History and DevelopmentMiracle Beauty and Nails Academy in Virginia Beach was founded in July 2001. Miracle offers training in Cosmetology, Esthetics, Master Esthetics, Nail technology, Waxing, and Permanent Makeup. We also offer an Instructor’s course as well as specialty classes and refresher classes. Miracle Beauty and Nails Academy’s goal is to train individuals in the beauty industry by producing students with a high level of ethical standards, great customer service skills and the knowledge that exceed the highest expectations.

Our Objective:

2. To prepare students with knowledge and skills to be eligible for entry level employment in the cosmetic art field.

3. To make sure students have the correct lesson plans that accommodates their program of choice that will be devoted to both theory and practical assignments.

4. To teach students to communicate effectively in order to build a strong clientele in the work force.

Our Facilities:Miracle Beauty and Nails Academy is a state of the art facility featured with a private classroom area, 2 clinic floors, 2 skin care rooms, 4 restrooms, and library area and lunch room. We also use all professional training equipment. Miracle Beauty and Nails Academy practices Universal Sanitation Procedures and ensures that students will have working equipment and necessary supplies for proper training.

The library room provides textbooks related to the program of study for programs offered, resource cards and materials useful for study. The library also has computers available for student to use for research and school projects.

Admission Requirement/ Conditions of provisional acceptance: The candidate must be at least 16 years of age (special guidelines apply), be able to completely understand and complete the application on their own and provide a picture ID. A $50.00 nonrefundable contract fee applies. The candidate must be interviewed prior to enrollment and complete an entry questionnaire.

Student is expected to arrive at scheduled interview on time and to call promptly in case of an emergency and they cannot make the scheduled interview. Student will be accepted based on interview

process and entry questionnaire. Student will be notified of acceptance via phone or in writing of full acceptance within 7 business days.

After signing the contract, a student has three business days in which the contract may be cancelled and refunded according to the refund policy.

Licensing Requirements:We are licensed through The Department of Professional and Occupational Regulations. It is important for our students to learn and be familiar with the regulations of the state while practicing all sanitary procedures so that they can be implemented in their future work place. Certain rules and regulations we follow come directly from the Virginia State Board of Cosmetology.

hours must turn in their availability at least a week in advance unless otherwise approved.

A contract fee and a registration fee is needed for each program prior to the class start date. All necessary books and supplies must be purchased prior to training.

PROGRAM/SCHEDULE CHANGE, REENTRY AND TRANSFER Student who wish to transfer from one program to another program may put in a written request.

Students who withdraw from any program will be required to remove all items from their school locker as well as all personal items from the school. Re-admittance for any students will be determined depending on student’s status; such as current tuition payment, current academic requirements with a 75% minimum grade average, a personal interview with school administration, payment of a re-entry fee of $50.00, and probationary status for the first 30 days after re-entry.

1. Transfer of hours will be based on program requirements and will be put in writing amount to be credited. 2. Student will be responsible for all books and supplies necessary for the program in which they are transferring into. (except items included in course cost)3. Student will receive any refund from previous program according to refund policy and will be charged according to price for new program. 4. Outstanding tuition, and all outstanding fees must be paid in advance or satisfactory arrangements must be made with the administration in order to transfer or be readmitted to the school. 5. Students requesting to be readmitted to the academy must submit a new contract and pay the contract fee of $50. 6. Students wishing to transfer to another institution must pay all monies owed and all applicable requirements must be met in order for the hours to be released. There will be a $50.00 fee for official transcript. The school does not make any guarantee that another school must or will accept hours. It is at their sole discretion.

Request of schedule change must be put in writing and turned into admissions office. Request is subject to being approved or disapproved. There should be a total of 30 days in between any schedule change request. *Requests will take 10 school days to become effective.

School will not grant credit for life or work experience.

Transfer credits accepted by the school:Miracle does not accept school hours/credit from another school based on the guidelines as set forth by the school licensing agency, DPOR (18VAC41-20-280.B.1/18VAC41-40-260.B.1).

Miracle Beauty and Nails Academy does not recruit students enrolled in the same program from another institute. Also, potential students being interviewed are strongly encouraged to research and visit other schools before making a final decision.

