A management system is the framework of processes and procedures used to ensure that an organization can fulfill all tasks required to achieve its objectives.

It prescribes systematic control of activities to ensure that the needs and expectations of customers are met. It is designed and intended to apply to virtually any process for Product Service, Environment or Health and Safety anywhere in the world.

CERTIFICATION SERVICESVQMI is an independent Certification Body under the VELOSI Group for providing system certification services. VQMI is accredited by the Dutch Accreditation Council RvA (Raad Voor Accreditatie) Holland, in accordance with ISO 17021 and international guidelines for certification body accreditation. RvA’s accreditation of VQMI signifies its confidence in the system and that it complies with international accreditation requirements.

VQMI’s audit process includes review of the manuals and procedures and on-site audits of the applicant organization to ensure that it is complying with the requirements of the relevant system standards. The above process is completed in two stages as per the requirements of ISO 17021.

After a successful audit, a Certificate of Compliance is issued and a register of Certified Companies is maintained which can be accessed on the VELOSI website. Surveillance audits (usually once a year) are also conducted by VQMI to ensure that the system is effective on a continual basis.

QUALITY MANAGEMENT SYSTEMS - ISO 9001:2008A Quality Management System (QMS) integrates the various internal processes within the organization with the goal of providing a process approach. HAZard and OPerability (HAZOP) analysis is a structured technique to perform a systematic study of a process using guide words to discover how deviations from the design intent can occur in equipment, actions or materials, and whether the consequences of these deviations can result in a hazard.

Why HAZOP?Hazard Identification is a key phase in the safety management of a facility. Only identified hazards can be analyzed, assessed, managed, and mitigated if warranted. However, the usefulness of the hazard identification process is now expected to extend beyond the initial identification phase; for example, to provide inputs to management of change, written procedures, and incident investigation.

HAZOP MethodologyVELOSI uses a number of techniques assisted by software tools and models for safety and risk management of process systems. Risk RankingVELOSI uses a 5x5 risk matrix for risk ranking of hazards. The hazard level consists of one number and one letter. The letter of the hazard level represents the Frequency of Occurrence. The number of the hazard level represents the severity level.

QRA’s provide numerical estimates allowing clients to understand risk exposure to people, business, the environment, markets or other areas of interest. QRA studies account for potential releases of hazardous material, their consequences (e.g., fire, toxic, explosion, etc.) and estimated frequency of occurrence. Risk Based Inspection (RBI) is a systematic approach to create an accurate, well targeted inspection strategybased on risk. The inspection plans are developed tomitigate the specific failure mechanisms identified. It isapplicable to offshore & onshore installations such asfixed or mobile units as well as FPSOs and pipelines.

IssuesThe oil & gas, process industry handles a wide range of flammable andtoxic materials which are potentially hazardous. To reduce the hazards to an acceptable level, the authorities through legislation, have imposedrequirements on the management of plants and facilities, requiring it todemonstrate that it has identified the existing hazards and undertakenmandatory inspection and mitigation measures to prevent accidents. Safety Integrity Level (SIL) is the discrete level for specifying the integrity requirements of safety functions to be allocated to the “Electrical/Electronic/Programmable Electronic Systems” safety related systems where SIL 4 has the highest level of safety integrity and SIL 1 the lowest.

Why SIL?Process plants are operated with many sophisticated process control systems. These systems are required for safe and reliable operation of the plant and address the safety functions such as process sectioning, fire & gas detection, process protection and blow down. These safety functions are often designed combining various E/E/PES technologies and other external risk-reducing devices. In addition, computer and software intensive systems are increasingly being used as part of the system. A malfunction of these systems can lead to loss of life, damage to assets and impact to the environment.

MethodologyVELOSI uses a number of techniques assisted by software tools and models to manage safety critical equipment. These are organized according to the Life Cycle model recommended for the safety critical equipment by IEC 61508. SILS versus reliabilityWhile the main focus of the SIL ratings is the interpretation of a process’ inherent safety, an important by-product of the statistics used in calculating SIL ratings is the statement of a product’s reliability. To determine if a product can be used in a given SIL environment, it must be shown to ‘BE AVAILABLE’ to perform its designated task at some predetermined rate. In other words, how likely is it that the device in question will be up and functioning when needed to perform its assigned task?

