Metro North HHS - Health Strategy
“The health of high needs groups
will be improved through the
provision of care that is timely,
targeted, accessible and
coordinated”
![Page 9: Mental Health Needs Assessment - Brisbane North PHN...• Aboriginal and Torres Strait Islander and culturally and linguistically diverse communities. Linking key groups to service](https://reader034.vdocuments.us/reader034/viewer/2022042308/5ed40fe38d46b66d22636211/html5/thumbnails/9.jpg)
Identified High Needs Groups
Metro North population groups that have complex care
needs requiring additional and/or specific care are:
• older people
• children and young people
• people with mental illness and/or alcohol and
other drug dependence
• alcohol and other drug dependence
• people with a disability
• Aboriginal and Torres Strait Islander and culturally
and linguistically diverse communities.
![Page 10: Mental Health Needs Assessment - Brisbane North PHN...• Aboriginal and Torres Strait Islander and culturally and linguistically diverse communities. Linking key groups to service](https://reader034.vdocuments.us/reader034/viewer/2022042308/5ed40fe38d46b66d22636211/html5/thumbnails/10.jpg)
Linking key groups to service reform?
• Addressing the needs of those who have complex
needs or experience the poorest health outcomes in
our communities is essential.
• Different expectations and experiences of health
services exist - often culturally or socially
determined, including stigma.
• Engagement requires identification of appropriate
conduits to engage with consumers and communities
• Actions require tailored activities and targeted
services.
![Page 11: Mental Health Needs Assessment - Brisbane North PHN...• Aboriginal and Torres Strait Islander and culturally and linguistically diverse communities. Linking key groups to service](https://reader034.vdocuments.us/reader034/viewer/2022042308/5ed40fe38d46b66d22636211/html5/thumbnails/11.jpg)
What we know about our region ?
• People with mental illness do not always experience
coordinated care when needed.
• Some people are admitted to hospital or remain in
our hospital when they could be better supported in a
less restrictive environment in the community.
• We understand many people with alcohol and other
drug dependence also have a mental illness.
![Page 12: Mental Health Needs Assessment - Brisbane North PHN...• Aboriginal and Torres Strait Islander and culturally and linguistically diverse communities. Linking key groups to service](https://reader034.vdocuments.us/reader034/viewer/2022042308/5ed40fe38d46b66d22636211/html5/thumbnails/12.jpg)
What we know about our region ?
• Effective coordination of services is required for
people with comorbid mental health and alcohol and
drug issues as well as for those who experience
comorbid physical health issues.
• We recognise people with alcohol and other drug
dependence are vulnerable, have complex needs
and require additional specific strategies
complementary to those identified for people with a
mental illness.
![Page 13: Mental Health Needs Assessment - Brisbane North PHN...• Aboriginal and Torres Strait Islander and culturally and linguistically diverse communities. Linking key groups to service](https://reader034.vdocuments.us/reader034/viewer/2022042308/5ed40fe38d46b66d22636211/html5/thumbnails/13.jpg)
Key Strategies
• Be leaders in delivering evidence based quality
care to people with mental illness and/or alcohol and
other drug dependence.
• Increase access to recovery focused mental health
and alcohol and drug services available in the
region.
• Elevate the focus on physical health,
psychological and social wellbeing to support
consumers and carers in their recovery journey.
![Page 14: Mental Health Needs Assessment - Brisbane North PHN...• Aboriginal and Torres Strait Islander and culturally and linguistically diverse communities. Linking key groups to service](https://reader034.vdocuments.us/reader034/viewer/2022042308/5ed40fe38d46b66d22636211/html5/thumbnails/14.jpg)
Key strategies
• Work with partners to increase and facilitate access
to a broader range of whole of life services, including
community based alternatives to hospital admission
and provision of meaningful vocational opportunities.
• Be leaders in quality health care and service
activities by delivering extensive and inclusive
education, training and research programs.
![Page 15: Mental Health Needs Assessment - Brisbane North PHN...• Aboriginal and Torres Strait Islander and culturally and linguistically diverse communities. Linking key groups to service](https://reader034.vdocuments.us/reader034/viewer/2022042308/5ed40fe38d46b66d22636211/html5/thumbnails/15.jpg)
Commitment through Action
• Provide alternatives to hospital admission and
support recovery of consumers through additional
step up/step down facilities across MNHHS.
• Build on existing relationships with emergency
services in joint responses to persons who may be at
risk/in crisis, including Co-Responder models,
implemented in priority areas of need.
![Page 16: Mental Health Needs Assessment - Brisbane North PHN...• Aboriginal and Torres Strait Islander and culturally and linguistically diverse communities. Linking key groups to service](https://reader034.vdocuments.us/reader034/viewer/2022042308/5ed40fe38d46b66d22636211/html5/thumbnails/16.jpg)
Commitment through Action
• Collaborate with partners to develop and implement
service models and associated care pathways for
inpatient services that meet the needs of older
people with a mental illness.
