• “There has been some increasing elevation of the 5th toe, with a callous/calculus over the dorsolateral/dorsallateral aspect of the toe at the PID/PIP joint.”

• “He has tenderness of the lower cervical/cervix spine and over the left trapezius/trapzoid, but he has full range of motion of his C-spine.

• Medications: • The patient received a codeine/cortisone injection

to relieve the inflammation• The patient was given Biaxin/Vioxx for his bone

infection.