Reliability numbers: What do they mean?It seems that every organization has its own special way of characterizing reliability. However, there are a few standards in the world of reliability datum, as indicated below:

• Probability of Failure on Demand (PFD) The Probability to Fail on Demand is a statistical measurement

of how likely it is that a process, system, or device will be operating and ready to serve the function for which it is intended.

• Safe Failure Fraction (SFF) The maximum rated SIL environment in which the device(s)

can be used.

• MTBF A statistical representation of the likelihood of a component,

device, or system to fail. The value is expressed as a period of time (i.e. 14.7 years).

• MTTR Mean Time To Repair is the average time to repair a system, or

component, which has failed. This value is highly dependent upon the circumstances of the system’s operation.

Field and Desk ExpeditingToday, expediting is an integral part of the procurement process. In the past, it was considered a corrective rather than preventive function. However, as both the schedule and the pricing of projects become increasingly critical, expediting should be incorporated into the project planning at the onset to ensure timely delivery.

Expediting is a continuous process, of ensuring that final delivery to the job site is achieved within the shortest possible timeframe. An expediting service should be implemented independently of inspection activities and be carried out by experienced field expeditors.

Depending on client requirements, expediting services may apply at different stages of a project such as fabrication and assembly, rectification work, material deficit, failure on test or inspection, shipment and packing, and submission of documentation.

Technical Capability AuditsQuality Assurance Engineers carry out Pre-Contract Award Surveys, Supplier Evaluations, Third Party Quality Assurance Audits and Vendor Surveillance.

The quality and quantity inspection may be for verifying that the goods conform to contractual specifications and that the value and Customs tariff code for Customs purposes, Packing review, and Loading supervision.

All clients / projects are provided with a dedicated office-based co-ordinator who is responsible for managing all call-offs from that client in a timely, precise and efficient manner. To achieve this, VELOSI has developed a complete computerised management control system known as AMIS.

AMIS is a software program designed in-house as a complete Quality Management package for vendor inspection and the expedition of engineered material and equipment. Electro Magnetic Array testing (EMA) EMA is an electromagnetic technique for detecting and sizing surface breaking defects in metals, and is particularly suited for inspecting painted and/or welded structures.

Computed radiographyCR uses an imaging plate (IP) to create the image, which contains critical and precise details far greater than film. The digital image can then be viewed and enhanced using software that has functions very similar to other conventional digital image-processing software, such as contrast, brightness, filtration and zoom. CR is capable of measuring thickness and lengths of parts and defects accurately electronically.

MFL tank floor scanning:The technique is designed to detect and size under floor corrosion for above ground storage tanks. Incotest Incotest is a unique corrosion survey method that allows ferrous pipes and vessels to be surveyed through thermal insulation without disturbing the insulation or coating.

Phased ArrayUltrasonic phased array systems can potentially be employed in almost any test where conventional ultrasonic flaw detectors have traditionally been used. Weld inspection and crack detection are the most important applications. Phased array produces accurate, detailed cross-sectional pictures of internal structures at fast inspection speeds. The benefits of phased array technology over conventional UT come from its ability to use multiple elements and electronic time delays to create beams that can be steered, scanned, swept, and focused electronically for fast inspection, full data storage, and multiple angle inspections.

Tube Inspection ServicesThere is no single non-destructive testing (NDT) technique that can be applied to inspect all of the tube materials. Selection of an NDT technique depends on the tube material and also on defect types expected.

The Tube Inspection techniques we offer for inspection include Eddy Current Testing (ECT), Remote Field Testing (RFT), ultrasonic IRIS, and Magnetic Flux Leakage (MFL) as well as laser optics. Each of the NDT techniques has its advantages and limitations. For example, conventional eddy current is very sensitive to pits and cracks but it is mostly limited to non-ferromagnetic materials. IRIS is accurate in measuring wall thickness, but it will miss small defects such as pinholes and cracks. Optical techniques are limited to ID defects. Proper selection of the NDT techniques is therefore a key to inspection of heat exchangers.

