INTRODUCTION

The purpose of this document is to provide a standard set of instructions and to outline the

measures needed to ensure successful completion of the job within the target time frame, on to

budgeted cost and in accordance contract requirements. If any part or complete document

contradicts with existing client documents, the Conditions mentioned in the client document will

prevail.

4.0 SCOPE OF WORK

Scope of work includes repairing of doors by replacing components, removal of existing doors,

window glasses, reconstruction by use of new doors and window glasses. Repair and reconstruction

carried out in accordance with Client documents and manufacturer installation procedures.

5.0 SEQUENCE OF EXECUTION AT SITE

As far as practically possible, the sequence of operation shall be as per agreed steps. In case of any

changes from the agreed sequence or steps are required for the safe and easy execution, then

necessary prior approval from Client to be obtained.

6.0 TOOLS AND EQUIPMENT

Common tools required to carry out this project are: - ladders, drill machines, Grinders and tool box contain general tools like:

Hammer, Duct sealant gun, Measuring Tape, Nylon Rope, Glass catcher, Rivet gun, Spanner set, Screw Drivers, Spirit Level Indicators Plumb, Markers, Scissors, Grip Player Multi-meter, Wire cutter, Crimping tool, Insulation tester and other tools not referred here shall be mentioned under appropriate section of this document.

All electrical tools / equipment are subject to periodic servicing and stamps maintenance record on equipment. Maintenance log is controlled by Maintenance Department (Plant & Equipment Department). Random site inspection is conducted to ensure proper servicing of relevant equipment / tools.

All personnel are instructed to check tools prior to usage and send unsafe equipment if any, for servicing immediately

Toolbox talk is given at site and address above mentioned maintenance requirements to all operatives.

Valid test certificates / tag shall be available on all relevant tools / equipment. Valid calibration certificates shall be kept with HSE personnel and ensure availability of the same at site to ensure HSE compliance upon tools usage at site. Valid calibration certificates shall be accompanied with tested date and expiry date.

7.1 PROJECT COORDINATOR

Coordinator role is to ensure proper coordination between COMPANY Project Team and Client effectively. This is so essential that project is delivered on time with agreed quality standards.

He is acting as central contact point for Project team, COMPANY Operations Manager and site supervisors. He is also responsible to maintain progress at site as per project schedule and report to Project Manager.

Coordinates site activities to minimize - amount of night shift, extended work hours and number of personnel involved in such activities

Inform Client regarding if any planned overtime or night shift activity happened at site. He also has to look into risk factors in every jobs and ensure risk assessment have been maintained.

7.2 SUPERVISOR / FOREMAN

Supervisor is directly responsible for scheduling day-to-day activities on site. He ensures, work is executed as per project specifications and project quality requirements. Follow-up and updating construction schedule to project coordinator. Prepare material requisitions and follow-up for timely material delivery at site. He ensures welfare and safety of personnel working under him.

Supervisor is responsible for managing their work together with other discipline teams and supporting project coordinator to ensure that project is delivered on time with approved quality standards.

He is responsible for resolving any quality issues raised during QC site inspections and assists Quality Control department to ensure timely closeout of comments raised.

7.3 SUPPORT SERVICES – FACILITATOR (CLIENT)

All Hot works, Painting & Coating works, Scaffolding, Craenage, Rigging, waste skip and accessibility, Ventilation / Heating / Lighting at construction facility to be arranged by Client. All site utilities are inspected, tested and maintained periodically by Client.

7.3.1 WASTE SKIP, RIGGING AND CRANE ASSISTANCE

All material delivered to site by COMPANY are packed and labeled for easiness of identification, handling and transportation. Material transference at worksite is with careenage and rigging assistance by yard. On-board material transference from crane dropping point to work location is with rigging assistance by Client / Yard.

Rigging and mobilizing of heavy items at site to be done by on-board riggers. COMPANY supervisor will coordinate if required.

