General Program Questions

Well-being Rewards 1. What are the rewards?

Benefits-eligible employees and their medical-enrolled spouses will be rewarded with support to improve your total well-being and build a healthier relationship with yourself, your colleagues, your family and your patients. Other rewards you can earn may include:

up to $780 or an equivalent reward at the discretion of Allina Health in 2020 (see eligibility) up to $150 in Amazon gift cards and entry into drawings for cash and other prizes.

Your Well-being site is personalized to you and displays the activities and rewards for which you are eligible. Sign into allinahealth.org/befit to see which of the above rewards you can earn this year. Not all participants are eligible for the same rewards or activities and the Well-being site experience may also differ based on if you are an employee or a spouse participant.

2. Are rewards taxable?Yes. All rewards are considered taxable income due to IRS requirements. Employees will be taxed on the value of the rewards they receive as well as any rewards that are received by an eligible spouse.

3. What rate are the value of my rewards taxed at?The value of the gift cards are taxed at the supplemental income tax rate of approximately 38.9%.

4. When will I be taxed on the value of my gift cards?

The Well-being Program is all about you and offers a personalized experience that supports you to improve your total well-being and build a healthier relationship with yourself, your colleagues and your family.

Employees will be taxed on the value of the gift cards they earn and those earned by their spouse, within two weeks of earning the gift card. Employees will be taxed when the gift card is earned, not redeemed.

5. If I earn a gift card, but do not redeem the code, will I be taxed?Yes, employees will be taxed on the value of the gift card as soon as they earn the reward, regardless of whether or not the gift code is redeemed. IRS Publication #538 indicates income which is constructively received must be taxed.

6. Are all benefits-eligible employees eligible for the gift card rewards and entry into the cash prize drawings?Yes! All benefits-eligible employees AND their medical-enrolled spouses be eligible for rewards for taking actions to improve their well-being throughout the year. This means that union-represented employees are also eligible for valuable rewards.

7. Who is eligible to receive a premium credits or an equivalent reward at the discretion of Allina Health?You must be enrolled in an Allina Health medical plan to be eligible for premium credits or an equivalent reward at the discretion of Allina Health. However, if you are a union-represented employee, you and your enrolled spouse are only eligible for the premium credits or an equivalent reward at the discretion of Allina Health if your collective bargaining agreement follows the non-contract premium subsidies. Your Well-being site is personalized to you and displays the activities and rewards for which you are eligible. Sign in to see which rewards you can earn.

8. How will I earn and receive my Amazon gift code rewards?To be eligible for any Amazon gift code rewards, benefits eligible employees and their spouses enrolled in an Allina Health medical plan must complete the Well-being Assessment in level one and two, which earns you 20,000 points. (Sign on to allinahealth.org/befit and click My Points to understand the levels and what you must do to complete each one).

Once you reach 30,000 points, you complete level three and will be emailed a link to redeem a $50 gift code reward from Amazon. Once you reach 40,000 points, you complete level four and will be emailed a link to redeem a $100 gift code reward from Amazon. The rewards must be claimed before Dec. 6, 2019 or they expire.

Contract employees and their spouses enrolled in an Allina Health medical plan will earn a $25 gift code reward from Amazon once you reach 10,000 points. Once you reach 20,000 points, you earn another $25 gift code reward from Amazon. You will earn a $50 and a $100 gift code reward from Amazon, for passing Level 3 (30,000 points) and Level 4 (40,000 points), respectively.

9. The site shows that I’ve earned 30,000 points, but I have not received my Amazon gift code. Why?Please go to the My Points section of the web site to confirm you have completed the required online Well-being assessment in level one. While you can earn points before you complete level one, gift codes will not be awarded until the requirements of level one are met. If you have completed all of the requirements, you would see a link to redeem your gift code in the My Points area of the site.

10. I confirmed I’ve completed all of the requirements for the rewards, but never received my Amazon gift code.If you have completed the required Well-being Assessment and earned enough points to qualify for an Amazon gift code, check your junk mail/spam folder for an email with your gift card code. It is important you provide an accurate email address to ensure you receive rewards and other timely information. To ensure that the email containing your Amazon gift code is not blocked or sent to your junk/spam folder, please add [email protected] and amazon.com to your safe senders list.