Electro Magnetic Acoustic Testing (EMAT)The big advantage of EMAT testing is the ability to produce accurate readings on scaled surfaces. This is because the probes induce ultrasound in the steel itself rather than by transmitting it via a coupling medium. In fact the probes will work at a small distance from the material surface. This allows the probe to function when the test surface is heavily corroded, has a large oxide scale build up or during high temperature operation. A major cost benefit of these probes is that expensive and time-consuming cleaning and surface preparation is no longer necessary.

Long Range UT Guided Wave inspectionLong-Range Guided Wave UT pipe screening system is used for testing long lengths of pipe to rapidly identify areas of internal/external corrosion and other defects. With minimal surface preparation and only removing a small section of insulation, the transducer collar placed around the pipe have a typical test range of up to 50 meters in each direction along the pipe.

Remote Visual Inspection Services (CCTV)Visual Inspection with the use of high tech video camera systems, of confined spaces or hazardous locations where human entry is impractical, physically impossible or unsafe. Remote visual inspection has a lot of applications and is ideal for preventive maintenance checks.

Thermography is a non contact and non destructive inspection technique that provides immediate results and cost savings to clients. Its applications are almost limitless for most industry types. It is easy to see a problem, but what is important is to interpret the results. In addition, the high capital cost of the equipment can make one-off requirements uneconomical. Using VELOSI is a cost effective way to ensure that the inspection is carried out by qualified and experienced personnel at a reasonable cost, using the most up-to-date equipment available.

“Nearly everything gets hot before it fails”.

Thermography is the use of infrared imaging technology and a measurement camera to ‘see’ and ‘measure’ thermal energy emitted from an object. Thermal, or infrared (IR) energy, is light that is not visible because its wavelength is too long to be detected by the human eye; it’s the part of the electromagnetic spectrum that we perceive as heat. Unlike visible light, in the IR world, everything with a temperature above absolute zero emits heat. Even very cold objects, like ice cubes, emit infrared.

The higher the object’s temperature, the greater the IR radiation emitted. IR thermography cameras produce ‘heat’ radiation images and precise non-contact temperature measurement capabilities.

High definition thermographic cameras have migrated into other fields as varied as engineering, medicine and archaeology. More recently, price reductions have helped fuel the adoption of IR viewing technology. Advanced optics and sophisticated software interfaces continue to enhance the versatility of this diagnostic tool. As the industry strives to improve manufacturing efficiencies, manage energy, improve product quality, and enhance worker safety, new applications for IR cameras continually emerge.

In modern applications, equipment failures are usually preceded by an abnormal temperature pattern, and this temperature rise can be easily detected by an IR thermographic inspection. The advantages are that it can reduce unplanned down-time, maintenance costs, and also improve production, equipment life and plant safety.

One advantage of this technique is the ability to detect faults in time without plant shutdown.

Temp measurementFinding a problem with an IR camera is sometimes not enough. In fact, an IR camera image alone without accurate temperature measurements reveals very little about the condition of an electrical connection or worn mechanical part. Many electrical targets are operating properly at temperatures that are significantly above ambient.

An IR image without measurement can be misleading because it may visually suggest a problem that does not exist. IR cameras that incorporate temperature measurement allow predictive maintenance professionals to make informed judgments about the operating condition of electrical and mechanical targets.

Temperature measurements can be compared with historical operating temperatures, or with IR readings of similar equipment at the same time, to determine if a significant temperature rise will compromise component reliability or plant safety.

Digital image storage on our equipment produces calibrated thermal images that contain over 78,000 independent temperature measurements that can be measured at any time with IR software products on standard PC platforms.

VELOSI’s accreditation for approval of procedures covers all types of permanent jointing to EN Standards. We also offer approval for other codes and processes such as brazing and various joining methods for plastics, such as solvent welding.

The process involves:

• Review and provisional approval of the procedure.• On site surveillance of the test piece production against Written

Procedures• Witnessing laboratory testing of the test pieces• Review of any Non-destructive or other Test Results• Approval of the Weld Procedure (and welder if appropriate)

The number of test pieces will be dependent on the procedure requirements. Many procedures cover a range of material types and sizes. Laboratory testing requirements are defined by the applicable code and vary from application to application.

Welder ApprovalOnce a procedure is approved, individual welders can then be assessed against the procedure, using the same process. The approval of a welder is usually valid for 2 years (3 for brazers) as long as the welder is regularly using the process defined in the procedure.

