The Foreign Records and Student Visas Departments assist with immediateinterpretation of foreign documents for recommended placements into our school system.
Foreign Records Grade Placement
➢ Foreign school records are used for recommended grade placementand verification of credits earned.
➢ Foreign school records are interpreted for new placement and toevaluate transcripts.
➢ Original records are required to grant credits. Submit a fullycompleted Foreign Records HEAT Cover Sheet (FM 6957) with thestudent’s records in PDF format.
➢ In the instances that student foreign records are not available at the time of registration, you may utilize the Temporary Student Placement Form (FM-6008). The student will be placed according to the Student Progression Plan guidelines.
Foreign School Records
➢If a Middle or Senior High school student registers after the firstgrading period has ended, the Foreign Records Department willmatch equivalent grades to the student’s current schedule.
➢Senior High School foreign grades will be converted into the M-DCPS grading scale. Preference is given first to seniors then tojuniors, and sophomore students.
➢Call the Foreign Records Department if a Grade Point Average is needed right away!

CASE # 2 MIDDLE SCHOOL
Henry and his parents presented themselves at Island Middle School
on March 5th, 2017. The parents provided transcripts indicating that
Henry had completed his 6o año in December 2016 in Brazil.
What grade should Henry be registered in for the remaining months of the
school year?
a) Register in 6th grade and promote in May with the matching of grades.
b) Register in 7th grade and retain in May with no matching of grades.
c) Register in 7th grade and promote in May with no matching of grades.

CASE # 3 SENIOR HIGH SCHOOL
On January 12th, 2018, while registering at Southern Senior High
School, Maria provided transcripts indicating completion of her 10th
year (Decimo Año) during December 2017 from Ecuador.

a) Register in 11th grade with annual courses.

Comprehensive Health ServicesImmunizations Requirements for 2018-2019
KG and 7th Grade Students
The Florida Department of Health in Miami-Dade County and M-DCPS
adheres to the Centers for Disease Control’s immunization schedule.
• Effective for the 2018-2019 school year
• ALL KG students must have the 2nd MMR vaccine
• ALL 7th grade students are required to have the Tdap booster
• ALL 10th grade students are required to have 2 Varicella vaccines
• Students displaced by Hurricane Harvey, Irma or Maria
• Families were granted a 30-day exemption for producing the required health records
• As of 1/19/2018 ALL students displaced by the hurricanes MUST have the proper health records including Florida Certificate of Immunization-DH 680

• All VPK Programs (Fee-Supported, Title I, Pre-K SPED Inclusion and Head Start) must collect COEs for all four year olds.

• Parents must obtain the COE by going online at www.vpkhelp.org. For further information call ELC at (305) 646-7220.
• COE number may contain 6 to 15 digits and letters.
• When entering the COE number into DSIS, enter it EXACTLY as it appears in Box#2 on the COE document. (ie. VPK762123-2018)
• Pay close attention to the letter “O” and the number zero “0”.

• In cases of consecutive student non-attendance of 15 days or more, document the Student Case Management System in DSIS with Home Visits (H2) attempts and any additional documented evidence of attempts to reach the parent.
• If home is vacant or family has moved, submit a copy of the Request Withdrawal of Student W-22 Form (FM-7633) signed by Principal to FASCO via HEAT Ticket.
• FASCO will review and withdraw the student utilizing the W22-Whereabouts Unknown, if appropriate.

• Principals are required to review on a daily basis W02 Withdrawals.
• When the registering parent request a withdrawal from your school (with proper documentation), parent must sign withdrawal form printed from PF15 Screen and the student is withdrawn immediately.
• When a student withdraws to the receiving school, the student will show-up in the receiving school’s attendance roster the next day.
