    Ladies and Gentlemen:Enclosed herewith is our Phase 1/11 Environmental Site Assessment Report prepared for the propertyidentified as the Proposed Medford Square Garage to be located off Governors Avenue in the MedfordSquare section of Medford, Massachusetts. The general site locus is shown on the enclosed Figure 1,and the boundaries of the subject site are shown on the enclpsed Figure 2.This report was prepared by McPhail Associates, Inc. in accordance with our proposal dated March 22,2010 and the subsequent authorization of Samiotes Consultants, Inc., and is sUbject to the limitationscontained in Appendix A.This environmental site assessment was conducted in general accordance to the provisions contained inthe following: (1) Massachusetts Oil and Hazardous Materials (OHM) Release Prevention and ResponseAct (MGL Chapter 21 E); (2) Massachusetts Contingency Plan (MCP) 310 CMR 40.0000; and (3) ASTM E1527-05 Standard Guide for Environmental Site Assessment: Phase I Environmental Site AssessmentProcess, as referred to in 40 CFR Part 312 (the All Appropriate Inquiries Rule).The objective of the environmental investigation was to identify the potential presence of RecognizedEnvironmental Conditions (RECs), as defined by ASTM, at the subject site or on nearby property that maypose a threat to the subject site. The objective of the Phase II portion of this environmental assessmentwas to assess the potential impacts to soil and groundwater due to the current and former presence ofundertakers located adjacent to the southeastern portion of the subject site.Our scope of work included the following: (i) a review of previous reports; (ii) a visual inspection of thesubject site and reconnaissance of the surrounding area; (iii) a review of readily available informationregarding the subject site history and usage; (iv) a review of readily available municipal, state and federaldatabases and records; (v) a subsurface investigation consisting of the performance of borings and theinstallation of a groundwater monitoring well; (vi) chemical analysis of selected soil samples obtained fromour recent subsurface investigation; and (vii) chemical analysis of a groundwater sample obtained from anewly installed monitoring welL. The conclusions presented herein and our professional opinions are basedsolely on the scope of work conducted. The scope of our Phase 1/1 Environmental Site Assessment did

    The purpose of this report by McPhail Associates, Inc. is to present theresults of a Phase 1/11 Environmental Site Assessment for the potentialpresence of Recognized Environmental Conditions at the propertidentified as the Proposed Medford Square Garage to be located off ofGovernors Avenue in Medford, Massachusetts. Refer to the ProjectLocation Plan (Figure 1) for the general site locus. The approximatelimits of the subject site, which define the limits of our investigation, areshown on the enclosed Figure 2.

    These services were performed and this report was prepared inaccordance with our proposal dated March 22, 2010 and the subsequentauthorization of Samiotes Consultants, Inc., and are subject to thelimitations in Appendix A.The environmental site assessment was conducted for the abovereferenced propert in general accordance with the American Society forTesting and Materials (ASTM) Standard Practice for EnvironmentalAssessments: Phase i Environmental Site Assessment Process (ASTM E1527-05) as referenced in 40 CFR Part 312 (the All Appropriate InquiriesRule). Standards utilized in our evaluation included the MassachusettsOil and Hazardous Materials (OHM) Release Prevention and ResponseAct (MGL Chapter 21E) and the Massachusetts Contingency Plan (310CMR 40.0000).The objective of the Phase II portion of this Environmental Assessmentwas to assess the potential impacts to soil and groundwater due to thecurrent and former presence of an undertaker located to the southeastand downgradient of the subject site.Our scope of services consisted of the following, (i) a review of previousreports; (ii) a visual inspection of the subject site and reconnaissance of

    Excluded from our Phase I ESA scope of work were a title search, anenvironmental lien search, an assessment for the presence of lead-basedpaint, mildew, mold, urea formaldehyde foam insulation (UFFI), asbestoscontaining materials and other naturally occurring pollutants such asradon. No attempt was made to check on the compliance of present orpast owners of the site with federal, state or local laws and regulationsexcept as documented herein. .The objectives of this Phase I ESA, as defined in the ASTM E-1527-05Standard, are to identify the presence of Recognized EnvironmentalConditions at the subject site or on surrounding properties that maypotentially pose a threat to the subject site.The term Recognized Environmental Condition (REC) is defined byASTM E-1527-05 as the presence or likely presence of any hazardoussubstance or petroleum product on a propert under conditions thatindicate an existing release, a past release, or a material threat of arelease into structures on the propert, or into the ground, groundwater orsurface water of the propert. In addition, ASTM E -1527-05 indicatesthat the term REC does not include conditions that generally do notpresent a material risk of harm to public health or the environment andthat generally would not be the subject of an enforcement action ifbrought to the attention of appropriate government agencies.Our professional opinion is based solely on the scope of work conductedand pertains to the subject site limits as shown on Figure 2 and definedbelow.