The prolongation of the Welder Approval is possible as long as determined criteria are met.

Material ServicesWith the EC Directives such as the Pressure Equipment Directive (PED), there is an increasing requirement that materials prove they meet the parameters in their chemical and mechanical properties. This has led to the authentication of material certificate requirements.

VELOSI offers surveillance at the mill to taking samples of material and subjecting them to a rigorous regime of laboratory testing.

Assessment and endorsement by VELOSI of the material certification meets the requirements of a 3.2 certificate as defined in EN 10204 ‘Metallic Products - Types of inspection document’. also It also covers the assessment of plastic materials soon to be incorporated in the above specification.

One result of the introduction of European Directives, such as the PED, has been a requirement for total material traceability. This means that all materials must be fully traceable and documented from the mill to the manufacture of the pressure equipment, or the repair of such equipment.

VELOSI is authorised to supervise and endorse material transfers and material certificates. In practice, material stockists can now issue material certificates for cut pieces with the same validity as the original certificate. Velosi will supervise and record the cutting of the piece, the transfer of the identification, heat or cast number and endorse a copy of the original certificate with an endorsement stamp.

In addition, if you have valuable material in stock that has lost traceability, VELOSI can authenticate it with mechanical and chemical testing at a recognized laboratory.A valid material can then be used in the manufacture of pressure equipment.

We carry out inspections in accordance to the local country statutory regulations like the The Lifting Operations and Lifting Equipment Regulations (LOLER) in UK. These Regulations usually apply to all premises and work situations that are subject to local Health & Safety laws.

The Regulations are primarily aimed at lifting equipment such as cranes, lifts, and hoists, but it also covers components such as chains, wire ropes, slings, hooks, shackles and eye bolts.

We will review your equipment specifications and advise you on the inspection requirements. If no local legislation applies, we can assist with assessing the equipment inspection requirements, including periodicity, by using best practice and international codes and standards.

VELOSI is able to carry out full equipment assessments. Once this is completed, an electronic management control system tracks and monitor each item for its inspection status before certification can be implemented. This is normally done using an electronic tagging system that can be tailored to a client’s need. Also, VELOSI can implement a colour-coded tagging system which is changed periodically in line with inspection regime so that equipment whose inspection and certification is out-of-date is easily identified and placed into quarantine until the inspection and certification has been completed. REGULATORY APPROVAL DOCUMENTS PROVIDED BY VELOSI:

Common Permits and LicencesRostekhnadzor (Gosgortekhnadzor in Kazakhstan) is the Federal Mining and Industrial Oversight Committee. These organizations are also known as RTN and GGTN. RTN and GGTN issue the permits required to operate industrial equipment in their respective countries. VELOSI is a licensed expert review body with authorization to prepare the Technical Expert Conclusions needed to obtain RTN and GGTN Permits to Use.

Technical PassportsTechnical Passports are a compilation of technical data related to specific pieces of equipment. The data is translated, formatted and compiled according to strict RTN / GGTN regulations. Equipment listed on RTN / GGTN Permits to Use must typically be supplied with Technical Passports.

Gosstandart Certificates of ConformityGosstandart (GOST) is the Federal Committee for Standardization, Metrology and Certification. GOST Certificates of Conformity provide proof that the specified items meet the country’s quality and safety standards. They must be presented during customs clearance for virtually all goods. VELOSI is an accredited GOST certification body authorized to issue Certificates of Conformity.

Explosion-Proof CertificatesGOST explosion-proof certificates are required for many types of electrical equipment used in hazardous areas.

Sanepidnadzor CertificatesSanepidnadzor is part of the Ministry of Health. Sanepidnadzor hygienic certificates are required for water treatment systems, oils, fluids, chemicals, building materials and other potentially hazardous materials.

Fire Safety CertificatesThe Ministry of Emergency Situations (MChS) is responsible for certifying the design of fire fighting and fire alarm systems as well as the equipment contained within them.

Metrology CertificatesRostekhregulirovaniye is the Federal Agency for Technical Regulation and Metrology (FATRM). FATRM is responsible for verifying the accuracy of measuring devices. Metrology certificates are required for devices used with emergency shutdown systems, alarms systems , environmental monitoring systems and devices used for custody transfer of assets such as oil and petrochemicals.