Terms of Expulsion/TerminationThe school reserves the right to dismiss a student for any of the following reasons:

1) Excessive absences-a student cannot have more than five unexcused absences during their enrollment. 2) Misconduct in behavior-a student must display professionalism at all times.

3) Disrespecting School officials-disrespecting or insubordination to school officials.

If students are dismissed due the above reasons, student will not be allowed to be re-admitted into the school.

Academics/Grading Policy/ make up policy

Students must complete all theory assignments, practical assignments and class participation in order to complete the course. Students must maintain a theory grade average of 75%, complete all practical performance, and past a written and practical final exam prior to completion of the course. Students must make up poor or missed test and incomplete assignments within 7 days of the original test or assignment date. Students are allowed one retake for any test in which their score is below 70%. The retest grade is the grade that

will be recorded. If a student’s average falls below 75%, extra assignment(s) will be given in order to bring up the average. If the average remains below 75%, student will have to meet with the appropriate staff to determine actions to be taken.

Grading System: Numerical grades are considered according to the following scale: Written, Practical and Class Participation90-100 A (above average) 4 Excellent80-89 B (average) 3 Very Good70-79 C (below average) 2 SatisfactoryBelow 70 (Poor-unsatisfactory) 1 Unsatisfactory

Please allow 3 school days for instructor(s) to grade test and 7 school days for special assignments. If you do not have your test grade returned to you in the time indicated above; you may ask for your grade to be given to you immediately.

Students will receive a progress report quarterly which will typically be done in the months of January, April, July, and October.

Probationary guideline:If a student fails to maintain an average as listed above, they are subject to be placed on probation for unsatisfactory grades/progress and also attendance. The probationary period will be 30 days to allow the student to improve their assessment. Once the student has satisfactory status as listed above, the probation will be cleared. If the student does not improve the grade or attendance rate within the 30 day probationary period, the student will be dismissed from the program. The school may allow student to be readmitted if they were dismissed due to unsatisfactory grades and or attendance after 30 days, if the student can provide written explanation of why they believe they experienced a difficult time and how they believe they will be able to maintain if readmitted. Upon re-admittance, student will be placed on a 60 day probation where they will have to be able to maintain a satisfactory status. If they are not able to maintain, the student will be terminated and not allow to be readmitted again. Student will then receive a refund according to the policy.

Special Assignment Students will have a Portfolio due prior to completion of program. Their portfolio will display before and after pictures of the services the performed. This is something they can also use to showcase to future employers.

Goal sheets are also due as assigned. The purpose of the goal sheet is to keep students on track with their finish date.

Students must always bring all school supplies, including school books, and any supplies and products for training.

Attendance PolicyStudents are expected to attend classes as scheduled and maintain at least an 75% attendance average to stay on track in completing program on time. If a student falls below this rate they will be advised to request to come in and make up time. A student may request time off by turning in the designated form to the office for approval at least three school days prior. Written approval must be granted in order to have a request excused.For emergency circumstances where three days’ notice cannot be given; the three day notice may be waived but it has to be approved through the admissions office. Students may also request to change their schedule permanently by making the request in writing on the “request schedule change” form. Once approved, it will take 10 school days to be effective. There is no penalty for an excused absence or emergencies however your graduation date will not change. Students that missed time and was unable to attend scheduled class will have to request to come in and make-up those missed hours. If a student falls below their 75% rate for attendance:

Student will be advised of their rate (student also access to student portal which updates attendance and grades weekly)

Student will have 30 days to bring their percentage to acceptance rate

If student is unable to bring up percentage in that time frame, student will be suspended for a total of 30 days

On student’s return, student must maintain a 75% attendance rate. If student’s rate falls below 75% on the next hourly report, student will be dropped from program. (refer to school’s terms of termination)

Students are considered tardy if they do not arrive at their contracted time to attend class. This rule may appear to be strict, but has been

enforced due to the type of career training the school offers. (In the industry, technicians must arrive on time or early to serve clients).Penalties for arriving to class late within a 30 day period:

First time: verbal warning Second time: written warning Third time: counted as absent and student is sent home

If a student does not attend class for more than seven days and was not on a LOA, student will be contacted by admissions. Student will then have to have an explanation for accrued absent days. Unexcused absences for longer than 14 days are grounds for termination and students will not be allowed to readmit if terminated on these grounds.

