TRANSITIONAL LIVING HOUSE
• Serves 3 people who need a temporary place to live• Staff Support at key times of the day or as needed• People can stay as long or as little as they need• Length of time based upon individual circumstances.• Consumers sometimes need a temporary living arrangement when:
– Leaving an inpatient unit and need a bit of time before independent housing is available.
– Leaving an LTSR and not quite ready for independent housing.– Leaving a Group Home or CRR and need some time before
transitioning to independent housing.– Someone is homeless for a period of time and need a place to live.– Mobile meds are available to residents– Case Management is provided– Bus lines are nearby– Laundry facilities available at residence– One block from the Drop in Center

HAPPY at HOME in LAWRENCE COUNTY
• Lawrence County has assisted individuals in returning home• These individuals have demonstrated independence and are enjoying being back
home. Each person discharged has told us what they enjoy the most about being back in the community:
• Walks on the property and being able to go on outings.• Reading books and taking long baths.• Fishing and having the independence of being able to walk down town
to get a cup of coffee or a haircut.• Spending time with her husband and family• Going shopping
• The individuals discharged through the MRSAP have thrived in Lawrence County. Lawrence County is proud in the fact that these individuals have required ZERO inpatient hospitalizations or more restrictive local treatment since our first discharge in July of 2006.