You can also access a link to any earned and not-yet-claimed Amazon gift code reward on the My Points section of your Well-being site.

11. Do I need to complete the Well-being screening to be eligible for the gift code and cash drawing rewards?No, Well-being Screenings are optional. The online well-being assessment is the only required activity to eligible to begin earning rewards. Participants who are newly benefit eligible as of May 2, 2019 or later will also be required to read a tile containing important program information.

12. What do I need to do to earn the maximum $780 in premium credits or equivalent reward at the discretion of Allina Health and $150 in gift code rewards by Amazon?You need to join and complete activities throughout the year to reach at least 40,000 points and complete the required online Well-being Assessment by Nov. 30 to earn $780 in premium credits or an equivalent reward at the discretion of Allina Health and $150 in gift code rewards by Amazon. Key dates are listed below:

December 13, 2018: Program launches

May – July: A limited number of onsite screenings will be offered at select locations will be offered.

Nov. 28: Complete all points-based challenges and activities to earn rewards.

13. How will I know when I have done everything required to earn my premium credits or an equivalent reward at the discretion of Allina Health?To confirm you’ve done everything required to earn your premium credits or an equivalent reward at the discretion of Allina Health, go to the My Points section on your personalized well-being site, then select the Completed tab on the right in the middle of the page. Look to see if you have passed Level Two and reached at least 20,000 points.

14. What is the deadline for earning points?You have until Nov. 28, 2019 to log all points-based activities for the year. You will not be able to log points after this date.

15. What does the change in language around premium credits mean? Allina Health is evaluating different ways to incent participation in the Well-being Program. The language change provides some flexibility in designing a well-being program for 2020 and beyond.

These measures were selected by Allina Health because improvements you make within your lifestyle, including addressing diet, exercise, sleep and stress, can make a big difference in your results – and your overall health.

19. How do my screening results determine my rewards?Screenings are optional. Participants who choose to complete a screening can earn points for completing the screening itself and for meeting the Healthy Outcome or low risk measures or 5% improvement over 2018 Well-being screening results. No points are earned for screening results that do not meet the low risk measures.

20. Do I get credit for progress if I improve?Yes! In response to your feedback, if your 2019 result improves by 5% or more compared to your 2018 screening result, you will earn the points for the screening measure, regardless if you achieve the low risk/healthy outcome measure. This benefits those whose hard work shows big results, but does not result in them meeting the low risk measure.

21. When do I get my Well-being Screening results?You will receive a copy of your BMI, waist circumference and blood pressure results during your screening. Cotinine results may not be available immediately. All results will be posted to your personalized Well-being site as early as two business days following your screening; however, assessments done by a provider may take up to two weeks to be posted.

You are responsible for confirming that results from a visit to your provider are posted by no later than November 28, 2019. Please follow up with your clinic if you do not see them posted within a week of your appointment.

22. Where do I review my results on my Well-being site?For participants who are eligible for premium credits or an equivalent reward at the discretion of Allina Health:Click on the Healthy Outcomes Bonus Points box on the right side of your Well-being site. Each measure and your results will be displayed.

Participants who are not eligible for premium credits or an equivalent reward at the discretion of Allina Health:Click on the “My Results” tab, then click on Health Risk and look for the results that are marked as verified. In addition to your screening results, the site displays results that are entered by you during your completion of the well-being assessment. The items you enter then are identified as self-reported. Only the verified results for Body Mass Index (BMI), waist circumference, blood pressure and cotinine/nicotine count toward the calculation of your premium credits or an equivalent reward at the discretion of Allina Health.

23. Can I have my doctor submit my screening results?You can see any provider to complete your screening. You must print the Well-being Clinic Screening Form and bring it to your appointment with you. Allina Health Clinic staff will complete and submit the form for you as part of your visit. If you go to another provider, you are responsible for faxing the form. Participants are responsible to check their Well-being site to confirm the results have been posted within two weeks of their appointment. All results must be posted by Nov. 28 to receive credit.

If you had an appointment with your provider, but the clinic did not collect all of the required data (BMI, waist circumference, blood pressure and cotinine/nicotine if required), you must schedule another appointment. Clinics cannot accommodate walk-ins or well-being screening-only appointments. The data must be collected as part of an exam.

