REPORT ON OFFSITE MOCKDRILL CONDUCTED

IN ASSOCIATION WITH

MUTUAL AID SCHEME MEMBERS

DEVBHOOMI DWARKA & JAMNAGAR DISTRICT

ON 31st

MARCH 2015

SCOPE:

Scenario preparation and pre drill communication.

Activation of scenario on the day of the Drill in sequential manner as per the scenario prepared.

Post observers at each crucial stage to monitor response and conduct.

Evaluation of the performance at the review meeting.

Lessons learnt by each working group.

Debriefing meeting to highlight the achievements as well as shortcomings and determine the future course of action.

OBJECTIVE

To test the Emergency management plan of Tata chemicals, Aid Agencies, District Authority in general.

To find out the alertness and response time of the personnel involved in the Drill.

To find out the effectiveness of the communication system.

To check the coordination of reactions and response of emergency services

To evaluate the performance and inviting suggestions for further modifications and strengthening the plan.

To observe commitment of teams at every level for controlling emergency.

To find out the clarity in role and responsibilities.

To educate and create awareness among the villagers.

Testing emergency support systems thru standard operating procedures.

To maintain better coordination between different agencies of the Mutual Aid & Government Bodies for better management of the disaster.

PRE MOCK DRILL ACTIVITY

As per discussion with MAS and TCL management date, time and scenario of the drill was finalised. It was decided to simulate the ammonia gas leakage from half inch pressure transmitter nipple of ammonia storage tank no.1 Accordingly, the information regarding the plan of mock drill was passed on to District Collector Office, District Disaster Management Office, Office of ADISH, Police Dept , Secretary Mutual Aid Scheme Devbhoomi Dwarka, Jamnagar and requested them to depute their representatives to witness the drill and to provide feedback.

INCIDENT SCENERIO DETAILS

• Event: leakage of ammonia gas from half inch pressure transmitter nipple of ammonia Storage tank no.1 • Wind direction at the time of incident: west to east • Time of initiating the mock drill – 02:30 p.m. • Time of declaration of offsite emergency by activating the siren: 02:41 p.m. • Casualty: Two people fell unconscious due to gas inhalation

Gas leakage at ammonia storage

14.40 Hrs Control room operator activated fire turnout bell and informed about the scenario to fire supervisor, Fire Team took turnout with tender.

14.42 Hrs Fire team with fire tender arrived at site

14.42 Hrs Ambulance arrived at location

14.43 Hrs Rescue team performed search operation & two casualty shifted to in plant clinic & from their referred to Mithapur hospital

14.44 Hrs Soda Ash operation & maintenance team reached at site with required PPEs and tried to arrest the leak

14.50 Hrs Site incident controller assess the situation and advised Main incident controller

14.52 Hrs Fire Team start fire fighting operation

Water curtain placed at downwind side to minimise gas dispersion

Applied direct jet with triple purpose nozzle to leaked location

14.52 Hrs Soda Ash operation team actuated Water spray system from Bicarb plant side

14.53 Hrs Second turnout team reached at site with Fire Jeep to support first turnout team

14.55 Hrs Incident controller reached at site, observes the condition & after discussion with site incident controller & Head safety & health informed to Main incident controller at ECR to Declare off-site.

14.56 Hrs Main Incident controlled called to FIRE control room operator & instructed to actuate siren to declare Off site. He also enquired on condition of the casualties at Hospital

14.56 Hrs TCL Security head asked Police department to block the highway traffic

14.56 Hrs Fire Control room operator activated siren of Gas Leakage as per instruct from Main Incident Controller.

14.56 Hrs Fire Operator informed to all concern HODs & Other plant CCR for evacuation

14.56 Hrs Head Fire & Safety informed to MAS office, ADISH Office and other external agencies and industrial members through MAS.

14.57 Hrs Soda Ash team started evacuating ammonia from leaked tanker to the process

14.58 Hrs Water spray continued at the leakage site

15.00 Hrs TCL employees assembled at various assembly points

15.05 Hrs Okha Fire fighting crew arrived at site with fire tender and started pouring water

15.10 Hrs Dwarka fire fighting crew arrives at site and joined the rescue operation

15.15 Hrs Site incident controller informed to incident controller about success in plugging the leakage nipple and leakage was arrested.

15.25 Hrs Incident controller after assessing the site condition asked EMS cell to check the ambient ammonia concentration

15.30 Hrs EMS cell send the report of areas out of danger

15.30 Hrs Incident controller briefed main incident controller on situation under controller and no leakage from the tanker

15.40 Hrs Main incident controller after assessment asked Fire control room to blow all clear siren

15.45 Hrs Main Incident controller arrived at site for assessment and directed team present to thoroughly investigate the incident and submit the report

15.55 Hrs All gathered for debriefing meeting at auditorium

Key Opportunity for Improvement suggested by Observers

1. Emergency siren audibility to be reviewed

2. Water drainage at the entrance of ammonia storage area need improvement

3. Security staff needs more training on Emergency Preparedness

4. On line Wind indication at CCR to be set right

5. MSDS to be made available at strategic locations

6. Mock scenario to be developed using DCP to locate actual site of leakage.
