Support When You Need It Personalized SupportWhether you have a quick medical question, are facing a new diagnosis, or need help finding a provider, CDPHP has a team of registered nurses, educators, pharmacists, and more to help you find answers and manage and monitor your health.

Human Arc’s Premium AssistThis free service provides Medicare Advantage members assistance with determining potential eligibility for Medicare savings programs and enrolling in the Medicare Part D Extra Help/Low-Income Subsidy. Members can call 1-866-533-2207.

Landmark HealthCDPHP has partnered with Landmark Health to provide a no-cost program that offers comprehensive and compassionate care to qualified individuals – wherever they are and whenever they need it.

Chronic Conditions SupportRegular checkups and monitoring blood glucose levels can help members prevent potential health issues and keep their diabetes under control. Ascensia (formerly Bayer) Diabetes Care is our preferred vendor for blood glucose monitoring systems. They offer blood glucose monitors and test strips at no cost. Because this is a medical benefit, members do not need a CDPHP pharmacy benefit to receive a meter or to order test strips. Just present a CDPHP member ID card at time of purchase. Members who use an insulin pump should contact their provider to determine which glucose monitor is right for them.

Prescription Drug AssistanceCDPHP MedCheckMedicare Advantage members with prescription drug coverage are eligible to receive a MedCheck medication review at no additional cost. Through this program, you can schedule a one-on-one comprehensive medication review with a pharmacist to:

Ask questions about medications.Discuss safety issues and possible side effects.Learn ways to lower costs.Discuss how to properly take medications.Receive a personal medication list and action plan. (We recommend that members share this list and plan with their doctors.)

EPIC (Elderly Pharmaceutical Insurance Coverage Program)EPIC is a New York state program administered by the Department of Health to help seniors save money on their prescription drugs. To qualify, a member must be a New York state resident, 65 or older, enrolled or eligible to enroll in a Medicare Part D drug plan, have an annual income below $75,000 (if single) or below $100,000 (if married), and not receive full Medicaid benefits.

Seniors can apply for EPIC at any time during the year; coverage runs from January 1 to December 31 of each year. To join, call 1-800-332-3742 (TTY: 1-800-290-9138) or visit http://health.ny.gov for an application.

Rx for LessIf you have prescription drug coverage, you can enjoy deep discounts on certain generic drugs when purchased at participating pharmacies. Some generic drugs are available for as little as a penny a pill.