24. Can I complete my well-being screening with a non-Allina Health Provider?Yes, you can see any provider of your choice. If you have questions about whether your provider can complete the well-being screening, call them prior to scheduling an appointment to confirm they are equipped to complete the screening and Clinic Screening Form. All component of the screening must be completed to be processed.

25. Can participants schedule a nurse-only or lab-only visit to accomplish their screening with a primary care provider?No, there is not an option for a nurse-only or lab-only well-being screening appointment. The Clinic Screening Form option is meant to be completed as part of a scheduled visit (annual exam/physical or office visit) with a provider.

26. Which physician specialty providers can complete the Clinic Screening Form?Participants can see a variety of providers for an annual exam/physical or office visit (i.e. OBGYN, endocrinologist, allergist, rheumatologist or some Penny George Institute for Health and Healing practitioners). If you have questions about whether your Allina Health provider can complete the screening, call them prior to scheduling an appointment to confirm they are equipped to complete the screening and Clinic Screening Form.

27. Why is Body Mass Index (BMI) being used in place of other metrics, such as body fat percentage? In a typical check-up, your health care provider will review your BMI to see if you are at risk for diseases linked to obesity. It is the most common tool used to screen for diseases and disease outcomes associated with being overweight/obese and having high body fat. These diseases include diabetes, high blood pressure, heart disease, stroke, arthritis, respiratory disorders, high cholesterol and certain cancers.

Measuring BMI can be done at any time and therefore allows you to better gauge your progress throughout the year. Even a small weight loss (just 5 percent of your current weight) will help you lower your risks.

28. How were the waist circumference measurements established?The waist circumference measurement guidelines align with those established by the National Institute of Health. Waist circumference, along with BMI, helps screen for health risks associated with being overweight or obese. Individuals with more fat in the abdominal area tend to be at a greater risk for heart disease and diabetes.

29. Why are my screening results rounded up or down?At this time, the site is not able to process anything but whole numbers. For the purposes of calculating your screening results, your weight and waist circumference are rounded down to the nearest pound/inch and your height will be rounded up to the nearest inch. This means your measures will be calculated in your favor. You can choose to document more precise measurements for your own knowledge and tracking of year-over-year results if you prefer.

30. Do I need to complete cotinine/nicotine testing during my screening?If you had negative cotinine/nicotine result in the past, you do not need to repeat the test in 2019. You will automatically earn the points for that metric when you complete a screening. Your personalized allinahealth.org/befit site will indicate if this test is required for you. If you see a tile called “Time for a Cotinine Screening,” that means the test is required. Please check for this tile before your screening. If you do not see the tile, you do not need to complete this test.

31. Can I complete multiple screenings to try to achieve better results?No. You may only complete one onsite screening event or one visit with a provider Nov. 3, 2018 through Nov. 1, 2019 to complete the Clinic Screening Form.

32. Will my results be available in Excellian (Allina Health’s electronic medical record system) or MyChart?Your results will be available in Excellian and MyChart only if you complete the provider form and your visit and labs are performed at an Allina Health site. If you choose to go through the onsite screenings, your results will not be displayed in Excellian or MyChart. You can view and print your results by logging in to your Well-being site.

33. I’m concerned about confidentiality; will my health information be protected?The Well-being program is compliant with HIPAA, including HIPAA’s privacy and security regulations. Compliance is

monitored through both periodic compliance reviews by the Limeade, Aduro and Penny George Institute for Health and Healing management teams. Your biometrics and health goals are confidential and are not shared with Allina Health. Allina Health only receives information on your reward level so that your premium credits or an equivalent reward at the discretion of Allina Health (if eligible) can be processed, all rewards earned by employees/enrolled spouses can be taxed appropriately, and de-identified health questionnaire and biometric data for program planning purposes.

34. What if I cannot participate due to medical reasons or I have a medical condition or medication that negatively impacts my results?If, due to a medical condition, it is unreasonably difficult or medically inadvisable for you to participate in all or a portion of the screening and other optional activities, you must provide a waiver from your provider by Nov. 1, 2019. Call 1-855-291-1459 for details.