Students must make up any test of assignments they missed due to any absences. A student will be granted 7 days to makeup work, and an excused absence or emergencies may be granted more time based on the situation. If a student fails to adhere to the attendance policy, they have to make up their time. Students will be allowed to put in extra hour(s) in a day by putting in writing the reason they have fell behind and also have any necessary documentation to follow. Students are given monthly reports and have to turn in quarterly goal sheets to help keep them on track. They will have 30 days to correct any missed times. All students should maintain 75% of their scheduled time in attendance for their monthly reports. Records are updated monthly in our system. If you are a VA student this may interfere with issued payments that but may be receiving.

Students Rights, Privileges and ResponsibilitiesStudents have the right to discuss any problems that they might have. Their first point of contact would be with their instructor. If the situation could not be corrected, their next point of contact would be with the school’s director.

Grievance PolicyIf a student has a complaint about the school or any staff member you must first allow the staff member to resolve the problem. If the student disagrees with the resolution they can submit in writing, a request to meet with the Director who will then address the problem.

(Remember: Complaints cannot be resolved by discussing them amongst each other instead of the school staff member or Director). Student will not be subject to unfair action and/or treatment by any school official as a result of the initiation of a complaint. The school desires to provide a pleasant environment for all parties of interest and will therefore address all issues professionally and fairly. Students (parent or guardian, if minor) have the right to access their records. A meeting must be scheduled with the school’s Director. If a student wants their records released to a third party, written consent must be submitted to the Director.

Student information will not be published in school directory unless approved by student (parent or guardian, if minor).

Student Right-To-Know and Campus Security Act information is availability in the school library and upon request.

There are lockers available for students. Students must bring in their own lock and key, or combination lock. Lockers are subject to inspection by the Director at anytime. The school is not responsible for any items left in the school.

Students are responsible for performing cleaning duties each day. Proper sanitation and disinfection are very important parts of training.

Graduation RequirementIt is required that all students complete all practical, theory and required hours according the state board regulations. The school will provide student with necessary information for state board testing once all obligations to the school has been met. State board testing and licensing fees are the responsibility of the student. Students must pass state board testing in order to receive a license.

Upon Satisfactory Completion of Course Student will receive a Certificate of Completion.

Optional TestingStudents will be able to schedule a mock test, courtesy of the school to get prepared for state board testing. *(Not required, optional)

Student records:The school maintains students’ records in the administrative office during student enrollment. The student record in the administrative office is available to school official only to maintain student confidentiality. The student records include their personal profile information, any notes regarding enrollment of attendance and their grades and progress reports. Upon completion of the course, the student records are kept indefinitely in an area that maintains confidentiality.

If a student chooses to only make minimum payments, there will be a balance upon completion of the course. Students are encouraged to make more than just the minimum payments available. If the balance owed is not paid by the completion of the course the student will not be released to take their state licensing test. Once the balance is completely paid in full then the student will be released to take their state board test.

In-House financing is also available with a 6% finance charge. Terms vary depending on program.

Payments are paid directly to the school for students with Post 911 GI Bill. Students will not be charge for fees that Post 911 GI Bill pays directly to the school.

Each course above prepares students for entry level positions in their field of training. Students must pass a state board exam and apply for licensure before working in the field.

Student must complete all theory assignments as well as required practical assignment in order to qualify for certificate of completion.

REFRESHER COURSE- $250.00 registration fee. $7.50 per hour, must purchase necessary supplies for training; must already have been licensed in field of study–n0 certification given for this class. Hours required is based on need and desire of the student. This course is strictly to refresh in areas student desire from training already received.

Students are expected to maintain a professional attitude at all times toward fellow student, school staff and clients.

Miracle offers a school association: Miracle Student Association (MSA), for students that are interested, which provide extracurricular activities for students to better prepare them for a successful career in the beauty industry.

The training motivates student to take initiative, be responsible, and to make sound business decisions. These techniques especially benefit that student who is searching to make their dream a reality.

