error. Lowering the value in the table moves the x-ray beam towards the front of the gantry. If the detector value from slice A differs by more than 5% from slice D, change the ZCO initialization parameter.

Change 20 numbers at a time. Go to MCU tables to edit. The TAB table. In order to take effect after the tables have been changed you must power the Gantry off/on

Procedure to Perform No Load Test on the X-Ray Generator of the MX8000In the event of X-Ray system instability or for troubleshooting purposes, we may want to separate the X-Ray tube from the rest of the generator and perform a No Load Test.1. Make sure that the system power is off.2. Remove the high tension cables from the HT transformer.3. Insert the dummy plugs (part No. SP711621791) in the HV outlets of the transformer4. Move switch 301 to position 0 to disable the scan drive and power up the system.5. Using CanPro load and run the scenario called sta_4.tsc (this scenario is locatedunder c:/mx8000/scenario/diag/xrs_tst/xrs_test.IMPORTANT: Because the X-Ray tube is not in the system during these tests the generator will stay charged for a long period of time. It is very important to wait 15 minutes between exposures to allow for the high voltage to discharge6. Wait 15 minutes and run the same scenario again7. FAQ

- Artifacts on Surview scan . The surview scan uses the air

calibration from the half second one mm for it’s calibration. The half-second

scan is an option and not all sites purchased it. The artifact scenario will

most likely occur if a GI card has been replaced. Solution - As a temporary

workaround, you will need to contact your service manager and COM to get

keys to enable this option. Once the unit is calibrated, then the old option

keys must be installed to disable the half second scan if the customer has

not purchased it.

- When trying to correct the CT number values in head images using HCOR, the values remain the same. Also,

the filter EB is not present.Solution - Most likely the system has ultra image, which is also known as

IBC. Per HCOR engineering, IBC enabled protocols use the body HCOR to generate the correct CT numbers. So, if you have a IBC enabled system,

then use the axial Sinus or TAC protocol to complete HCOR for small FOV.

Then complete the large FOV HCOR. Go back and check the IBC enabled

protocols to verify that the CT numbers are correct. if not then try to split

the difference using the large FOV HCOR for the respective KV (low,

medium, & high).

A - Although the above should update the files, at times, it like other computers become obstinate In this case, you will have to manually modify the /etc/sys_id file to reflect the host name and /etc/hosts to reflect the IP address to the host name. Filming

04/05/01 Q - Connecting a Dry view 8700 to the system. The ip address, AET, etc. are configured, but we can not film.A - Ensure that Kodak service is setting up to receive an AET of 'proFILM'. This is usually missed and unless you are careful, it looks like our AET to the camera is 'proDISK'. So the setup for Kodak should be port 104, AET of proFILM, and the ip address.

A - Per Bob Pittman: "I have experienced F9 circuit breaker randomly tripping. F9 is the circuit breaker for T1, the low voltage transformer. I found the voltage on the output side measuring 34v AC. The transformer output is rated, and should be 24v AC. The primary side was tapped for a 230v AC input. The input actually measures 277v AC. I moved the primary tap on T1 to the 240V position, which is the highest position. This lowered the output voltage to 29v AC. Since I moved the tap, I haven't had any more F9 breaker trips." This has solved the issue at two other sites and should be checked at your site. Retrospective02/22/01

Q - I have Cardiac Retrospective option on this system. After troubleshooting, which required the formatting of the IRS SCSI drives, the unit now has blank images for the cardiac reconstructions. What happened?A - If the IRS SCSI drives are formatted as a result of troubleshooting or installing new software, a special file required for cardiac retrospective reconstruction will be deleted. Unfortunately, restoring tables from the host does not correct the problem. The file must be restored manually from the Cardiac Retrospective Patch CDROM. This problem will be fixed in a future software release (possible V 2.2). Check whether the file "nn.dat" is missing on the SCSI by typing: '/usr/diamond.root/IRSbin/Scsi_shell/sls -t TAB nn.dat' (w/o ''). If the Table directory says "No such file", the file must be restored from the CDROM. You can do this two ways: 1.) Reinstall the patch or 2.) copy the nn.dat file manually.

