• 2011 - Electrician struck and killed by LHD while coiling electrical cable.

• 2011- Geologist picked up in bucket of 8 yard• 2010 - Mechanic had his legs slightly pinned between the boom

truck‘s bumper & the Toyota’s open tail gate• 2010 - LHD operator sounded his horn and moved ahead about a foot

and stopped, at the same time he heard a yell.• 2008 - Toyota truck operator seeing the scoop tram come towards

him, started backing down ramp but could not move out of the way quickly enough and the scoop came into contact with the front light and hood of the Toyota truck.

• 2007 - Worker struck and killed by LHD while going to the face• 2004 - The Dodge pick up truck driver who was stopped

approximately 50 ft. behind the loader tried frantically to contact the loader operator by radio, to no avail, and the loader struck the pickup, pushing it approximately 20 ft. before stopping.

• 2002 - Rock truck backed into the driver’s side door of the pickup and then stopped as he could see the pickup behind him.

• 1999 - Worker unloading screen from tractor was struck and killed by LHD coming around corner.