Students are responsible for performing any services in their scope of training during the time enrolled in school. We accept customers on practical days and students will have the opportunity to practice their skills on customers.

Students are required to maintain a regular schedule until they have completed the course, which includes-required hours, performances and test. Test includes chapter test, unit test and final exam.

Students are provided with a very detailed syllabus and students are required to read it in its entirety.

Minimal school assignments will be given for home. Students will have allotted classroom time to read and do study guide. Training includes classroom theory training and hands-on training.

Additional InformationMiracle provides scheduled time and days before or after school for students requiring additional assistance. This time can be used for academic advising and/or tutoring.

Official school transcripts fee is $25.00. Transcripts will only be released when all fees are paid in full.

Miracle does not guarantee licensing in another state

Students will be using the smart portal to clock in daily using their finger print scan. Students are required to clock in while present and to clock out each time they leave the premises (for example, lunch). If you fail to do so, you will only be given credit according to your time

sheet, which will be given to you on a regular basis. You are responsible for informing school director of any errors on the part of the school and to keep your copy of time sheet to keep count of your hours. In the event that there are problems with time sheet, we will use manual time sheets.

Students may bring in family members or friends to practice services on. Please be advised, you must schedule them through the school, and they will be required to pay for the service they receive. Family members will receive a 25% discount on week days and a 10% discount on weekends (Friday and Saturday).

Student ViolationsMiracle is also an alcohol and drug free environment. If a student is suspected of being intoxicated or having any level of alcohol in their system while present in school, they will be required to have an alcohol and/or drug test done. If it is verified that students have violated these rules as per Terms of Expulsion/Termination they will be terminated. Students will receive a written notice of termination with guidelines for termination. Student may request information in writing regarding termination and will be provided with such information.

Appealing TerminationA student may appeal a decision made by the school to terminate student due to conduct within 15 days. This appeal must be presented to the school director in writing. The student will be notified in writing of the appeal meeting and given a written notice of the school director’s decision.

Requesting a Leave of AbsenceStudent may, in the event of an emergency situation, or in work related conditions, request leave, at which time the student will become inactive. School requests that student update administration with status as often as possible. If the student must be absent for more than a month a written notice of time length and reason for extended leave should be in writing. If a student does not return to school at the time they are scheduled to return they will have 7 days to have a written letter stating their reason for not showing up to class. This letter will have to be approved by the director in order for the student to be accepted back into school. If no letter is received, student will be written off as a personal withdrawal or voluntary termination done by student and refunded according to the refund policy.

There are no limitations of number of leave of absence that may be granted, as long as student is approved for the leave of absences.

Upon completion of the program, students are asked to complete an exit questionnaire, regarding their experience at the school. This questionnaire is used to improve the quality of training and to assure the school is consistent with the mission statement.

MIRACLE BEAUTY AND NAILS ACADEMY RULES &

REGULATIONS1. Students are to arrive to school on time. Tardiness will be handled as followed: A. First time: Oral warning B. Second Time within 30 days of the 1st violation: Written warning C. Third Time within 30 days of the 2nd violation: Probation for three weeks (if within that time period, student is tardy, he/she will be suspended for 2 days.)*If no tardies are accumulated 30 days of the last violation, tardy violations will reset. If a student is to be suspended for the 2nd time due to tardy violation, they will receive three day suspension. If a student is to be suspended for a 3rd time due to a tardy violation they will receive four days suspension.

2. All tardies are to be accompanied by a phone call 30 minutes prior. If no phone call or voice message is received by the school, student will receive a verbal warning, and if the violation should occur again, a written warning. Third time automatic 1 day additional suspension to be added to the student’s current suspension. *If a student simply fails to call he or she may be suspended for the 3rd

violation alone. There will be no 30 day reset for failing to call. If a student is to be suspended a 2nd time for failing to call they will receive 2 day suspension. A 3rd time 3 day suspension, and so forth.*If a student is more than two hours late it is then considered an unexcused absence.

3. Unexcused Absences are to be handled as followed: *An unexcused absence is any absence not pre-approved, or any absence without a doctor’s note or mitigating circumstance accompanied by proper documentation. A. First Time: Oral warning B. Second Time: Written Warning C. Third Time: Suspension of 2 day.

