At the heart of hospice is a dedicated inter- disciplinary group. Together, we create a warm and compassionate atmosphere where patients can be among family and friends. Mercy Hospice serves adults with any terminal illness, wherever they may live, including: • Homes and apartments • Nursing homes • Assisted living facilities • Independent care facilities The Hospice Interdisciplinary Team Physicians and clinical staff develop detailed care plans for each patient. • Our Medical Director coordinates the patient’s care with the attending physician and staff. • Our Nurse Practitioner assists the Hospice Medical Director and attending physician by providing assessments, Face to Face patient visits. Nurse practitioner is also available to provide clinical in-services to nursing facilities within service area. Nurses assess and manage pain, provide individualized care and preventive education to reduce medical complications and rehospitalizations. Hospice aides work under the supervision of the nurses to assist with personal care and homemaker services. Social workers help patients and families cope by giving emotional support and assisting with community resources. Chaplains offer spiritual support in ways that are familiar and comfortable with each individual and family’s beliefs and religion. • Trained volunteers provide emotional support for patients and families. Other professionals provide services upon request, such as physical therapists, occupational therapists, speech therapists and dieticians. • Support continues through personal visits, phone calls, grief support groups, individual meetings, and mailings. Our bereavement professionals help families and friends prepare and embrace life without their loved ones.

When is hospice care appropriate?• When a terminal illness has been diagnosed,

life expectancy is six months or less and cure-oriented treatment is no longer being pursued. Hospice care is appropriate when quality of life is the priority.

• You and/or your family, desire only comfort/supportive measures be given.

• You or your family has noted that you have lost weight and have had a decrease in appetite.

• You no longer desire to be hospitalized or treated in the Emergency Room.

• You are not “bouncing back” to your usual level of functioning after repeated illnesses.

Hospice care is covered 100 percent by Medicare and Medicaid. Most private insurance also provide hospice coverage.

Our Mission is to ensure that we take care of those in our community who are in need and no one is denied hospice services based on an inability to pay.
