How does an erection occur?During an erection, the blood vessels that let blood into the penis relax and dilate increasing the blood flow to your penis, causing the sponge-like tissues of the penis to swell and harden. The veins that carry the blood away from the penis are then compressed restricting the blood flow back out of the penis.

What causes ED?The process of achieving an erection is complex and requires a precise sequence of events. Problems may occur for a variety of reasons and can be of a physical or psychological nature, or a combination of the two.

Physical causesThe sequence of events that lead to an erection involve nerve impulses in the brain, spine, and penis as well as the subsequent response in the muscles, fibrous tissues, veins and arteries in and near the penis. Physical causes of ED are related to a breakdown of, or damage to, these sequence of events. This could be a result of a disease, hormone imbalance, surgery, prescription medicine or lifestyle factors.

Diseases Such as kidney, neurological or cardiovascular diseases or diabetes may affect hormone circulation, blood flow or nerve function that can lead to ED.

Hormone imbalancesSuch as thyroid, prolactin, and testosterone, can affect a man’s response to sexual stimulation. These imbalances can be the result of a tumour of the pituitary gland, kidney disease, liver disease, or treatment of prostate cancer.

SurgeryPerformed to treat diseases such as prostate and bladder cancer, often require the removal of nerves and tissues around the affected area which can lead to ED. Injuries to the pelvis, bladder, spinal cord or penis that require surgery may also lead to ED.

Prescription medicinesThere are a number of medicines which can cause or contribute to erection difficulties. You should discuss all medicines, whether prescription or not, with your doctor or pharmacist. Do not stop taking any prescribed medicine without first consulting your doctor or pharmacist.

Lifestyle choicesSmoking, alcohol intake or drug use can damage blood vessels and/or restrict blood flow to the penis causing ED. Smoking in particular plays a large role in causing ED in people with arteriosclerosis (narrowing/hardening of the arteries).

Psychological factorsSuch as higher than normal anxiety and stress levels, or depression and low self-esteem, may also contribute to ED.

Modern life can be full of deadlines, frustrations and demands. For many people stress is commonplace in life, but beyond a certain point, stress can stop being helpful and can start affecting mood, productivity, relationships and quality of life.

Once a man experiences ED, he may become overly worried that it will occur again. This can lead to “performance anxiety”, or a fear of sexual failure and consistently cause ED.

Low self-esteem or depression can affect someone physically and psychologically and may be due to prior episodes of ED, or can be the result of other issues unrelated to sexual performance.

How can I be sure I have ED and what can I do?ED is a common issue that will affect many men over the age of 401 at some time. It can be the total inability to achieve an erection, an inconsistent ability to do so, or a tendency to sustain only brief erections.

Use the Erection Hardness Grading Scale2 below to find the erection score that best describes your recent experiences:

Grade 1. Penis is larger but not hard

Grade 2. Penis is hard, but not hard enough for penetration

Grade 3. Penis is hard enough for penetration, but not completely hard

Grade 4. Penis is completely hard and fully rigid

If you are not confident in your ability to get and/or retain an erection when aroused, or if you are not getting a grade 4 erection, talk to your doctor or pharmacist to see if Vedafil® is right for you.

How can this affect my relationship?Although men can often feel frustrated and embarrassed, ED is common, and it happens to a lot of men. Your relationship with your partner may be affected. They may not completely understand why you may be withdrawing from physical contact, so it’s important to discuss these issues with them. This will not only ease any additional strain on the relationship but also help resume a rewarding sex life once treatment is established.

What to expect at your appointmentRemember ED is a common disorder and can be a symptom of other things that may be occurring with you. Although you may be concerned, don’t be embarrassed. Talk to your doctor or pharmacist about your concerns – they have had this discussion with many of their male patients and are familiar with the possible causes, any underlying health concerns and treatment options.

Your doctor or pharmacist will usually access your general health and may check your blood pressure. Your pharmacist may refer you to your GP who might conduct some blood tests or may refer you to an urologist.

If you are concerned or unsure of how to raise the subject of your erection difficulties with your doctor or pharmacist, use the tips below to help generate the discussion:

• Visit your pharmacist or book a general health appointment with your GP and commit to the date.

• Prepare a list of questions in advance. Before you visit your doctor or pharmacist, write down a list of questions you have and what you would like to discuss.

• Be direct. Addressing the issue at the beginning of your appointment can be the easiest and best way to convey to your pharmacist or doctor what you are going through. You will no doubt feel relieved that you have got it off your chest, and your pharmacist or doctor will be able to give you the appropriate treatment. Remember they’ve had this discussion many times before.

Quick and easy ice breakers may include: • My relationship with my partner is not as physical as

in the past.

• Lately I’ve been having difficulties in the bedroom…

• I heard about a new medicine called Vedafil®, can you tell me more about it and do you think this would be right for me?

• I think I have erectile dysfunction (or ED – your doctor or pharmacist will be familiar with the term).

TreatmentsAlthough there are many possible causes of ED, there are treatment options available and also simple lifestyle changes you can make to help with your erection difficulties and to assist in your general health. Depending on the cause of your ED, your doctor or pharmacist may suggest one or more of the following options:

• Lifestyle changes – be it reducing alcohol intake, quitting or cutting down on smoking, avoiding the use of illegal or recreational drugs, becoming more active or reducing stress levels.

• Counselling – either with or without your partner. Sometimes it may be as simple as discussing any anxiety, stress or performance issues you have about your sex life.

• Substituting existing medicines – that can cause or may be contributing to your ED, with other medicines less likely to do so.

Vedafil® – what you need to knowVedafil® is a medicine in oral tablet form, used to treat erection difficulties, specifically erectile dysfunction (ED) in men. Vedafil® contains the active ingredient sildenafil citrate, a treatment that has been prescribed for erectile difficulties since the late 1990s. Sildenafil improves erectile function and is generally well tolerated3.

How does Vedafil® work?Vedafil® works by relaxing the blood vessels that let blood into the penis when sexually aroused, leading to increased blood flow. This increased blood flow causes the sponge-like tissues of the penis to swell and harden resulting in an erection.

How can I get Vedafil®?As Vedafil® is a prescription medicine, you will need to talk with your doctor or pharmacist to see if Vedafil® is right for you. Most doctors will have free samples of Vedafil® for you to try. This will assist in finding the correct dosage for you and managing any possible side effects.

If you are currently prescribed an oral medication for ED, Vedafil® may offer a more cost effective option to your current medication. Once prescribed, Vedafil® is available from most pharmacies nationwide and reputable on-line pharmacies registered in New Zealand. Vedafil® is available in three tablet strengths, 25mg four tablet packs, 50mg four tablet packs and 100mg four and twelve tablet packs.