• Strengthen community resources particularly in
the northern part of MNHHS to improve service
responsiveness to people with mental illness and
people with alcohol and other drug dependence.
![Page 17: Mental Health Needs Assessment - Brisbane North PHN...• Aboriginal and Torres Strait Islander and culturally and linguistically diverse communities. Linking key groups to service](https://reader034.vdocuments.us/reader034/viewer/2022042308/5ed40fe38d46b66d22636211/html5/thumbnails/17.jpg)
Commitment through Action
• Expand perinatal mental health services across
specialist community & inpatient services.
• Integrate inpatient and community AOD services
& unify clinical and operational governance
structures for improved performance outcomes.
• Collaborate with partners to grow capacity and
capability of addiction services including alcohol
withdrawal management for adults & young people.

Background
• Requirement for all PHNs to produce a regional plan,
developed and agreed with HHSs and other
stakeholders
• A plan for and by the region about what we all will
do together to improve quality, coordination and
integration (not just a plan about what the PHN will
do)
• Not just primary care – also wider community
services, hospital services, private sector
• Developed with and by stakeholders, including
consumers and carers
![Page 21: Mental Health Needs Assessment - Brisbane North PHN...• Aboriginal and Torres Strait Islander and culturally and linguistically diverse communities. Blue Sky – March Symposium
Compiled our learnings from reviews and consultation prior to March and asked our stakeholders for feedback on the vision and features – have we heard you?
Overall vision for the system
9 features of the overall vision
![Page 27: Mental Health Needs Assessment - Brisbane North PHN...• Aboriginal and Torres Strait Islander and culturally and linguistically diverse communities. Linking key groups to service](https://reader034.vdocuments.us/reader034/viewer/2022042308/5ed40fe38d46b66d22636211/html5/thumbnails/27.jpg)
Blue Sky – March Symposium
Participants
• Over 70 stakeholders including people with a lived experience
attended
Process
• Range of activities to gather feedback on:
• Vision
• Features (that form the vision)
• Suggested actions to achieve the features
Outcomes:
• 1 additional feature recommended (9 10)
• A total of 82 actions suggested to achieve features and move
us from where we are now to where we want to be
![Page 28: Mental Health Needs Assessment - Brisbane North PHN...• Aboriginal and Torres Strait Islander and culturally and linguistically diverse communities. Linking key groups to service](https://reader034.vdocuments.us/reader034/viewer/2022042308/5ed40fe38d46b66d22636211/html5/thumbnails/28.jpg)
Blue Sky – post workshop
With stakeholder feedback from the symposium and other
consultation activities, the PHN MHAOD team:
• Revised vision statement
• Developed the 9 features 10
• Developed consistent structure and language for all features
(i.e. A community where…took the focus off the system and
made it about people)
• All 82 actions were analysed and themed
• 10 common themes found across all 82 actions
• Based on themes, developed 10 ‘system enablers’ to achieve
the features of the vision
• Consolidated 82 actions into 47 actions across each ‘system
enabler’
![Page 29: Mental Health Needs Assessment - Brisbane North PHN...• Aboriginal and Torres Strait Islander and culturally and linguistically diverse communities. Whole of system vision
Consistent with the Queensland Mental Health
Commission:
Our shared vision is a healthy and inclusive community,
where people experiencing mental health difficulties or
issues related to substance use have a life with
purpose and access to quality care and support
focused on wellness and recovery, in an understanding
and compassionate society
![Page 31: Mental Health Needs Assessment - Brisbane North PHN...• Aboriginal and Torres Strait Islander and culturally and linguistically diverse communities. Linking key groups to service](https://reader034.vdocuments.us/reader034/viewer/2022042308/5ed40fe38d46b66d22636211/html5/thumbnails/31.jpg)
10 system features (that make up the whole of system vision)
Together we seek to build a community where:
1. People have the resources and supports to create and maintain healthy,
meaningful lives
2. People are free from stigma and discrimination
3. People are in charge of their own recovery, and services and supports respond
to what people need
4. People achieve their desired outcomes, assisted by services and supports
when needed
5. People know about and get connected to the right services and supports at the
right time and in the right place
6. People seamlessly access different services and supports as their needs
change
7. People and their needs are understood holistically, and are connected to
broader health and community services that address the social determinants
of health
8. People with lived experience are actively involved in all levels of policy,
planning, delivery and evaluation
9. People, their experience and evidence informs services and supports, and
drives innovation and improvement
10. People have confidence in services and supports that are appropriately
resourced, work collaboratively and maintain a stable, skilled workforce,
including peers and carers