    SITE AND LOCUS Location and DescriotionDESCRIPTION The subject site has a footprint of approximately 25,850 square feet andcurrently consists of an existing asphalt paved parking lot that is locatedbetween Governors Avenue to the west and Bradlee Road to the east.The southern side of the parking lot is bounded by the rear of commercialbuildings that front onto High Street. The northern side of the site isbordered by properties that contain a 5-story brick residential structureand a 1-story brick commercial structure.

    The approximate limits of the subject site are shown on Figure 2, whichwas prepared from a draft 20-scale existing conditions plan prepared bySamiotes Consultants, Inc. and dated April 6, 2010.The subject site is located at longitude and latitude 42 25' 8.8" north and71 6' 40.7" west, respectively, and at UTM coordinates 326,290.8 meterseast and 4,698,255.5 meters north in Zone 19.Site and Vicinity General CharacteristicsThe subject site is located in the Medford Square area of Medford,Massachusetts. The area surrounding the subject site is populated bymulti-story buildings which are primarily occupied by residences and retailbusinesses.Currently, the subject site is occupied by a municipal-owned, pavedparking lot. Access to the parking lot is provided by entrances off ofGovernors Avenue and Bradlee Road. The site slopes sightly upwardfrom the southeast towards the northwest, with ground surface varyingfrom about Elevation +31 to Elevation +42.

    as defined by the Massachusetts Department of EnvironmentalProtection. Further, the Priority Resources Map indicates that there areno public or private drinking water supply wells, no Areas of CriticalEnvironmental Concern, no fish habitats, no habitats of Species ofSpecial Concern or Threatened or Endangered Species within specifieddistances of the subject site. There are no water bodies or wetland areason the subject site. The nearest water body with associated wetlands isthe Mystic River, which is located approximately 250 feet to the south ofthe subject site. There are no solid waste sites (landfills) noted as beinglocated within 1,000 feet of the subject site. Based on EDR's search ofFEMA Flood Plain Maps, the subject site is not within a 100 year or 500year flood plain.Current Uses of the Subiect SiteThe subject site is occupied by a municipal-owned, paved parking lot,which is utilized by the public.Current Uses of Surroundina ProoertiesSurrounding properties are primarily occupied by residential andcommercial buildings. A law offce and a multi-unit residential structureare located to the north of the subject site. A funeral home and a multi-unit commercial structure occupied by restaurants, a vacant unit, a beautysalon and an art gal.lery are located to the south of the subject site. Thesubject site is bounded by Bradlee Road to the east. A bank and a healthcenter are located further east of the subject site, on the opposite side ofBradlee Road. The subject site is bounded by Governors Avenue to thewest. A multi-story offce building, The Medford Historical Society and asingle-family residential structure is located further to the west of thesubject site, on the opposite side of Governors Avenue.

    A walk through and visual inspection of the propert was performed by arepresentative of McPhail Associates, Inc. on April 9, 2010. Observationsof readily visible portions of adjacent and nearby properties were alsomade. However, in general, these observations were made from outsidethe boundaries of these properties. The extent of the subject site isshown on Figure 2. Photographs taken during our site visit on April 9,2010 are included in Appendix B.As mentioned above, the subject site is occupied by an asphalt pavedmunicipal parking lot. The parking lot appeared to be in good conditionwith only minor cracking observed. Minor staining likely attributable tovehicular traffc was observed on the asphalt paved areas, but appearedto represent de minimis conditionsIn summary, surficial observations of readily observable portions of thesubject site did not identify spils or releases of OHM that would beconsidered to represent RECs with respect to the subject site.Surrounding PropertVisual observations of surrounding properties were made from outside ofthe propert limits. Surrounding properties are primarily occupied byresidential and commercial buildings. A 1-story brick structure occupiedby Colonna Law Offes and a 5-story, brick, multi-unit residentialstructure are located to the north and northeast of the subject site. Nostains or odors were observed at the properties located to the north andnortheast of the subject site.Gaffey Funeral Home and a multi-unit commercial structure occupied byColleens Restaurant, a vacant unit, Sea Waves Nail and Beauty Salon,Rosas Roast Beef and The Paint Box, an art gallery, are located to the

