At CCC, we provide integrated health
care. This means the team starts with the
patient and includes: a primary care
provider, behavioral health care
professional, care coordinator,
psychiatric nurse specialist, medical
assistants and a clinical
pharmacist...sometimes all in one visit.
This integration helps eliminate gaps in
care and makes sure patients are getting
consistent care.