To install the patch:

login as root

insert the CD into the O2.

type cd /CDROM/

type ./install

To copy manually:

insert the CD into the O2

open a window

cd /usr/diamond.root/IRSbin/Scsi_shell

./scps -t TAB /CDROM/nn.dat

Q - The customer wants to put their MxView on a UPS. However, the UPS units are specified in VA (volt amps). What size do I need?A - The O2 MxView with a 21" standard SGI monitor uses 355 watts which equates to ~ 496 VA. The O2 MxView with dual monitors uses 463 watts which equates to ~ 649 VA. This is of course assuming that you would want the O2 and monitor(s) to be supplied by the UPS. 5/3/01

Q - I just set up the MxView and added the Mx 8000 scanner as a node. However, I can not do a query retrieve of the Mx 8000. What is the proper way to set this node up.A - You must have memory under devices set up properly. The Memory AE title must be the same. 6/3/02 Q - I have a system with V2.21 and Pinpoint. After upgrading the software on the scanner to 2.21, some of the protocol virtual buttons are gone and others have scrambled text. how can I fix this?A - A problem has been identified between the compatibility of Mx8000 V2.21 software and Pinpoint 2.2 software. When pinpoint 2.2 software is loaded on top of Mx8000 V2.2 software, there are no issues. However, with the release of

Mx8000 V2.21, Pinpoint 2.2 software may cause some abnormalities with some of the virtual buttons for the protocols. Some examples might be that a protocol button disappears, or that the text in the button becomes unrecognizable. Engineering is aware of this issue and a rectification process in under way and will be implemented by the end of July. In the mean time,we suggest not to have Mx8000 V2.21 and Pinpoint 2.2 reside on the same system.WCS5/17/02 Q - How do I check if the chiller is working correctly?A - Connect your laptop to the serial port on the WCS and issue a command STA 1. This will sample the status of the WCS every second. The value of R1 is the temperature of the water coming into the WCS (primary/chiller side) and the value of R2 is the temperature on the secondary side (water leaving the WCS to the gantry).

5/17/02 Q - When I send images to a remote node, I get an error saying "can't communicate with spooler".A - Log in as root and type cd /usr/temp/ <CR> at this point remove the file called spooler.bak by typing rm spooler.bak <CR>

CANPRO (4.8)

MX8000 Error definitions and decodingIn the MX8000 there are four kinds of errors (priorities) and they are defined as follows:INFORMATION, WARNING, NORMAL ERROR and FATAL ERROR

The priority of an error depends on the reaction, which the machine

requires.

An INFORMATION (priority 3) causes no reaction in the machine. It is used

to exchange information between the control system and ICS (e.g. state of

the gantry door switch).

A WARNING (priority 2) causes no reaction in the machine except a logbook

entry. All decisions are done by the ICS depending on the type and

combination with other WARNINGS.

A NORMAL ERROR (priority 1) is sent, if a condition is detected in the

machine, which is self repairing (e.g. tube arcing, door opened while SCAN

EXECUTE with X-ray) but needs a cancel of a running scan or series. If one

of the components sends an normal error, the MAS/MCU cancels the CYCLE

and switches all components into MODE LOADED (see CANCEL SCAN). The

MAS/MCU loads the first scan of the mode table. In all other states, no

reaction is caused by this error.

A FATAL ERROR (priority 0) in the INIT, INIT DONE, TABLE LOAD or FLASH

LOAD causes a HALT state, in all others an ABORT or STOP

A fatal error in Aborted results in no further reaction.

In this example we can see two errors:

1.- E 90 00 42 the error’s priority is 0 (Fatal) and is reported by the Delphi

2.- E 50 00 17 the error’s priority is 0 (Fatal) and is reported by the XRS

Let’s look at the first error:

The first thing to do in order to decode the error is convert the second byte

of the error to decimal format

42 Hex = 66 Dec.

We know the error was reported by the DLP so we look at error number 66

reported by the DLP under the HTML directory created during your CanPro

installation:

FTP Commands

Here is a general list of FTP commands. Some may vary with different operating systems.

ftp machine.address

establishes an FTP sessions with the named machine. The machine address may be specified with either the domain name address or the numerical IP address.

ls or dir

lists the files and directories in the current directory.

cd directoryname

changes the directory to the one named. If the directory named is not a subdirectory of the current directory, then the path (eg. cd /pub/library) must be specified.

get filename

transfers the file to your local computer (or to GPU if you are FTPing from the GPU system prompt). If you want to specify a different filename type get ftpfilename localfilename.