32. I’m pregnant or recently had a baby. Do I still need to participate?Yes, call 1-855-291-1459 to receive a waiver form for the screening. Allina Health will waive waist circumference or BMI measurements, but you may still need to provide blood pressure and cotinine results (depending on our individual situation and your doctor’s discretion) and should complete a well-being screening by attending an onsite screening event or by completing a Clinic Screening form with a provider. Both your screening results and your waiver form need to be received in order to process your waiver request. You also have the opportunity to participate in your choice of activities to earn points toward rewards.

35. I’m on medical leave or about to go on medical leave. What are my options?Call 1-855-291-1459 for information about your options.

36. How do I know if my waiver/appeal has been processed and approved?Look for the “Your Appeal Has Been Processed” tile under the “Other things to do” section of your well-being personalized site to confirm your appeal was received and processed. You will earn points for the measures your appeal/waiver were approved for.

If you do not experience the above on your Well-being site, your form was either processed but not approved or not received. Please contact [email protected] for more details.

37. What if I have a concern with my experience or the results I achieved?Call the Well-being program support line at 1-855-291-1459.

38. What if I missed the deadline to complete challenges and track points on the Well-being site? Call 1-855-291-1459 to request a Well-being Program Appeal Form. Special considerations will be reviewed by the Allina Health Well-being Appeal Committee. Determinations are final. Individuals will be notified via email of the decision within three weeks of submission.

Creating your personal account on the Well-being site39. Who is Limeade?

Limeade is the vendor that powers the Well-being site and supports employees who may have questions about their site experience, rewards, eligibility or the screenings.

40. How do I activate my account on the Well-being site?Go to allinahealth.org/befit and click on the “Get Started” tab in the middle of the page. You will need your employee ID number (your six digit number). Your medical-enrolled spouse should use your employee ID plus the letter “s” to activate their confidential account. Remember to choose a unique password to sign on to this site – employees should not use the same password they use to log in to the Allina Health network/Excellian. You will also need to provide a valid email address to ensure that you receive updates and links to your gift card rewards when earned.

41. What email address should I use?Employees are asked to use their Allina Health email address. Spouses should enter an email account they use frequently as messages about reward redemptions and deadline reminders will be sent to the email they enter. NOTE: Spouses and employees must have separate email addresses for registration.

42. Who should I contact if I am unable to log in, or if I have any other technical questions or problems with my account?Call the Well-being Program support line at 1-855-291-1459.

43. Can I choose any username and photo I want? Yes, as long as they are appropriate and not offensive. Remember that your username may be visible to others if you comment on activities, your device activity posts to challenges or if you manually track activities. Usernames are also visible on leaderboards for participants in the top 20 places of a challenge.

You will be asked to set your username when you create your account – and you can choose an icon from a library or upload a photo of your choice by clicking on the settings option located in the upper right of your homepage. You are able to change your username at any time.

44. How do I change my username/password or upload my photo?Go to the settings tab located at the upper right.

45. How do I connect my device?More than 80 devices and apps can connect to your Well-being site, including Fitbit, Nike, Garmin Jawbone and Map My Fitness. To connect a device or view the devices that can be connected, log in to your personalized Well-being site at allinahealth.org/befit and click on the “Settings” link in the upper right hand corner and then to “Apps & Devices”.

Connecting a device makes it easier for you to track your steps or other progress in challenges and other activities. You will also be encouraged to connect your device/app when you join challenges or activities that are enhanced by having this connection in place.

46. Can I connect my Apple Watch?The device cannot directly be connected but there is a way to connected through an app. See tip sheet for directions.

47. Is there a Limeade app available? Yes! Install go to your app store and search for the Limeade app. Once the app is installed your will be prompted for a code, enter “AllinaHealth”.

How to use the Well-being site48. Why does the well-being assessment ask me questions that seem unrelated to my physical health?

Being well goes beyond being physically healthy. That’s why Allina Health expanded the Well-being program to invest in your total well-being. Your mental, physical, financial and relationship health and the satisfaction you get from your day-to-day tasks play a significant role in how you feel. More importantly, the way in which these elements interact can contribute in positive and negative ways to your overall health.

Based on your responses to the well-being assessment and your location, the site customizes which tools and resources it displays to help support you in your specific areas of strength and opportunity. As a reminder, all of your responses to the assessment are confidential and Allina Health does not receive information about your individual responses.