Waste skips are arranged by Client at a nearest location to dispose construction waste. Client shall

dispose waste from skip as necessary by regulatory requirement.

7.4 MOBILIZATION & PRE-CONSTRUCTION PROCEDURES

The appropriate date for the actual start of first phase construction operations at the site will be

determined by the Project Coordinator, after consultation with the Client.

Accommodation for personnel and other containers for tools & equipment storage at site will be

planned prior to the start of Construction by project Coordinator as per agreement with Platform

in-charge.

Consideration will be given to the location of storage container, tool rooms, change rooms and plan

rest rooms to minimize walking by employees.

Care will be taken to locate office and material storage containers to minimize liability to damage

from fires or explosions which might occur from other works nearby.

Construction is controlled by the Project Coordinator who will delegate the administration of the

different construction activities to Supervisor.

7.4.1 MOBILIZATION OF EQUIPMENT

Equipment required for the job execution will be identified by concerned department and intimated

to COMPANY Plant & Equipment department.

Plant & Equipment department will make sure necessary checks and tests are done as per COMPANY

procedures and Client / yard requirements prior to mobilizing to construction site.

Tagging / labelling to indicate validation status will be provided on all portable equipment, lifting

equipment & accessories, welding machines and measuring instruments.

7.4.2 GENERAL SERVICES

All construction utilities and services shall be arranged by client as per platform safety regulations.

In the absence of established arrangements, temporary utilities shall be made available as follows:

ELECTRICAL POWER & LIGHTING

Power requirements to support all planned and anticipated needs shall be discussed with Client

SITE MEDICAL FACILITIES

Project Coordinator in consultation with Client shall ensure that arrangements are available within

platform to provide medical assistance for injured employees.

FIRE FIGHTING

COMPANY in consultation with Client shall ensure appropriate arrangements for firefighting

equipment and shall develop emergency response plan specific to construction area.

PERMITS - WELDING, WORK AND FLAME

If any hot work required to be carried out by COMPANY, then Project Coordinator shall determine

and communicate to all concerned the policy of the Client related to welding, work and flame

permits.

8.0 ACCOMMODATION, FOOD AND TRANSPORTATION OF COMPANY STAFF

It is anticipated that, during construction, Client will provide accommodation and catering.

Necessary assistance for daily conveyances (if any) from accommodation to work location and back

to accommodation from work location shall be arranged by Client.

9.0 WASTE SKIP, RIGGING AND CRANE ASSISTANCE

Construction waste and household waste shall be handled as per approved waste management

procedure. Waste skips shall be arranged by Client at a nearest location in order to dispose

construction waste and the removed existing installation components. Client shall make necessary

arrangement to dispose waste from skip as necessary by regulation.

Materials delivery to work location shall be by Client and shall be in accordance with list furnished

by COMPANY with necessary segregation and location wise identification for easiness of usage.

Rigging and mobilizing of heavy materials at site near to the installation location shall be done by

Client representative. COMPANY supervisor shall coordinate as necessary.

10.0 COMFORT AND SAFETY FOR THE PLATFORM CREW WHILE CONSTRUCTION

10.1 NOISE & VIBRATION MITIGATION PROGRAM

While carrying out removal / refurbishment works, COMPANY will ensure that risk due to exposure

to noise is either eliminated at source or, where this is not reasonably practicable, reduced to as

low a level as is reasonably practicable using suitable work planning and adopting proper work

methods.

Construction areas will be isolated from the living space; proper barricading and warning signs

will be erected to alert other personnel.

Appropriate work schedules: noise generating activities will be planned for day shifts (to

minimize disturbance to Platform crews, limitation of the duration and intensity of exposure to

noise;

Use of power tools will be limited to minimum to reduce noise levels

Use of appropriate work equipment and tools emitting the least possible noise, taking account

of the work to be done;

Toolbox talks / awareness programs - suitable and sufficient information and training for

employees, such that work equipment may be used correctly, in order to minimize the noise;

Other measures to control noise - ensuring appropriate maintenance programmes for tools &

equipment, safe operating procedures, proper supervision etc.