If a student is to be suspended a 2nd time for unexcused absences, they will receive three day suspension. A 3rd time three day suspension, and so forth.*If student should have more than five unexcused absences in a term they are subject to termination.*In the event that a scheduled client is stood up by a student, on the day of an unexcused absence: automatic three day suspension. *In the event that a walk in client must be turned away, due to a student having an unexcused absence, automatic one day suspension.

4. Students are only permitted to attend class during their regularly scheduled time. A student may not “show up” to school at their leisure, and may not clock in more than 15 minutes earlier than their scheduled time. If a student wishes to come in during a non-scheduled time they must put in a written request three school days in advance, and will thereby be held accountable to the attendance and tardy policies. If student arrives during a nonscheduled time, they will not be able to attend class that day.

5. No cell phones, or electronic devices are allowed while clocked in for class. If a student is caught texting, making phone calls, talking inside/directly outside of the building they will be given a verbal warning. 6. Student must arrive to school in proper uniform, neatly groomed, with black shoes and socks. No head gear such as du rags or scarfs. No opened toe shoes.

7. Students are to be in their area of study (classroom or library,) or assigned area of practice during class hours.

8. Students are to show up with all kits and supplies and materials needed for the course of enrollment. *In addition, if a client should arrive or is scheduled and student does not have necessary supplies, they will be required to purchase supplies from the school if applicable.

*Rules and Regulations #5, #6, #7, #8: Violations will be handled as followed: A. Oral warning B. Written warning C. Suspension of one day.

9. If student is in violation again after 1st suspension, an automatic 2 day suspension will be in place, followed by an automatic three day suspension for a 2nd violation. And so Forth.

Students will be assigned duties to perform daily; your duty must be done before leaving that day. School official must give you approval to clock out for the day.

10. Students are to be dismissed for lunch by the instructor at the same time, unless working on a client, or completing scheduled training. Lunch begins at the time of dismissal. If a student is not clocked in one hour after dismissal they will be considered late.

11. Students are not to clock out for the day, for lunch or leave the building at any time unless they have been dismissed from school by a school official. All duties must be checked and purple folder must be verified.

12. Students must maintain an average grade of 75% or higher to maintain a place in the class, pass the course, and be recommended for state board examinations, or employments.

13. If a student fails to turn in special assignments ten days after original due date student will automatically receive a 0 as the grade.

14. The school is not responsible for any loss or property damaged in the school during school hours or items left at school. This includes books, manikins, etc. You are encouraged to write your name on all of your supplies. A locker is provided for students to store possessions during class. You are responsible for providing the lock.

15. When a student has been given an assignment by the instructor, student is expected to promptly execute, and complete the assignment. When the student has finished the assignment, student shall come to the Instructor for further assignments.

16. Students must maintain at least an 75% attendance average to stay on track in completing program on time. If a student falls below this rate they will be advised to request to come in and make up time.

17. Students are to maintain a professional attitude and behave in a professional manner (no profanity, gossiping, and no arguing).

18. Students are allowed to perform services on each other if there are no customers. This will only be allowed at designated times. Students will not choose when to perform services on each other. Students must be in designated area when performing services.

19. Basic services are free. These services include shampoo and basic styles, basic facial, facial waxing, and all nail services. All other services are 50% off.

20. Students are responsible for making payments on due dates. Late payment will result in a nonrefundable $25.00 late fee.

21. Students must request in writing needed time off and get written approval from the school administrator.

22. Any suspension will not change our graduation date. Students are advised to request time to come in to make up for any lost time received.

23. Disrespecting, or insubordination to school official will not be tolerated.

24. Students who fail to comply with the rules and regulations of the school may be suspended and/or terminated and charged according to the refund policy.

Cancellation Refund PolicyAny applicant not accepted into this school is due a complete refund of any fees paid (except non-refundable fee). This school reserves the right not to accept someone if it is deemed that it is not beneficial for the school or the student.

Students may cancel this contract, in writing, within three business days of signing, and will receive a refund of any fees paid except non-refundable fee. Contract fee of $50 is non-refundable

Students who fail to comply with the rules and regulations of the school may be terminated and charged according to the refund policy.