    area located adjacent to the southwestern portion of the subject site, butno indications of spils of Oil or Hazardous Materials (OHM) wereobserved in the vicinity of the dumpster or transformer. The transformerwas not labeled to indicate the presence of PCB containing oiL.The subject site is bounded by Bradlee Road to the east. MemebersTrust Credit Union and a 2-story, brick structure occupied by a women'shealth center are located further east of the subject site, on the oppositeside of Bradlee Road. No stains or odors were observed in the vicinity ofthe properties located to the east of the subject site.The subject site is bounded by Governors Avenue to the west. A 3~storybrick structure with a basement was observed to be occupied by theMedford Professional Building, a 2-story, stucco structure occupied byThe City of Medford Historical Society and a single-family residentialstructure are located further to the west of the subject site, on theopposite side of Governors Avenue. No stains or odors were observed inthe vicinity of the properties located to the west of the subject site.In summary, no surfcial indications of RECs were observed on adjacentor nearby properties.

    Our research into the history of the subject site included a review ofSanborn Fire Insurance Maps dated 1971,1950,1936,1910,1903,1897,1892 and 1887, historic USGS Topographic Maps, historic AerialPhotographs supplied by EDR and a City Directory Search completed byEDR. Copies of the Sanborn Maps and the City Directories are includedin Appendix C.The 1971 Sanborn Map documents the subject site as being utilzed for

    and a residence and a bank existed to the east of the subject site duringthis time.The 1950 and 1936 Sanborn Maps indicate the subject site as beingvacant during this time. This Sanborn Map indicates that buildings whichcurrently occupy properties surrounding the subject site have generallyremained unchanged from the previous Sanborn Map except that theoffce building located to the northeast of the subject site and the offcebuilding located to the southwest of the subject site are not depicted onthe 1950 Sanborn map. The 1936 Sanborn Map indicates that thebuilding located to the south of the subject site had been utiized forrooming, a dwellng, a store and a office, a bLlilding located to thesouthwest of the subject site was occupied by New England Telephoneand Mystic Exchange Telephone Company and a structure located to thewest of the subject site is indicated to have been occupied by pollngbooths.

    The 1910, 1903, 1892 and 1887 Sanborn Maps indicate the subject siteas being vacant during this time. These Sanborn Maps indicate thatbuildings which currently occupy properties surrounding the subject sitehave generally remained unchanged from the previous Sanborn Mapexcept the properties located to the north of the subject site are indicatedas being vacant and the properties located to the south of the subject siteare indicated as being occupied by dwellngs during this time. The 1910Sanborn Map indicates that the Telephone exchange existed to thesouthwest of the subject site and an auto house existed to the south ofthe subject site. The 1903 Sanborn Map indicates that dwellings existedto the southwest of the subject site. The 1892 Sanborn Map indicatesthat Bradlee Road did not exist at that time.A City Directory search was completed by EDR. The City Directories

    occupied by apartments in 2008, 1995, 1990 and 1970, the propert withthe address of 17 Governors Avenue was occupied by apartments in2008, Fowler Dance Studio in 1995, 1990 and 1985, Governors Halls in1990 and 1985 and the Medford Womens Club in 1975 and 1970, andthe propert with the address of 3 Governors Avenue was occupied byBond & Nickerson in 1990.In summary, a review of historical records indicated that the subject sitehad been utilized for parking or occupied by vacant land since at least1887. The historical records reviewed further indicated that thesurrounding properties had been occupied by residential structures andcommercial structures including undertakers since at least 1887. RECswere not identified during our review of historical records of the subjectsite and surrounding properties. However, as part of the Phase II portionof this environmental assessment, soil samples and a groundwatersample obtained from the recently conducted subsurface investigationwere submitted fro chemical analysis to assess potential impacts to soiland groundwater due to the current and former presence of undertakerslocated in the vicinity of the subject site. The chemical analysis results arediscussed in detail further in this report.

