3-Tier Prescription Plan new for AACPS

• A new 3-Tier Co-pay Structure is Effective January 1, 2012

• A 3-Tier plan design means there will be three co-pay levels based upon whether your prescription is a generic drug, preferred brand drug, or non-preferred brand drug

– A Preferred Brand is a drug listed on the CVS Caremark Primary / Preferred Drug List

– A Non-Preferred Brand is a drug not listed on the CVS Caremark Primary / Preferred Drug List

What is a 3-Tier Rx Plan?

• It is the most common structure in employer prescription plans

• It permits employees to pay less when they take medications that also cost AACPS less

• It is an approach to contain the growth in prescription expense in the future.

What Are Your Co-pays for 2012?

• Generic medications now cost you less, whether retail or mail order!

• You still have the ability to fill your 90-day supply of maintenance medications at either Caremark Mail Service OR a CVS/pharmacy retail location of your choice. You will pay the same co-pay at CVS/pharmacy as you would at Caremark Mail Service for these maintenance medications.

How is the CVS/Caremark Drug Formulary Developed and Managed?

• All FDA approved drugs are reviewed quarterly by the CVS Caremark National Pharmacy and Therapeutics (P&T) Committee. 

• The drug review process consists of a review of  • scientific evidence,

• standards of practice,

• peer-reviewed medical literature,

• accepted clinical practice guidelines

• Other appropriate information such as pharmacoeconomic studies

• The Committee consists of nationally recognized, practicing physicians, pharmacists and a medical ethicist.

Formulary Drug Selection

• The drug selection process includes the following considerations:

– Drug safety relative to other drugs with the same indication and therapeutic action

– Drug effectiveness for all FDA-approved indications and relative to other drugs with the same indication and therapeutic action

– Available dosage forms of drug and dosing interval for each available indication

– Potential for participant disruption in comparison to other drugs with the same indications or therapeutic actions

• Use Generics– You may be eligible to receive a free first fill for a generic alternative

for a brand medicine you are currently taking. • Yes, you may receive a 90-day supply at no cost!

– Your doctor is best qualified to decide whether a generic alternative is right for you. Obtain a copy of CVS Caremark’s Primary/Preferred Drug List from www.caremark.com and discuss generic alternatives with your physician.

• Remember– If you request a brand-name medicine when a generic alternative medicine

is available, your total out-of-pocket costs for your prescription are higher. You pay the brand co-pay plus the difference in cost between the brand-name medicine and the generic medicine.

• Preferred Brand Drugs– If a Generic medication is not available, obtain a copy of CVS Caremark’s

Primary/Preferred Drug List from www.caremark.com and discuss Preferred Brand alternatives with your physician.

• Those taking Non-Preferred brand medications will receive a letter identifying Preferred Brand and Generic alternatives. Take this letter to your physician to discuss your options.

• CVS ExtraCare Health Card– Provides a 20% discount at CVS/pharmacy retail locations for CVS brand

over-the-counter products
