A CH-75E Caterpillar Challenger Tractor towing a pipe line tamer trailer was stringing 110mm HDPE water pipe in a gas gathering area. When reversing the line tamer into position on an incline to commence unloading two coils of pipe, the Tractor engine stalled and the machine rolled forward down the incline for more than 250m. It narrowly missed a 36 tonne Hitachi excavator and came to rest in a creek bed.Nobody was injured and equipment damage was minor.

Uncontrolled movement of a CH-75E Caterpillar Challenger Tractor occurred when the engine stalled and the brake accumulator failed to provide emergency braking to the hydraulic system. This could have resulted in a collision with people, other mobile plant, or gas gathering equipment, potentially causing severe injury, fatality and/or equipment damage.

✔ Experienced operator.

✗ Incomplete pre-start checks.

LearningsThe brake accumulator had no nitrogen gas pressure to provide emergency braking to the hydraulic system when the engine stalled.

The brake was not tested in accordance with the operating manual prior to starting the machine.
