Reactions 1453 - 25 May 2013 X Metoprolol/propafenone interaction Tiredness and dyspnoea in an elderly patient: case report A 66-year-old woman developed tiredness and dyspnoea due to an interaction between metoprolol and propafenone. The woman, who had comorbidities including ischemic heart disease without angina pectoris syndrome and chronic venous insufficiency, was receiving cardiovascular medication including metoprolol 200 mg/day [route not stated]. After admission with decompensated hypertension, she was prescribed propafenone 600mg daily [route not stated] as prophylaxis against atrial fibrillation. Later, amlodipine was added to her medications. Her hypertension improved; however, she reported increased tiredness and dyspnoea on exertion [durations of treatment to reaction onset not stated]. Investigations revealed a serum metoprolol concentration of 412.2 µg/L 3 hours after intake, and a metoprolol-α- hydroxymetoprolol metabolic ratio of 104.3. Metoprolol was reduced to 100mg daily, and the woman’s symptoms resolved. About 6 months later, she was readmitted with global cardiac failure with atrial fibrillation with slow ventricular response. Metoprolol was reduced to 12.5mg daily and propafenone was withdrawn. Two weeks later, she was haemodynamically stable and was discharged. Several days later, she increased her metoprolol to 100mg daily. At her next follow-up, investigations revealed a substantial decrease in her metoprolol-α-hydroxymetoprolol metabolic ratio following propafenone discontinuation.