Cathodic Protection is used to control the corrosion of a wide variety of buried and submerged metallic structures. It is a cost-effective way to extend the life of a structure and to ensure integrity throughout its operating life.

Cathodic Protection can be defined as…“the control of electrolytic corrosion…by the application of direct current in such a way the structure to be protected is made to act as the cathode of an electrolytic cell”.

WHY Cathodic ProtectionCathodic protection provides effective corrosion control on steel structures and pipelines by creating a potential (voltage) gradientopposing the flow of ions away from the surface, thus preventing the anodic corrosion reaction. Cathodic protection may be used alone or in combination with an insulating coating. In such systems, the coating forms the first line of defense and where it is adherent and undamaged, the corrosive environment is kept away from the structure: where the coating is damaged, however, corrosion can occur and cathodic protection provides protection. It is worth nothing that cathodic protection can only be applied in the presence of an electrolyte – it is not an antidote for atmospheric corrosion.

Choosing a Cathodic Protection SystemTo achieve no corrosion at the cathode surface, it is important to ensure that protective current is passing onto all parts of the cathode surface. The current for full protection can vary considerably with environmental conditions - from 200mA/sqm in turbulent aerated seawater, to 1mA/sq-m for some onshore pipelines. Deciding which type of protective system to adopt therefore, depends on many factors - size and complexity of structure, nature of the electrolyte, availability of power supply, proximity of ‘foreign’ structures which may affect, or be affected by, the flow of current, etc. A detailed economic appraisal based on technical surveys is required.

External Corrosion Direct Assessment (ECDA) is a structured process to evaluate buried onshore ferrous pipeline integrity. The ECDA goal is to enhance safety by managing the risk of pipeline corrosion failures while minimizing the cost required for excavations and repairs. ECDA may also be used when more established methods such as in-line inspection (ILI) and pressure testing are not possible or practical.

Indirect Assessment – The purpose of indirect inspection is to identify the locations of coating faults, insufficient cathodic protection, electrical shorts, interference, geologic current shielding, and other anomalies along the pipeline. The technique also defines areas where corrosion may be occurring or has occurred. Typical pipeline survey techniques that may be used are:

• Close-interval, on/off potential surveys• DCVG surveys• Electromagnetic current attenuation surveys• Alternating current voltage gradient surveys

Direct Examination – Exposing the pipe for verification of the above-the-ground surveys results and physical inspection of pipe coating, pipe surface and soil electrolyte. Direct examination includes:

• Ranking and prioritizing indications identified during the indirect inspections

• Excavating to expose the pipe and collecting data where corrosion activity is most likely

• Measuring coating damage and corrosion defects• Performing a root-cause analysis• Evaluating the process

Post Assessment – Integration of the results gathered in tier 1, 2, & 3 for overall integrity assessment, validation of the ECDA process and determination of the interval period to re-do the ECDA process. Post-assessment can determine the overall effectiveness of direct assessment and define reassessment intervals. Reassessment intervals are based on remaining-life calculations. Various methods can be used to calculate the remaining life - from the corrosion growth rate, wall thickness, calculated failure pressure, yield pressure, maximum allowable operating pressure, and appropriate safety margin. Under some conditions, corrosion rates can be estimated by measuring linear polarization resistance or using coupons.

The PED Pressure Equipment Directive was adopted by the European Parliament and the European Council in May 1997, and from 29 May 2002 has been obligatory throughout the European Union, with penalties for non-compliance. The PED specifically covers equipment designed to contain fluid at a pressure greater than 0.5 bar gauge.

VELOSI Certification has been accredited as a Type A inspection body by The United Kingdom Accreditation Service (UKAS). This accreditation covers the 97/23/EC PED.

Conformance with the directive is complex as it affects quality control, manufacturing, procurement, inspection, etc. VELOSI’s experience and approach to implementation simplifies the process and lessens the burden on manufacturers. Whether you are a small or large business, or whether you produce one-off items or series production, VELOSI is able to give the same high level of service.

