Tens of millions of Americans suffer with daily heartburn or reflux symptoms such as regurgitation, chronic cough, hoarseness and dental erosions. Only one hospital in the Treasure Valley offers an incisionless surgery to help alleviate these problems.

The Gastroesophageal Reflux Disease (GERD) Services at West Valley Medical Center, which will begin within the next couple months, include the new Transoral Incisionless Fundoplication procedure (TIF). With the use of the innovative EsophyX device, TIF is performed completely without incisions and is now available from Drs. Ryan Hardy, Advanced Surgery of Idaho, and Steven Koga, West Valley Medical Group, Caldwell.

Drs. Hardy and Koga are the first specialists in the Treasure Valley receiving TIF procedure training.

“The TIF procedure can significantly improve quality of life for our patients.” Dr. Hardy

said. “Gastroesophageal Reflux Disease is an anatomical problem that needs an anatomical solution. Reflux medication such as PPIs (Prilosec, Nexium, Protonix, etc.) can help relieve patients’ heartburn symptoms but don’t solve the underlying anatomical problems or prevent further disease progression. Even on PPIs, many patients are still unable to eat the foods they want or have to sleep sitting up to reduce nighttime reflux.”

Where it beginsGastroesophageal reflux, also

known as acid reflux, occurs when the stomach contents back up into the esophagus and/or mouth. Reflux is a normal process; most episodes are brief and do not cause bothersome symptoms or complications.

People with GERD, however, experience symptoms as a result of reflux. Symptoms can include heartburn, vomiting or pain with swallowing. The reflux of stomach acid can affect vocal cords or even be inhaled into the

lungs.Most GERD patients will not

develop serious complications, particularly when reflux is adequately treated. However, some serious complications can arise in severe cases, including:

Washing hands isn’t simply a method of cleaning up before sitting down to dinner. Frequent hand-washing is the single most effective way to prevent illness.

Proper hand-washing has proven effective against the spread of 99 percent of germs that travel via droplets -- and that includes the flu virus. The fact of the matter is that dirty hands have the potential to be deadly, especially if they are the carriers of hazardous germs.

It’s impossible to avoid germs. Simply going about daily activities exposes a person to thousands of

germs. Shopping in a store, going to school or even spending time around the house can spread germs. When a person touches his or her eyes, mouth or nose with hands that are harboring viruses or bacteria, illnesses can easily take root. Germs can also be spread via touch.

Proper Hand-Washing Techniques

There is a right way to wash hands. Follow this procedure, courtesy of The Mayo Clinic.

1. Turn on the water to desired temperature (cold or hot water is equally effective).

2. Wet hands under running water.

3. Apply soap and lather for 20 seconds or more, paying careful attention to all areas of the hands, including the backs of the hands.

4. Rinse well.5. Dry hands on a towel or

with an air dryer. Use a

towel to close the faucet. Many germs live on wet faucet handles, and a person can contaminate him or herself again.

Keep in mind that antibacterial soap is no more effective at killing germs than regular soap. In fact, antibacterial products -- when overused -- actually may kill off good bacteria or cause certain bacteria to become resistant. This will make it harder to kill these germs in the future.

When soap and water are not available, alcohol-based sanitizers are effective at keeping hands clean. Look for commercially produced products that contain at least 60 percent alcohol. Enough of the product should be used to wet hands completely. Rub hands and wait for the product to be completely dry for maximum effectiveness.

As temperatures drop it is easy to slip into eating more “comfort” foods. We want food to fill and warm us up, but sometimes we forget the fruits and vegetables we should be eating, as well as the other high protein, heart healthy foods we need to include in our winter diets.

So, without putting too fine a point on it, here are some heart healthy foods facts. Five helpings (palm size) of fruits and vegetables a day will rid you of free radicals in the blood stream, protecting blood vessels. Whole grains, beans, legumes, nuts and fatty fish are incredibly important and offer complex heart-proactive phytonutrients.

Some foods to work into your winter diet are:

They also increase good HDL levels, lower triglyceride levels, protect arteries from plaque buildup, are anti-inflammatory and lower blood pressure.

B-complex vitamins like Vitamin B-12 (foliate) and Vitamins B-6 protect against blood clots and atherosclerosis or hardening of the arteries.

