GUIDELINES FOR COMPLETING THE ASSESSMENT FORM

This form should be completed by the supervisor alone and then discussed with the student. Please choose an individual mark for each assessment category from 0 – 10 reflecting the

student’s level of ability. Add the totals and place in the final mark box. You may leave categories unmarked if you feel you are unable to assess the student. The final average and grade awarded will take this into account.

85-100 An area of strength for this level of training. This grade may also be given to recognise particular originality or creativity.

75-84 Superior performance together with efficient organisation and selectivity.

65-74 Good performance showing more than satisfactory achievement for this level of training.

50-64 Satisfactory performance.

0-49 Unsatisfactory performance (Fail)