    In accordance with ASTM 1527-05, Article 8.3.2 and Article 8.3.2.1 theuses of the propert shall be identified back to the propert's firstdeveloped use, or back to 1940, whichever is earlier, and the maximuminterval between historical sources is 5 years. During the time periodbetween earliest reasonable ascertainable evidence regarding the subjectsite to the present time, some intervals between historical sourcesexceeded 5 years; therefore, data failure was encountered. However, theindicated use of the subject site between sources was consistent and,therefore, the data failure is not considered to constitute a significant data

    properties was conducted at the City of Medford Board of Health on April13, 2010. According to a representative from the City of Medford Boardof Health, their records indicated that there were former sanitaryviolations at the former parking garage located at the subject site. Thedocumentation reviewed did not indicate evidence of the storage of OHMor a spil or release of OHM at the site.5. City of Medford Fire Department -Fire Prevention Division

    Our written request for available records related to the historic storag.e oruse of oil and hazardous materials for the subject site and adjacentproperties with the listed addresses of 7 Governors Avenue, 15Governors Avenue, 16 Bradlee Street and 61-45 High Street wassubmitted to the City of Medford Fire Department Fire Prevention Divisionon April 13, 2010.According to City of Medford Fire Department Fire Prevention Division'se-mail response on April 15, 2010, their records included oil burner withassociated oil tank permits for the addresses of 15 Governors Avenueand 47-59 High Street. The documentation reviewed did not indicateevidence of a spill or release of OHM at these properties. A copy of theCity of Medford Fire Department Fire Prevention Divisions' information isincluded in Appendix D.6. UST Registry On-Line DatabaseOur review of the Massachusetts State Fire Marshall's UndergroundStorage Tank (UST) Registry Online Database on April 16, 2010, did notidentify records of USTs at the subject site or in the immediate vicinity ofthe subject site.

    The information provided by EDR indicates that the subject site locatedon Governors Avenue and Bradlee Road, is not listed on any of thedatabases reviewed by EDR.Based upon information provided by EDR, there are no U.S.Environmental Protection Agency (EPA) National Priority List (NPL) sitesor Proposed National Priority List sites located within one-mile of thesubject site according to the database updated as of December 1, 2009.The EDR report indicates that there are no listed ComprehensiveEnvironmental Response, Compensation and Liability Information System(CERCLlS) and RCRA Corrective Action Activity (CORRACTS) siteslocated within one-half mile of the subject site based upon the EPAdatabase updated as of June 30, 2009.There is one (1) Comprehensive Environmental Response,Compensation and Liability Information System - No Further RemedialAction Plan (CERCLIS-NFRAP) site located within one mile of the subjectsite. The CERCLIS-NFRAP is identified as Havard Community HealthPlan located at 26 City Hall Mall, which is located 1,300 feet to thesoutheast of the subject site. The CERC-NFRAP listing indicates that thesite has been archived and removed from the inventory of CERCLISsites. Given that the status of this site and its location, the CERC-NFRAPfacility is not considered to be an REC with respect to the subject site.The EDR report further indicated that there are no Large QuantityGenerators (LQG) listed in the Resource Conservation and RecoveryInformation System (RCRIS) dated January 13, 2010 that are locatedwithin 0.25-miles of the subject site.The EDR report indicated that there is one (1) Small Quantity Generator(SQG) site listed in the Resource Conservation and Recovery InformationSystem (RCRIS) database last updated on January 13, 2010, that is