55

Page 56: Mx8000 Reference Hand Book 01-18-06

binary

changes the transfer mode from ASCII (or text) to binary. Use this method for compressed files.

ascii

changes the transfer mode to straight text. This is the default.

help

lists available FTP commands.

quit

disconnects from the remote machine and quits FTP.

mget characters*

multiple get - By using the wildcard character *, this command transfers all files that match the filename characters specified from the remote machine.

cdup

changes the current directory to the parent directory or the one above it -- therefore "up" in the hierarchy.

dir

lists the files and directories in the current directory, showing sizes of files and other information.

! ls

Lists the files on your local system. (Only works for FTP with a UNIX system).

put filename

transfers the file specified from your local computer to the remote computer (requires correct authorization)

mput characters*

56

Page 57: Mx8000 Reference Hand Book 01-18-06

multiple put - works like the mget above only used for transferring files from your local computer to the remote computer.

lcd

changes your current directory on your local computer.

prompt

toggles interactive prompting during mget and mput commands. If interactive prompting is turned on, you will be asked to confirm each file transferred.

pwd

prints (displays) the name of the current working directory on the remote machine.

bye

same as quit

remotehelp

requests help from the remote FTP server. If a command name is also specified (eg. remotehelp cdup), it is supplied to the server as well.

UNIX NETWORK COMMANDS

ipconfig /all : shows the nic cards MAC address

This form of the ifconfig command is not supported by all versions of UNIX for example Digital UNIX 3.0 (DU 3.0) will not support the '-a' flag.  DU 4.0(MR Workstations) and NetBSD 1.2(Site Manager Servers) will support the '-a' flag.

To IP alias a ethernet card live:

Example:

# ifconfig tu0 alias 172.16.96.1 netmask 255.255.255.0

Files:Place this command in the configuration file /etc/inet.local on Digital Unix(DU) workstations to have it run on bootup. Place the ipaddress and interface (Example: 172.16.96.1  ed2) in /etc/ifaliases for NetBSD to have it run on bootup. If you have DU3.0 you will have to add the following to /sbin/init.d/inet

     if [ -f /etc/inet.local ]; then         /etc/inet.local     fi

This will run the file that holds the IP aliases.  The above 3 lines should be added just after the "/usr/sbin/umount -b" command that looks

SCAN PARAMETERS FAIL: this error comes up while in the Application screen when trying to scan. The most common problem is the Gantry rotation sw is turned off On the Left side of the Gantry.Tube conditioning Fails: Check to see if the rear switch cover is Stuck in

QUALITY SERVICE: If you see this message in can pro then the Table service switch is on. It must be off in Order to run a scenario. The Green rocker SW should be in the O position(towards the Gantry). Move the Switch to the “1” position to manually Lower it.

Auto Voice Does not work: This problem usually occurs on new Installations. Ensure that the auto voice cables Are plugged into the correct RCA jacks in Side of The O2 Computer. When looking at the side of the O2Computer it’s the bottom left two jacks.

Scan does not initialize: After clicking OK when beginning a scan the initializing message comes up and nothing happens after a couple minutes. Stop the scan then go to the viewer and right clicks the local drive. Do a quick rebuild and try again. Sometimes the database will get corrupt and needs to be rebuilt. Also another problem could be an open fiber optic link. See the notes under FIBER OPTIC LINK

Id timeout: If the service sw on the ZCO is accidentally left in the service position

Troubleshooting and DiagnosticsTroubleshooting Tools10.10.3 Download Tamar_Log File Using Ui_loggerFor troubleshooting MxViewexp and connectivity issues, it is advisable to download the Tamar_Log file by using the Ui_Loggerapplication. Tamar_Log is a file that receives messages from various software packages. By using Ui_Logger, it is possible toincrease the notification level. A higher level means more messages sent to Tamar_Log. This makes it easier to evaluate specificproblems in specific software packages.• The default notification level is 5 for all software packages.• Maximum notification level is 7.• It is advisable to change the notification level on only one software package at a time.

