• Create transparency on PI data.
• Create accountability within site staff. Help staff be accountable .
• Ensure all staff abreast of training infrastructure, PI outcomes & PDSAs.
• Help PCMH teams & Coach circumvent implementation barriers.
• Guide PCMH teams & Coaches.
• Build highly effective teams.
• Design team-based workflows and train PCMH teams on how to implement them.
• Train teams on our national standards (NCQA, ACO).