    The EDR report further indicated that there is one (1) ResourceConservation and Recovery Act Conditionally Exempt Small QuantityGenerator (RCRA-CESQG) site located within 0.25-miles of the subjectsite, based on the database updated on dated January 13, 2010. TheCESQG site is identified as Medford Gas & Service with an address of 85Main Street located approximately 1 ,100 feet to the south of the subjectsite. This CESQG site is listed with violations which have achievedcompliance. Since this CESQG site is listed with violations which havesubsequently achieved compliance, this site is not considered an RECwith respect to the subject site.The EDR report further indicated that there are two (2) RCRA Non-Generator (Non-Gen) sites located within 0.25-miles of the subject site,based on the database updated on January 13, 2010. According to theEDR report, there are no violations on record for the two (2) RCRA Non-Gen sites. According to the EDR report, both of these RCRA Non-Gensites are located at a topographically lower elevation than the subjectsite. Based on the location of these two (2) RCRA Non-Gen sites fromthe subject site and given that no violations have been reported, thetwo (2) RCRA Non-Gen sites are not considered to pose a threat ofimpact to the subject site. Therefore, the RCRA Non-Gen sites are notconsidered RECs with respect to the subject site.EDR also reported no state Solid Waste Facility/Landfill Sites within 0.5-miles of the subject site based upon a state file updated as of January 1 ,2010.EDR indicates that there is one (1) state-listed dry cleaners within theASTM standard distance of 0.25-miles based on the Massachusetts DEPdry cleaners database of January 28,2010. The dry cleaner is identifiedas Grace Cleaners, Inc. with an address of 33 Riverside Drive. The drycleaning facility is located approximately 1,000 feet to the southeast of thesubject site. Based on the EDR report, this dry cleaning facility is located

    EDR reported that there are seventy five (75) release sites located withinone-mile of the subject site. Based on the EDR Release database datedFebruary 12, 2010, most of these sites are located at a distance equal. toor greater than 0.25-miles from the subject site, and based upon theUSGS quadrangle map, the majority of these sites are indicated to belocated topographically cross-gradient or down gradient with respect tothe subject site. In general, most of the sites are not considered to posea threat of impact based upon their location and distance from the subjectsite, the type of release, and the remedial activities undertaken.However, one (1) of these disposal sites is located approximately 500 feetof the subject site. Therefore, based on their proximity to the subject site,this release site may have the potential to impact the subject site and wastherefore assessed for its potential to be an REC. Files for this releasesite was therefore reviewed for information relative to its potential toimpact the subject site, and the information is summarized below in theMassachusetts DEP Records Review section.

    The current Massachusetts DEP database, updated through April 16,2010, indicates that the subject site located off of Governors Avenue inMedford, Massachusetts is not a DEP-listed release site.

    As noted above, according to the EDR review of state databasesupdated through February 12, 2010, a total of 75 DEP-Iisted releasesites are located within the ASTM search radius of one-mile from thesubject site. Most of the DEP-listed sites are located at distances greaterthan 500 feet from the subject site and some are located topographicallycross gradient or down gradientwith respect to the subject site. With theexception of one (1) release site that is located less approximately 500feet from the subject site, the DEP~listed sites are not considered to posea threat of impact to the subject site based on the response actionsperformed, their distances from the subject site and local topography.

    This release site is located approximately 500 feet to the southeast of thesubject site. According to the EDR report, this site is locatedtopographically downgradient with respect to the subject site. Basedupon our review of DEP's on-line files, a release of tetrachloroethylene(PCE) to soil and groundwater triggered a 2-hour release notification tothe DEP on May 23, 2007. The DEP on-line database indicates that aPhase i Initial Site Assessment Completion Statement, a TierClassification Report, a Release Abatement Measure (RAM) Plan and anImmediate Response Action (IRA) report were submitted to the DEP onAugust 10,2009, August 27,2009, November 19,2009 and February 24,2010, respectively.According to a report entitled "Immediate Response Action Status Report#6,8-14 Forest Street, Medford, Massachusetts" dated February 24,2010 and prepared by IES, Inc., the former presence of a dry cleaningfacility is the source of the PCE release. The report indicates that thegroundwater gradient in the area of this release is to the south towardsthe Mystic River. The report indicates that a sub-slab depressurizationsystem (SSDS) has been active at this site since January 7,2008 andindoor air testing had been conducted at this site in November 2007, April2008, and March and December 2009. The report concludes that nocontaminants potentially associated with this release were detectedabove laboratory detection limits in the indoor air samples collected inDecember 2009. Further, the report indicates that IES proposes aRelease Abatement Measure (RAM) which would consist of excavationand on-site treatment of up to 400 cubic yards of contaminated soil, alongwith pumping and treatment of contaminated groundwater encountered inthe excavation.Given that this release site is located topographically downgradient to thesubjectsite, this site is not considered to pose a threat of impact to the