Grilled Miso SalmonServes 4

Ingredients:½ cup Mirin (rice vinegar)

2 tablespoons minced fresh chives or green

(spring) onion tops1 tablespoon yellow miso

1 tablespoon low-sodium soy sauce

1 teaspoon peeled and minced fresh ginger

4 salmon fillets, 5 ounces each, skinned

2 tablespoons chopped fresh cilantro

1 teaspoon sesame seeds, toasted

(Mirin, Miso and Tahini can be found in Asian food stores)

Directions:In shallow baking dish, whisk together mirin, chives, soy sauce, tahini and ginger. Add

fish to the marinade and turn to coat. Cover and marinate in the refrigerator for 1 to 2

hours, turning occasionally.Remove the fish from the marinade and pat dry. Discard marinade. When the grill pan is

very hot, place the fillets on it and cook, turning carefully with a spatula, until grill-marked,

firm to the touch, and opaque in the center, about 4 minutes on each side.

Transfer fillets to a serving platter. Garnish with cilantro and sesame seed. Serve

immediately.

Acorn Squash with ApplesServes 2

Ingredients:1 Granny Smith apple, peeled, cored and sliced

2 tablespoons brown sugar

1 small acorn squash, about 6 inches in diameter

2 teaspoons trans-free margarine

Directions:In a small bowl, mix together apple and brown

sugar. Set aside. Pierce squash several times with a

sharp knife to let the steam escape during cooking.

Microwave on high until tender, about 5 minutes;

Turn squash after 3 minutes to ensure even cooking.

Place squash on a cutting board and cut in half.

Scrape the seeds out of the center of each half and

discard. Fill the hollowed squash with apple mixture.

Return squash to the microwave and cook until apples are softened, about 2 minutes.

Transfer squash to a serving dish. Top each half with 1 teaspoon margarine and serve

immediately.

Running could just be the perfect exercise. As an avid runner, I am quite biased on the subject. Running doesn’t require a membership, you don’t need a personal trainer, it gets you outside and the only equipment you need is a pair of running shoes. Research shows that runners get sick less, have reduced depression rates, cancer rates, cholesterol, decreased incidence of Diabetes and have greater longevity. In addition, running may not be as stressful to the joints as once thought. A Stanford researcher found that there was no difference in osteoarthritis in the knees between runners and nonrunners. In fact, a study out of the British Journal of Sports

Med showed that the onset of disability (difficulty walking, standing, performing daily activities) in people over the age of 60 was 16 years later in runners versus nonrunners. Recently, the concept that humans have

“evolved” to run long distances has got a lot media attention. An evolutionary biologist out of Harvard has proposed that early humans evolved to run long distances to outlast their hunted prey in a technique called

“persistence hunting” . So, the current evidence suggests that perhaps humans are built more for running than for sitting at a desk, running is not “bad for the knees”, can prolong your life, improve your quality of life as you age and make you happy.

If you have never ran before, it is important to take it slow. The human body is an amazing biomechanical machine that responds to physical stress by getting stronger. If the stress is

applied in the correct dose, the muscles, tendons, ligaments and joints will adapt and get stronger. However if the dosage is increased too quickly, there is breakdown in these tissues and injuries occur. One reason for high injury rates, especially in runners and athletes over the age of 30, is that cardiovascular fitness improves faster than tissue fitness. In other words, your lungs and heart get in shape in less time than the ability of your body’s tendons, joints and ligaments to tolerate the increased loading of running. Another reason is the high forces of running can produce anywhere from 6-10 times your body weight at impact. The tips listed below will help you avoid overstressing your tissues and preventing injuries.

1. Run/Walk program. If you are a new runner or returning to running after taking more than a year off, start with a run/walk program. A good ratio to start with is 1 min run: 4 min walk. Add 1 min to your ratio every 2 weeks (2 min run: 3 min walk) until you have progressed to a ratio that is pain free and comfortable.

2. 10% Rule: Do not increase your weekly mileage by more than 10% per week. If you add hills and track workouts to your training you may even want to decrease your mileage that week.

3. Stretch after you run: Studies have shown that stretching before a run can actually increase your risk of injury. Take the time to stretch key muscle groups after your run. A link for a copy of

running stretches can found at the end of the article.

4. Run with short strides: To reduce the impact of your body weight attempt to strike your foot under your body (not out in front of you).

5. Purchase good shoes: Go to a store that specializes in runners’ shoes.

You can count on having some soreness as you start your running program, but all pain is not bad. Our body responds to physical stress through inflammation. Inflammation is the process that our body uses to heal itself and this process can make you sore (this is also the process that makes us stronger!). Generally, significant soreness usually only lasts 24-48 hours after a workout. Signs and symptoms that indicate that you have overstressed your body include severe sharp joint or tendon pains, pain that keeps you awake at night, swelling or redness, excessive fatigue

and frequent illness. If you are experiencing these types of symptoms, take a week off from the running and then slowly ease back into your normal workout program.

If you need more guidance, join a local running group or a coached training program. In Boise, we have several running programs and groups that meet 1-2x/week for organized runs and coaching. Another way to get motivated is to pick a local event and train for it. Most communities will have races at distances ranging from 5 Kilometers up to 30 Kilometers.