66

Page 67: Mx8000 Reference Hand Book 01-18-06

• Usually you only change the notification level based on instructions from the Philips Service Support department.• Make sure that you restore the notification level to the default value after you are done.1. Login as root.2. In the Windows Start menu, select Run. Type "csh" in the command line and press <Enter>.Type "usp1" and press <Enter>.Type "ui_logger" and press <Enter>.3. In the logger window click on the line which represents the software package requested by the Service Engineer.4. Click the button labeled 6 information or 7 as requested and click Apply.5. Perform the operation that caused the problem.6. In Windows Explorer go to: c:/usr/tmp.7. Copy TAMAR_LOG to TAMAR_LOG.TXT.8. If the TAMAR_LOG.TXT file is large, compress it to a .ZIP file on your laptop.NOTE:Before downloading the file, rename tamar_log to old tamar_log9. Send Philips the TAMAR_LOG.TXT file by e-mail to [email protected]. In the logger window change the desired software package back to 5 notice (default reporting level), click Apply and closethe window.11. Delete TAMAR_LOG.TXT.12. Rename the old tamarlog file with its original name.

IRS Notes:

SCSI Diagnostics: All SCSI Diagnostics are default to run destructive tests. You must manually select non-destructive tests.

You can run one disk at a time by removing the SCSI cable from all of the drives withThe exception of the one under test. Warning, this takes a lot of time to complete all of the tests.NOTE: after running the destructive SCSI tests you must format the SCSI’s.

ACQISITOR COM ERROR: If you see this error you might have a bad SCSI disk drive. Check the SCSI drives access lights and see if one stays on all the time or doesn’t come on at all. This will indicate a bad SCSI. Also, just because you can format the SCSI’S does not mean all of your drives are good. One of your SCSI’S can be bad and you still can format without any problems.

While formatting the SCSI’s all of the LED’s on the disk should be flashing. The top disk’s led will flash a little slower. If one of the lights are not coming on then that disk is bad. Also if a LED is on all of the time then the disk is bad.

2.21 software release will have the ability to see a good disk graph and a bad disk graph. This way you can compare the results of the SCSI diagnostics with known good and bad results. The graphics file have a .plt extension.

The SCSI drives in the IRS cabinet are very sensitive cannot tolerate any sudden movements when spun up.This will cause the heads to strike the platter causing intermittent problems in the future. The SCSI drives should be strapped 1,2,3,4

The SCSI tables are written to the last SCSI disc

Diagnostic Tools:In Procmon under the IRS server a red highlight means there is an error. Also there should be a number in parentheses, which identifies the CE#

Hit ALT/TAB to show the active windows.

For T.S. purposes you can swap the 1st and 2nd expansion board. You can also remove one of the boards from the IRS as log as there is always a board in the first expansion slot. The IRS is always looking for a board in the 1st expansion slot. You do not have to change any jumpers when doing this.

You can also turn on the system up without the SCSI’s hooked up for troubleshooting. This is useful ifThere is a complete lockup of the system when running diagnostics.

Troubleshooting

By-passing the raceway1. Remove the Acquisitor (leave out)2. Remove the raceway cable (leave out)3. Remove the bridge4. connect the bridege between the system pcb and the 1st

expaqnsion(remove the 2nd expansion card)5. Run diagnostics. this should prove that the raceway cable

(part#0700771)is good or not.

To isolate the PCI BridgeDo the same thing as above except use the cable raceway instead of the bridge to connect from the sys pcb to the 1st expansion. Then you have to add the jumper at w3 and w1. You might have to remove the daughter pcb to get at the jumpers config for 16 sharcs

PCI Bridgeuse the middle two plugs. If the wrong plugs are used the O2 will not boot upor it will lock up at the Philips screen

Commands to Be Used in Case of a Reconstruction Problemps24 CommandThe ps24 command is to be used after attempting the reconstruction of raw data. The ps24 command is useful when reconstruction halts or no image isproduced. The ps24 command shows the current state of all CEs.Normally all CEs are in a READY state after a reconstruction has been performed. To verify the state of all CEs following an attemptedreconstruction:

1. Attempt to reconstruct an image.2. From the “Service” pull down menu open the “Service Terminal”and enter the password (letmein).3. Type: cd / and press <Enter>.