49. I want to invite people to be on a team with me for a team challenge. How do I do that?Enter their email address or their user name (if you know it). Spouses will need to be registered and you need to enter the email they used to register or the user name they selected to invite them to participate. They will receive an invitation in the email account they entered when registering for their Well-being site.

50. Can I keep my activities private?Some of the activities allow you to engage socially with other participants. When you register for a challenge or activity, you always have the option to keep your participation private by sliding the button here to the locked icon. Usernames are always visible on leaderboards for participants in the top 20 places of a challenge.

51. Why doesn’t the Well-being site always highlight Allina Health resources?Allina Health has customized all of the links that are viewed only by Allina Health employees and their medical-enrolled spouses. In addition to the content we can customize, the web site automatically displays core content the vendor, Limeade, provides to the million+ other users of their well-being program. This core content is not customizable and may link to national health care sources, including WebMD and Mayo Clinic.

As we continue developing our relationship with Limeade, we will explore ways in which Allina Health participants will see more great resources from allinahealth.org and Allina Health experts.

52. How can I tell if an activity is worth points?The points value will display on the tile. If the tile does not display a points value, it is not a points activity.

53. How come some activities are worth more points than others?Points were assigned based on the effort associated with the activities and whether or not the activity completion can be verified.

54. Are the challenges on the honor system?Allina Health trusts its employees and their spouses to be honest and truthful about what they submit through the Well-being Program. Most challenges are on the honor system. A few rely upon reporting data and will automatically give you credit for points once reports are confidentially submitted to the vendor. Examples include certain volunteering efforts, preventive care exams, etc.

55. Will Allina Health audit if I have completed an activity that is self-reported?Allina Health may require participants to show documentation that they have completed select self-reported activities. Participants who falsely attest to completing any activity will be disqualified from earning any rewards in 2019 and 2020.

Overview:The best way to determine which rewards you are eligible to earn is to sign in to allinahealth.org/befit.

All benefits-eligible employees and their spouses enrolled in an Allina Health medical plan are eligible for Amazon gift card rewards and cash prize drawings.

You and your spouse must be enrolled in an Allina Health medical plan to be eligible for premium credits or equivalent rewards at the discretion of Allina Health. If I enroll in benefits mid-year, do I need to complete the Well-being program to earn 2019 premium credits or an equivalent reward at the discretion of Allina Health?If you, and your spouse (if applicable), are enrolled in an Allina Health medical plan for 2019 with a coverage effective date of May. 1, 2019 or earlier, you will need to complete level one and two of the Well-being program to receive the highest 2020 premium credits or an equivalent reward at the discretion of Allina Health. Sign in to allinahealth.org/befit to determine what steps are required to complete level one.

If you or your spouse enrolls in an Allina Health medical plan for 2019 with a coverage effective date of May. 2, 2019, or later, you will default to full premium credits or an equivalent reward at the discretion of Allina Health for the remainder of 2019 and for 2020. This includes adding coverage for yourself or a spouse during benefits enrollment for the 2019 plan year.

59. If I enroll in an employee health plan with a coverage effective date on or after May. 2, 2019, can I still participate in the Well-being program?Yes, if you are enrolled in an Allina Health medical plan with a coverage effective date of May 2, 2019 or later, you can still participate in the Well-being program. While you will default to full premium credits or an equivalent reward at the discretion of Allina Health for the remainder of 2019 and for 2019, you can still earn the gift card and prize drawing rewards in 2019 by completing challenges and activities offered through the program.

All benefits-eligible employees and their spouses enrolled in an Allina Health medical plan are eligible for Amazon gift card rewards and cash prize drawings.

You and your spouse must be enrolled in an Allina Health medical plan to be eligible for premium credits or equivalent rewards at the discretion of Allina Health. However, if you are a union-represented employee, you and your enrolled spouse are only eligible for the premium credits or equivalent rewards at the discretion of Allina Health if your collective bargaining agreement follows the non-contract premium subsidies.

All participants must complete the online well-being assessment in order to qualify for any rewards. If you change jobs and/or your medical plan eligibility or enrollment changes, your rewards eligibility may

change. If you terminate employment with Allina Health, you will not be eligible for any rewards that have not already

been claimed or paid to you. The Well-being program, program rewards and reward structure are subject to change at any time at the