    Plan, Figure 2, which is based on the existing conditions plan provided tous by Samiotes Consultants, Inc.The boring.s were performed by a truck-mounted drilling equipment using3-1/4-inch hollow-stem augers. Standard 2-inch 0.0. split-spoonsamples and standard penetration tests (SPT) were typically obtained at5-foot intervals of depth. The borings were terminated at depths rangingfrom 12 to 27 feet below the existing ground surface within a glacialoutwash deposit. A groundwater observation well was installed to adepth of 24 feet below the existing ground surface in completed boreholeB-203.Boring logs prepared by Carr-Dee Corp. are presented in Appendix Ffollowing the text of this letter. Our Groundwater Monitoring Report forboring B-203 (OW) is enclosed as Appendix G.The borings were monitored by a representative of McPhail Associates,Inc. who performed field layout, prepared field logs, obtained and visuallyclassified soil samples, made minor relocations of the explorations anddetermined the required exploration depths based upon the actualsubsurface conditions encountered. The approximate field locations ofthe borings were determined by taping from existing site featuresindicated on the above-referenced existing conditions plan. At thecompletion of driling, the plan location and existing ground surfaceelevation at each borehole was determined by representatives ofSamiotes Consultants, Inc. and indicated on the existing conditions planprovided to us.In addition to the recent subsurface exploration program, existingsubsurface information was obtained from a geotechnical engineeringreport prepared in 1982 by McPhail Associates, Inc. The report includedlogs of nine (9) borings performed in February 1973 and logs of two (2)borings conducted in July 1982. The location of the previous exploraUons

    The borings performed across the site indicate the bituminous concretepavement to be underlain by a granular fill deposit that varies from about3 to 9 feet in thickness. The fil deposit consists of a loose to very densebrown sand and gravel with a trace of silt. The recent borings alsoencountered several concrete slabs associated with previous sitedevelopments that varied from about 8 inches to 2 feet in thickness.Underlying the granular fil deposit, the borings encountered a 15 to 50-foot thick, dense to very dense glacial outwash deposit. The glacialoutwash deposit consists of a brown sand and gravel with a trace of silt,varying with increasing depth to a sand and gravel with some silt. Theglacial outwash deposit was also observed to contain varying amounts ofcobbles.Based on the 1973 subsurface investigations, the proposed garage site isanticipated to be underlain by a granitic bedrock at depths of 27 to 50 feetbelow the existing ground surface.Groundwater observed during the advancement of the hollow-stem augerboreholes in 1982 varied from about Elevation +13 to Elevation +14.4.Stabilzed groundwater levels recorded in B-203 (OW) ranged fromapproximately Elevation +15.0 to Elevation +15.3. It is anticipated thatfuture groundwater levels across the site may vary from those reportedherein based on such factors such as normal seasonal changes, runoffduring or following periods of heavy precipitation, and alterations toexisting drainage patterns.

    The Massachusetts Contingency Plan (MCP) 310 CMR 40.0000implemented on October 1, 1993 and most recently revised onDecember 14,2007 established "... requirements and procedures fornoti.fying the Department of releases and threats of release of oil and/orhazardous materiaL" The MCP defines categories for soil and

    Soil samples obtained from the borings were screened for the presenceTotal Volatile Organic Compounds (TVOC). The TVOC screening resultsare summarized in Table 1.The heads pace screening was performed in accordance with DEP's "JarHeadspace Analytical Screening Procedure," Attachment II to the InterimRemediation Waste Management Policy for Petroleum ContaminatedSoils, #WSC-94-400. The screening was performed with an MiniRae-3000 Photoionization Detector equipped with a 10.2 eV lamp.A total of twelve (12) soil samples were screened for the presence ofTVOC. The headspace screening indicated TVOC readings ranging fromo to 1.2 parts per milion(ppm) with no visual or olfactory indications ofthe presence of OHM. In the absence of visual or olfactory indications ofthe presence of OHM, TVOC results below 10 ppm are not consideredlikely to indicate the presence of a release of OHM.