Noise level assessment will be done (if needed) and corrective measures will be taken to

improve the situation

10.2 HOUSEKEEPING PRACTICES

Housekeeping is a continuous process to prevent accidents to employees and our customers.

Each employee will be responsible for cleaning up his / her own work area.

Aisles, passageways and exit corridors will not be used for the storage of material / tools.

All tools will be collected and returned to their proper places after completing the work.

COMPANY will make sure that no tool or appliance has been left in any place where it might fall

or cause damage when the power is turned on.

Walkways, stairs and fixed ladders shall be kept free of obstructions.

Any surplus materials will be returned to stock or proper storage areas.

Work areas will be kept clean and neat. Supervision will ensure tools and equipment arranged

in a safe, orderly manner.

Rubbish shall be placed only in appropriate containers.

Equipment or materials will be neatly stored when not in use.

Fire and hazard prevention inspections shall be conducted regularly by Safety Officer onboard

Safety Officer will ensure exits are clear at all times.

Exit and fire doors will be unblocked and Operational always.

10.3 WASTE MANAGEMENT

All removed material will be taken out onto designated waste storage area, where it will be

disposed in appropriate waste skips provided by Platform.

Waste storage area shall be properly labeled and warning signs will be erected.

Separate skips are to be provided by the platform according to type and nature of waste as per

Platform waste management procedure

Designated waste storage area shall be free of any fire hazard and shall not pose any

obstruction to personnel or material movement

11.0 WORK METHODOLOGY

11.1 INSTALLATION OF DOORS MANUFACTURER

11.1.1 MATERIALS AND TOOLS

A-door, lock systems, door holders (clamps), screws (bolts), steel treatment tools (drill, thread...),

Measure tools, Installation drawing

11.1.2 MANUFACTURER INSTRUCTION

All doors should be installed according to door plan and accommodation drawings.

Cut-outs should be prepared based on door list.

A-door are installed in steel bulkhead. Make sure that doors are levelled during and after

installation.

Pay attention that top line of all doors in common areas are at the same level. Keep in mind to

secure space for additional accessories (door holders, door closers and other if specified). Strictly

follow door plan, door list and lock system scheme.

11.1.3 INSTALLATION PROCEDURE

1. Check that the size of the steel cut out is correctly measured and cut, distance between frame

and cut out to be 15 mm (±2 mm)

2. Check that the steel bulkhead is straight and has the correct angles. Tolerance for length in

range 400-2500 mm : ± 2mm

3. Place the door in the cut out, bulkhead and frame should overlap 40 mm (± 2 mm), once again

check if bulkhead adhere frame (length 400-2500 mm : ± 2mm).

4. Fasten the hinge side of frame with clamps, after ensuring is vertical and distance between

frame and bulkhead all side is 15 mm, bolt the frame to the bulkhead in position 1 and 2.

5. Check distances between door leaf and door frame, adjust by lifting and lowering the lock side

part of the frame. Check width and diagonals in positions indicated on enclosed drawing and

bolt in pos. 3. Tolerances for lengths ± 2 mm, tolerance for diagonal difference ± 4 mm

6. Limited corrections of inequalities can be made before placing 4th bolt by pressing the down

corner inwards or outwards door leaf.

7. Fasten the frame with remaining bolts (max. distance between bolts can not be grater than 300

mm). Force used for screwing can not cause frame or bulkhead deformation! ( we recommend

to stop screwing after reaching first screw resistance when using hand wrench - around 15 Nm).

8. Make opening and closing test, adjust the hinges if necessary.

Note: hinges and locks should be oiled from time to time.

11.2 INSTALLATION OF DOORS (MANUFACTURER)

11.2.1 PRODUCT DESCRIPTION

Hinged Single & Two leaf A60 Rated Hinged Door with a pressed steel frame.

11.2.2 HARDWARE

Emergency exit panic bar operation fitted to the inside face of the door leaf.