We recognise that the extent to which the PED affects each company is different and many factors influence costs. It is essential to understand how it can be kept to a minimum. PED compliance is achieved by satisfying the requirements of a Conformity Assessment Module dependent on the equipment risk category. For categories II and above, which require Notified Body involvement, there are two main routes - QA and non-QA. However, only some of the modules allow monitoring of the final assessment. Others require unit verification.

Although there are higher initial set-up costs for QA modules, ongoing costs can sometimes be lower. Knowing where the break-even point lies determines the best route for compliance.

Our experience with a wide range of companies has made us adept at helping clients determine the best compliance route.

The Pressure Equipment Directive (97/23/EC) was adopted by the European Parliament and the European Council in May 1997. It has initially come into force on 29 November 1999, and from 29 May 2002 the Pressure Equipment Directive became obligatory throughout the European Union.

The non prescriptive Directive arises from the European Community's Programme for the elimination of technical barriers to trade and is formulated under the ‘New Approach to TechnicalHarmonization and Standards’. Its purpose is to harmonise national laws of Member States regarding the design, manufacture, testing and conformity assessment & assemblies of pressure equipment. It therefore aims to ensure the free placing on the market and putting into service of the equipment concerned within the European Union and the European Economic Area.

The Directive provides for a flexible regulatory environment that does not impose any detailed technical solution. This approach allows European industry to develop new techniques thereby increasing international competitiveness.

The Directive concerns manufacturers of items such as vessels pressurized storage containers, heat exchangers, steam generators, boilers, industrial piping, safety devices and pressure accessories. Such pressure equipment is widely used in the process industries (oil & gas, chemical, pharmaceutical, plastics and rubber and the food and beverage), high temperature process industries (glass, paper and board), energy production and in the supply of utilities, heating, air conditioning and gas storage, and transportation.

Under the Community regime of the Directive, pressure equipment and assemblies above specified pressure and/or volume thresholds must:

• be safe• meet essential safety requirements covering design, manufacture and testing• satisfy appropriate conformity assessment procedures; and• carry the CE marking and other information.

Pressure equipment and assemblies below the specified pressure / volume thresholds must:

• be safe• be designed and manufactured according to sound engineering practice, and• bear specified markings (but not the CE marking).

As a Notified Body, VELOSI meets the requirements defined in Article 12 either for approval and monitoring of the manufacturers' quality assurance system or for direct product inspection. To determine how the Directive will apply to specific items of pressure equipment, a manufacturer needs to classify the equipment into one of four conformity assessment categories, or categories I to IV. Category I relates to the lowest, category IV to the highest, hazard category.

Before putting pressure equipment related to categories I to IV on the market, the equipment has to be subject to the appropriate conformity assessment procedures (modules). Depending on the category of the equipment, manufacturers will be given a choice of the ‘modules' summarized below. Manufacturers may choose either an assessment procedure based on product control or an imprementation of the PED within their systems.

Modules attributed to a higher hazard category may be used in lower categories. The modules for products in categories II, III and IV require the involvement of ‘Notified Bodies’ such as VELOSI, either in the approval and monitoring of the manufacturers' quality system, or in direct product inspection.

Onshore pipelinesDuring the construction of an onshore pipeline, VELOSI can provide the full range of quality control services such as welding and other NDE-type inspectors to monitor the actual jointing functions, coating inspectors to ensure that on-site coating of the welded joints are completed, experts in cathodic protection systems, terrain specialists for road, rail or river crossings, way-leave considerations such as agricultural and urban experts, and environmental engineers for rainforests or areas of scientific interest.

Clients can choose either a complete packaged management solution or any of the individual elements mentioned above. A packaged solution would include a Project Manager to oversee all quality aspects of the construction project. He would then, in conjunction with the client, assess the expertise required for each element.

Offshore PipelinesVELOSI has extensive experience in verification services on major offshore pipeline projects and supervision work on high specification, dynamically positioned pipeline installation vessels during deep-water construction. Our specialist personnel are fully conversant with S and J lay installation systems, automated welding techniques and advanced NDE techniques, such as automated ultrasonic testing.

As part of the project management, we can mobilize personnel to ensure that project staff are supported during the project. This support service is provided by our local offices at both the point of embarkation and point of arrival. All personnel are checked for medicals and visas and have emergency support, and other rotational leave assistance.