Finally, physical therapists are great resources for runners. PT’s are experts in biomechanics and injury treatment. Many offer free screenings and running classes. At our clinic we offer a weekly running strengthening class, Video Gait Analysis, Running Biomechanical evaluations and free running injury prevention lectures. Several of the

Nearly two percent of the American population applies for Social Security disability (SSD) benefits every year. Statistics show that nearly 1/2 of those over the age of 65 will fall at least one time per year accounting for 65 % of injuries to the elderly. These injuries of-ten result in broken or fractured bones, surgery and with other medical complications, death.

Ongoing monitoring of medications, with an initial as-sessment and periodic reassess-ments, will help support the effort to reduce these risks. It is also found that seniors receiving non-medical home care have significantly reduced both falls and the number of hospital ad-missions for other health issues. Non-medical home care is an option available to you or your loved one who may be aged or disabled and would like to con-tinue to live in their own home or that of a family member. It is a service that supports an individual’s independence while

providing assistance with activi-ties of daily living (ADL). This assistance means that loved ones are not trying to accomplish an activity that puts them at risk.

It can be extremely difficult to admit that you need help or challenging to convince a loved one they need assistance. A further challenge is finding an agency you can trust. There is a vast abundance of resources in the community to assist you in your time of need, guiding you to the answers you need in hopes of replacing the fear and confusion of all the options with peace of mind.

While working hand in hand with doctors, rehab centers, home health and hospice agen-cies, non-medical home care can assist in providing quality care and assuring that all needs are met with dignity and com-passion in your own home.

Things to look for and keep in mind when seeking a non-medical home care agency:

Idaho has no standard of rules or regulations to govern on how a nonmedical home care agency shall operate un-less it is funded by Medicaid. Therefore it is important to look at an agency that has their own set of standard en-forced rules and regulations that meet and go beyond Medicaid requirements.

Look for management with experience and knowledge.

Ask other professionals about an agency’s reputation, community relations and ref-erences. Look for an agency with Honesty, Integrity, Compassion and Empathy.

Ask about care provider training, screening processes and turnover. All caregivers should have CPR certifica-

tion, certification for As-sistance with Medications, receive extensive initial train-ing or testing of knowledge, and receive continuing edu-cation.

What types of services do they offer? Will they meet your present and future needs?

Ask about their minimum number of hours. What tasks are they willing to provide? Are they willing to work out a schedule that meets your needs but stays within your budget?

Shop around. Avoid choos-ing an agency on their $$ per hour fees.

Remember you can never ask too many questions.

Making the decision to have someone care for a loved one isn’t easy, in fact, it’s often downright scary. After all, you’re making a decision that impacts the physical and emotional well being of your loved ones and, to an equal extent, yourself.

Choosing the right agency is the key to a successful and fulfilling decision. When selecting a home health care agency, here are a few things to consider:

Make a List of the Services Needed

Is your loved one going to need skilled services, such as nursing or physical therapy? Or would personal care services such as housekeeping, laundry, bathing assistance, meal preparation and shopping best meet the needs?

It’s possible that a combination of the two is needed.

Types of ServicesPersonal Care Services are

provided by trained caregivers who assist with the activities of daily living (ADL’s). This service may also be referred to as Non-Skilled Service, Private Duty, Respite or Companion Care.

Home Health Services provide rehabilitative skilled services such as nursing or physical therapy. A physician’s order is required and the Home Health episode is generally covered by Medicare or Private Insurance.

Determine the Frequency of Care

What times of the day and how many days per week will care be needed?

For Personal Care Services this may be determined by the type of care needed and how much other support is available prior to adding an agencies help.

The agency should be able to help answer these questions. To assist in the determination, it is a good idea to put together a list of service needs before initiating the search for an agency. Knowing exactly what you’re looking for is an invaluable shopping tool, especially when comparing multiple options.

Home Health is generally prescribed by a physician.

Identify the Strengths and Weakness of the Agency

Not all agencies are created equal!

The best way to ensure a decision that’s in the best interest of your loved one is to ask questions. Though the following isn’t an all-encompassing list, it should be a good starting point:

Is the agency licensed? (required for most insurance companies)

Is the agency Insured and

Bonded?How long has the company

been in business?What types of services does

the agency offer? (do they of-fer a variety of service types)

Who will I talk to if I call after normal business hours?

How does the agency screen the caregivers?

Is there a backup plan if the caregiver becomes sick or is absent?

How often are caregivers available?

Does the agency work with Medicare, Medicaid and other private insurance com-panies?Choosing a home health care

agency is a big decision. Take your time, research the agency thoroughly, and ask plenty of questions.