Lockable Lever handle fitted to the outside face of the door leaf.

Overhead door closer fitted to the inside face of the door leaf.

Perimeter neoprene rubber seals and seal trims, fitted to the doorframe.

Vision panel fitted to the Active door leaf.

Lift off stainless steel hinges, fitted to the door leaf & frame.

Top & Bottom shoot bolt fitted to the Passive leaf (Two Leaf Door Sets).

11.2.3 INSTALLATION PERSONNEL

Installation should be carried out by experienced door installers under strict supervision.

11.2.4 TOOLS AND EQUIPMENT

All hand tools should be fit for purpose and periodically checked for damage. All electrical tools to

be no greater than 110V and have current PAT test certificates.

All lifting equipment to have current test certificates (Client assistance).

11.2.5 LIFTING EQUIPMENT

The installation should be assisted by mechanical lifting equipment e.g. - crane, fork truck, chain

blocks with lifting beam, etc.

The installation area itself would govern the choice of lifting equipment to be used.

11.2.6 PRE–INSTALLATION

Barrier off the installation area, as reasonably practical, ensuring emergency access / egress is

always available.

Check the structural opening dimensions against the approved detailed drawings.

Check the supplied door assembly components against the approved detailed drawings.

Any major deviations to the dimensions should be reported to the site supervisor before any other

works are carried out.

11.2.7 INSTALLATION DOORFRAME (WEIGHT 45 KG-APPROX)

The doorframe supplied in one piece and have pre-drilled fixing holes at the required positions.

The hinge “pin” half of the door hinges will be bolted to the frame ready to install the door leaf

onto them, via the door leaf half hinge.

Manually, insert the frame into the structural opening, and clamp it to the structure temporarily

with suitable “G” clamps.

M10 x 30mm long hexagon head bolts will be supplied to fix the doorframe to the structure.

The frame fixing holes can now be drilled into the structure using the frame itself as template,

insert the M10 fixings and securely tighten them.

Remove the temporary clamps.

11.2.8 INSTALLATION – DOOR LEAF (WEIGHT 161 KG APPROX)

The door leaf should be installed via mechanical lifting equipment where possible.

If mechanical lifting equipment cannot be used due to a restricted / inaccessible area, enough

labour should be available for manual handling.

With the door leaf placed horizontally on the floor, with timber blocks under to support, sling the

door leaf, top to bottom, with a suitably long 1T capacity webbing sling, “choking” the sling at the

top of the door.

Connect the sling to your chosen mechanical lifting equipment, i.e. fork lift truck, manual genie

lift, etc, and raise it to the vertical position.

Manoeuvre the door into position at 90 degrees (door open) to the installed doorframe and engage

the door leaf hinges onto the frame hinge pins.

Remove the lifting equipment.

Repeat the above procedure to install the second door leaf on a Two Leaf Door Set.

Check the operation of the door by opening & closing it.

Check that the door seals are effective. The seals are adjustable by loosening the M6 x 12 long cap

head seal holder trim fixing bolts, adjust as necessary via the slots in the seal holder trim, and re-

tighten the bolts.

Using the door closer installation instructions, supplied with the closer, fit the door closer with the

fixings that are supplied with it.

The door & frame will be already prepared with the fixing holes (drilled & tapped).

The door closer can be adjusted (speed & pressure) as per the closer instructions.

11.3 INSTALLATION OF WINDOWS (MANUFACTURER)

11.3.1 DESCRIPTION

Holes in the plating for windows and portholes should be laid out according to the fixed frames, if possible. The size of the slit between the edge of the burnt-out hole and the adjacent surface of the frame must not exceed 1.5 mm.

The plating within the hole should be levelled and checked with a ruler. Straightening of walls after assembly windows is forbidden. In assembly of screwed-on windows and portholes, particular attention should be paid to precise sealing of the slit between the wall surface and the window or porthole frame flange and the holes for clamping screws. Arrangement of holes for clamping screws should be done according to the pattern or laid out according to the frame.