DFS error during calibration. Parameter too large out of tolerance.

1. You can restore earlier calibration files from EOD And start over.2. If 1. Does not work you may have to copy a file from the software CD to the local disk that will restore the original calibration file for DFS and start over. The initial values file is located in /usr/diamond.root/private_cfg XRT_InitCalibTable.prcfg. This file must be overwritten with the same file on the CDROM disk. From the above directory change the directory to cd /CDROM/V????/diamond.root/private_cfg the ???? is the version of software you are running. Try using the ls command to find the directory name. Copy the file from the CDROM to the local directory and try DFS again. Reboot the system and try DFS again.

16. Press on Service.17. Select Bugrep.

2. Basic Mx8000 troubleshooting

2.1 Examining Bug Report Files

It is better to analyze bug reports in Mx8000 Service Terminal window – it has necessary environment for running utilities for data files analysis. Most of the files in bug report are in text format and may be viewed with any text editor/viewer like “jot”, “vi” or “more” – on Mx8000 O2 computer or laptop. See “compressed/uncompressed storage…” part for handling compressed files. Grep, wc, head, tail, cat and find utilities help a lot in work with text files.

Binary files should be converted into text format for problem analysis. All the conversion utilities work with uncompressed files – you have to uncompress them with gunzip utility.

thf.dat and othf.dat filesThe following command will create tube history in text format - thf.txt:/usr/diamond.root/bin/tubefile.exe thf.dat

mdu_after_align, raw.dat, recon.dat files are in MDU binary formatTwo utilities convert MDU files to text:/usr/usp.root/bin/d3dump “mdu_file_name”and/usr/diamond.root/bin/PrintMDU “mdu_file_name”

D3dump prints generic DICOM fields and PrintMDU prints Mx8000 specific structures.

study_file.dat fileThe following command will print content of the study file:/usr/diamond.root/bin/PrintSTUDY study_file.dat

77

Page 78: Mx8000 Reference Hand Book 01-18-06

D3dump, PrintMDU and PrintSTUDY utilities print text on standard output. Unix shell redirect may be used to save output to file:/usr/usp.root/bin/d3dump “mdu_file_name” > “mdu_file_name”.txt

2.2 Troubleshooting of frequent problems

2.2.1 Not all the images are arrived to viewer/archive

It is necessary to check data path from acquisitor to host:

log/acqmon.log file - shows requested/acquired number of fansin “Last SCAN” line:

<ACQ>Scan succeededIn Fnction: _FncMkDataNotFound ()In Fnction: ScanOKTerm()In Fnction: _SchedIDLE ()In Fnction: BiasInitComm()In Fnction: BiasStopComm()In Fnction: BiasDoCdrh()<ACQ>CDRH not SelectedIn Fnction: BiasInitComm()<ACQ>Req.:_Hndl_X_RAY_ONupIn Fnction: BiasFoundMkData ()In Fnction: ScanInit ()In Fnction: ScanCountDummy()In Fnction: ScanAcquireFast()In Fnction: ScanAcquireData()Last SCAN(Req = 0000916c, Acq = 0000916c)

Number of fans is shown in Hex. In this example exactly the requested number of fans was acquired. In some cases acquired number of fans is less than requested and if all the images are reconstructed it is not a problem. But if, for example, only part of the images was received and about the same relative part of fans was acquired by acquisitor - there is a good chance of gantry/acquisitor fault. In cases when acquisitor stops the scan because of critical errors the following lines may be found in acqmon.log:

In Fnction: ScanErrTerm()<ACQ>Set up ACQ_OK line

78

Page 79: Mx8000 Reference Hand Book 01-18-06

The next check point is trc/IrsErr.log file. It shows scan/recon requests, number of reconstructed images and IRS errors:

In some cases when IRS reconstructed all the images but only a part was received by Viewer/archive, the problem may be caused by IRS – Host communication. In this caseLog/RS_server.log file may be checked to see how many images were received by Host. IRS server prints the following lines into the log file for each image received:

CRS_SMB_Read:: from SMB Header 2097148, 1610084, 1743814, 0CRS_SMB_Read:: Alignment (MSG_MAGIC=0)CRS_SMB_Read::After bcopy :cccccccc 69 133725 22CRS_SMB_Read: reading from SMB 16 bytesCRS_SMB_Read:read # 22 name I2H_IMAGE_SENDRS_Message: Image size = 534900Tilt Corr: DeltaY=-19.4952 DeltaZ=-3.08773CRS_Image::set_image_position FOV = 228, x0 = 0, y0 = -71, BedHeight = 125, BedPos = -638.588CRS_Image::tilt_image_correction dy_pix=0 d=0 FOV=228 matrix=512 tilt=-18dy= 0

If all the images arrived to Viewer but not to the archiveit is necessary to check procmon.out file for crashed processes and ls_usp_data for old Dsh files in /usr/usp.data/dsh/recovery directory.

In Prep Data if you are troubleshooting a ring, you will have a vertical line

Veiwing prep data the detector data is the X, the Views are the Y

Don’t swap GI’s to #1 or #42

CT# Shift could be from a bad reference detector

Use Fused 5’s on P/F to reduce the partial Volume artifact from the Bone

If One Slice has an artifact, you could have a bad phantom cal, to troubleshoot take out phantom calibration and image balance

Factors that effect image quality MAS = (I*RT) / Pitch Pitch (.325 – 1.5) Pitch is = (Bed Velocity * Rotation

Time) / Slice Width Rotation Time .5, .75, 1, 1.5 & 2 (no DFS on .5) kV Filter Slice Width Focal Spot

Small Focal Spot = 40 kW Large Focal Spot = 60 kW

Small Pitch will always give you better Images, but you get less coverage

Pitch Over 1, the recon goes from 360 to 180, and the reduces the MAS

MAS of Axial is (I * RT)

.5 Rotation Time has 431 microsecond integration time!

Troubleshooting O2 Crashes and Clearing Old Core Dumps

O2 crashes and you don't know why. Log into the unit as root. Go to the toolchest and open a unix window. Change directory to /usr/adm/crash. Use the command 'ls -l' and find the files dates that correspond to the O2 crash (core dump). If the core dump was caused by something on the O2 and not the application then you should see an analysis file that corresponds to the time and date that the O2 crashed. Do a more on the analysis.X filee.g. - at the command prompt type 'more analysis.8' This information contains all the data and explanations as to the crash. Also, be advised that everytime that there is a core dump that the dump also does a copy of the IRX and you should see this in this directory. If you have numerous large files in this directory, then you should delete them. This will add stability to the O2 by not hogging the free disk space. Application induced core dumps can cause the system to slow down. These instructions will lead you through deleting these unnecessary files. Login as root, go to the tool chest, select find, then search for files. Once the box opens; type in 'core' (w/o '') for the file name, and '/' (w/o '') for look in. You will then see icons with file names. Look for the car crash icons. Once the above is found, on the left side you will see 3 boxes just above the thumbwheel. Select the second box down. This wil give you the icon along with the path and the file name. Write down the path andfilename for ONLY the car crash icon. Open a unix window and go to the directory, then do a rm -r filename. If necessary, do this for each path and filename with the car crash icon. The SGI contact is George Kozak and is number is 216.520.1906

FIBER OPTIC LINK 6/01

If the fiber optic link between the ACU in the IRIS box to the MCU in the Gantry is open, an initializing message will come up when trying to scan. Nothing else will happen. Check PROCMAN then look under ACQUISITOR. The first thing the DMS sends back to the Acquisitor when ready is the BIAS. If you do not see BIAS then the link is open. Also you should see the Acquisitor counting up. There are also visual indicators on the back of the Acquisitor. The top Led is the VCC and should be red.

84

Page 85: Mx8000 Reference Hand Book 01-18-06

The next one down is the comm_En and should be on if everything is ok. The third one down is comm_Er and it should be out if everything is ok. If the link is open then this led will be on. Also if the comm_Er led is blinking or dimly lit then there is a problem with the Acquisitor, Rcom or fiber cable. Also if the comm. 1 and comm. 2 cables are swapped at the computer you will get an Acquisitor Comm Error.
