Blue Shield. $374 / state funded nurse in your county.

• To purchase hand-held computing devices which supports application for WVEIS Wellness Data Entry and Kid’s First BMI.

Electronic Health Data Grants

• With this grant your county can purchase:– Apple devices (iPad $499, iPod touch $299 32GB, iPhone $199-399,)

• You will decide which is best for your county

– Supporting Hardware (wireless internet access points)• Wi-Fi Booster device may be necessary $75

– Travel to trainings held within RESA 5.

• Grant Obligations:– Obligation period From: 8/15/10 To: 11/30/10 Ending Liquidation: 12/31/10– Enter health screening data from all 2011-2012 enrollees utilizing WVEIS Wellness

Screens. – County should purchase equipment in October because RESA will conduct

trainings for county identified staff who will be receiving the hand-held devices in November/December

– Any county who accepts the grant and does not enter data into the devices must return these funds to the WVDE.

Electronic Health Data Grants

• Each county superintendent will determine which staff members are

to enter data and receive devices– Consult with stakeholders (pupil service directors, early childhood directors,

special education directors, school nurses, WVEIS/technology coordinators, data entry staff, etc.) to decide which hand-held device will be most effective for their county.

– A county contact, inventory, and distribution list will be required to be submitted

• By accepting this grant and purchasing devices you are agreeing to ensure that all health screening data is entered into WVEIS by

WVDE deadlines