    The soil analytical results are included in Appendix G and aresummarized in Table 2. The results of headspace screening, and visualand olfactory observations were used to determine the selection of soilsamples that were submitted to the laboratory for analytical testing andthe analyses performed. Also, the soil samples were selected to .accesspossible soil impacts due to the current and former presence of anundertaker located to the southeast and downgradient of the subject site.Soil sample 8-1 obtained from boring B-203(OW) at a depth of 0.5 to 2.5feet below ground surface and soil sample S-1 obtained from boring B-201 at a depth of 0.5 to 1.4 feet below ground surface were eachanalyzed for the presence of total metals (arsenic, barium, cadmium,chromium, lead, mercury, selenium, and silver) and volatile organiccompounds (VOCs).

    A Phase 1~I Environmental Site Assessment has been completed for thepropert identified as the Proposed Medford Square Garage to be locatedoff of Governors Avenue in the Medford Square section of Medford,Massachusetts. The purpose of this report was to document the potentialpresence of RECs with respect to the subject site. The Phase I portion ofthis report has been prepared in a manner that conforms with the ASTM1527-05 standard for Phase I Environmental Site Assessment (ESA)reporting, as referenced in 40 CMR Part 312 (the All Appropriate InquiriesRule).Our assessment included a visual reconnaissance of the subject site andthe surrounding areas, a review of the site history relative to the possiblepresence of oil and hazardous materials, a review of readily availablemunicipal, state and federal records, and a review of a database searchcompleted by EDR Sanborn, Inc. of Milford, Connecticut in accordancewith the applicable ASTM 1527-05 criteria.In addition, a Phase II investigation was performed to assess the potentialimpacts to soil and groundwater due to the current and former presenceof undertakers located in the vicinity of the subject site.The site has a footprint of approximately 25,850 square feet and currentlyconsists of an existing asphalt paved parking lot that is located betweenGovernors Avenue to the west and Bradlee Road to the east. Thesouthern side of the parking lot is bounded by the rear of commercialbuildings that front onto High Street. The north end of the site is borderedby properties that contains a 5-story brick residential structure and a 1-story brick commercial structure.

    been occupied by residential structures and commercial structuresincluding undertakers since at least 1887. RECs were not identifiedduring our review of historical records of the subject site and surroundingproperties.A site reconnaissance of the subject site was performed by McPhailAssociates, Inc. on April 9, 2010. Surfcial observations of readilyobservable portions of the subject site did not identify RECs. Visualobservations of surrounding properties were made from outside of thepropert limits. No stains or odors were observed in the vicinity of thesurrounding properties. No RECs were observed on the propertiessurrounding the subject site.A search for records pertaining to the storage of oil and hazardousmaterials at the subject site and surrounding properties was conducted atthe City of Medford Assessors' Office, City of Medford Engineering Office,City of Medford Building Department, City of Medford Board of Health andthe City of Medford Fire Department Fire Prevention Bureau and theMassachusetts Fire Marshalls on-line database. The files reviewed didnot contain records for the subject site or surrounding properties whichwould indicate the presence of an REC.Research of Federal and State records was conducted by EDR Inc. ofMilford, Connecticut, and is summarized in a database report datedMarch 30, 2010. The report includes a records search of federal andstate database information indicating potential environmental matterswithin ASTM-established minimum search distances. Based on ourreview of the EDR report, the subject site is not a DEP-listed MCP site.Further, a review of the information provided in the available databasessearched by EDR indicated that the properties located in the vicinity ofthe subject site did not pose a threat of impact to the subject site andtherefore are not considered RECs.

    Accordingly, the release site is not considered an REC with respect to thesubject site.To assess possible impacts to soil and groundwater due to the currentand former presence of an undertaker located to the southeast anddowngradient of the subject site, selected soil samples obtained from ourrecently conducted subsurface investigation were tested for the presenceof total metals and VOCs and a groundwater sample obtained from thenewly installed monitoring well was tested for the presence of dissolvedmetals and VOCs. The analysis did not detect the presence of the abovementioned contaminants of concern at concentrations above theapplicable MCP reporting thresholds. Therefore, based on these results,the former presence of undertakers located in the vicinity of the subjectsite is not considered an REC.We have performed a Phase I Environmental Site Assessment inconformance with the scope and limitations of ASTM Practice E 1527-05for the propert identified as the Proposed Medford Square Garage to belocated off of Governors Avenue in the Medford Square section ofMedford, Massachusetts. This assessment has identified no recognizedenvironmental conditions in connection with the Subject Site.