11.3.2 MODE OF PROCEDURE

1. Clean and degrease the sealing surfaces of the flange and the wall.

2. Place a seal or apply a sealing compound (e.g. Sika 221) on both sides of the clamping holes.

3. Apply the compound to the wall around each clamping hole.

4. Place a window or a porthole in the hole. In case of a porthole, the flange holes should be

placed in the wall holes right in the desired position.

5. Screw on the bolts.

6. Remove excessive sealing compound.

11.4 REPLACING WINDOW GLASS (MANUFACTURER)

Before installing the glass onto the frames, check the thickness of glass (t) and glass laying area (s) and compare with drawing

Clean the glass laying area on the main frame and check the requirement of surface coating, if necessary get painted by Client.

Check the gasket for its appropriateness (selected acc. to IBH-038) cut for length off and stick on frames. Place of joint should be on t op, Seal the joint of gaskets with seawater resistant, flexible sealant (for ex. SIKA221)

Put glass pane (5) into the frame keep gap between glass and frame approx. 4mm

Align it using wooden (oak tree) spacers (on bottom 3pcs, other sides 2pcs, 1pc. on every radius) and secure with i.e. IKA221.

Screw glass list using H6x16 screws and set proper torque (for brass screw 4Nm & Steel Screw 8Nm),

Conduct water Hose Test if required by Client.

12.0 INSPECTION

Ensure all installations are as per Client requirements and in accordance with manufacturer

instructions. Ensure that accessories are not damaged during installation. Make sure level of fittings

is correct.

12.1 CLOSE OUT / COMPLETION REPORTS

With reference to approved Inspection test Plans, at defined stages - all activities carried out by

COMPANY are offered for Client Inspection Personnel.

After completion of entire activities within COMPANY Scope of work- without any outstanding punch

lists or issues and all equipment are ready for operation, this stage is considered as customer

acceptance stage. During this stage, all safety aspects of the project have been satisfactorily

completed, Customer Acceptance Certificate signature obtained on completion report / Job card,

and as a result, the entire process is considered closed out.

14.0 ENVIRONMENTAL, SAFETY AND HEALTH

The HSE rules and regulations at site shall be maintained as per Platform HSE policy and project

specific HSE PLAN (if any).

14.1 RISK ASSESSMENT

Hazards identified, Risks at each stage are evaluated, utilizing the risk rating matrix (table 1) as to

determine the level of significance in terms of likelihood of occurrence and severity of hazard based

on the allocated numerical values.

14.2 SPECIFIC EMERGENCY PROCEDURES

A Certified First aider shall be on site at all times whilst a First Aid Clinic shall be available for

treatment of light injuries.

In the event of an emergency, all workers will assemble at an area which shall be allocated by a

post sign and wait for further instructions. This point will be normally marked with a sign as

“MUSTER POINT” or “ASSEMBLY POINT”. The supervisor can assist employees and direct them to the

assembly point.

14.3 TOOL BOX TALK

Supervisor shall conduct daily toolbox talks relevant to work activity prior to commencing daily job.

Before signing the tool box talk sheet (Table-2) supervisor shall ensure the below (when applied) to

the employees involved in the work activity during the TOOL BOX TALK.

No work at height to be undertaken without a safe working platform or suitable fall protection

system.

Work on electrical equipment must only be performed by a qualified electrician.

No hazardous work to be undertaken without a permit system in place including: electrical

energisation, confined spaces, hot works.

No person to work on scaffolding which has not been inspected and approved for use with a

Green Tag.

All lifting operations shall be controlled by a competent person and all lifting equipment must

be inspected and correctly colour coded (Client scope).

No person will operate plant or equipment unless they have been trained, certified and

authorized to do so.

Do not commence any work activity unless suitable lighting for the task is available.

Any deliberate interference with safety equipment, abusive behaviour, fun or deliberate failure

to comply with site will result in removal from site.
