I, __________________________________, volunteer applicant for Sean Casey Animal Rescue (SCAR) understand that while volunteering I must follow specific guidelines established by SCAR to maintain my own safety, the safety of the animals housed atSCAR, and the safety of SCAR volunteers and employees. If my volunteer application is accepted, I agree to the following terms and conditions and understand that if I do not follow the below listed regulations, my volunteer opportunity at SCAR may be terminated by any of the SCAR staff, including but not limited to Sean, Theresa, Mark, Freddy, or Lorena: 1. I will always follow directions given to me by SCAR staff and senior volunteers.2. I will never enter the dog kennel area.3. I will treat all animals with respect and kindness.4. I will ask SCAR staff or senior volunteers before removing an animal from a cage. 5. When walking dogs:

I will never leave a dog unattended.I will never take a dog outside of a 3 block radius from SCAR without

permission.I will never take an animal off leash.I will never feed anything to an animal I am with.I will never treat an animal disrespectfully.I will always clean up after the animal I am walking.

6. I will utilize my volunteer time at SCAR for the betterment of SCAR, myself, and my community.

Waiver: I have read my child's responses to the above questions and attest to the truthfulness of the content. As a parent or guardian, I understand that my son/daughter volunteers at his/her own risk. I hereby grant permission for him/her toperform volunteer work for Sean Casey Animal Rescue. On behalf of myself and my son, daughter, or ward, I hereby hold harmless Sean Casey Animal Rescue, and its employees and volunteers, from any claims which could in any way be associated with his or her services with Sean Casey Animal Rescue, including but not limited to animal bites, accidents or injuries.

Release: I understand that public relations are an important part of volunteering at Sean Casey Animal Rescue and I consent to allow Sean Casey Animal Rescue to use any photographs taken of my son, daughter or ward for use in public relations.
