Welcome to our Safety First Brief for May.
Welcome to this month’s Safety First Briefing, please take a few minutes to read through and familiarise yourself with the information here.
Over the last few months we have had instances of staff leaving site without signing out, which has caused ourselves and our clients major issues. It is vital that you sign in and out of every shift, to ensure we can account for your whereabouts.
Every office has an on-call number and is monitored 24/7 so if you have any problems before, during or after your shifts, this should be your first point of contact. If you are unaware of the on-call number, call your local office and they will advise.
We’ve had an increasing number of vehicle accidents, so I would urge you to be cautious and stay alert at all times when travelling to and from work.
There are some contact details and numbers for advice on drug and alcohol related issues later in the briefing. We recognise that there can be times when we all could use some help or advice, so feel free to contact me if you want to discuss any issues, or use the independent sources and advice on offer.

Accidents and IncidentsDuring April there were 3 accidents and 4 incidents reported:
1. Accident Details
On 01/04/2018 at Kiverton the IP was helping to load a van with equipment when a lighting pole struck his face causing asmall cut on the nose.
Investigation
The investigation found that the placement of one item of equipment caused another piece to raise up and strike the IP onthe face.
Recommendations
Care to be taken when loading equipment or items into van to ensure that they are placed in appropriate order to preventsimilar injury.
2. Accident Details
On the 16/04/2018 at Devonshire Road Compound the IP was helping to move a drum of cable close to a fenced area. The IPwas restricted in movement & unable to move his foot when the drum rolled - causing bruising & swelling to the upper foot(behind the toe protection).
Investigation
The investigation indicated that the position that the IP was standing in relation to the fence meant he was unable to movehis foot away in time to prevent the injury.
Recommendations
Walk through undertaken on site with client to assess the action and activity, and see if work review required.
3. Accident Details
On 19/04/2018 at NWEP Phase 3 the IP was helping to move equipment when a piece of rail fell over, and caused the finger on the left hand to be trapped and pinched.
Investigation
The investigation found that the cause of the accident was due to the rail moving in relation to the IP’s fingers.
Recommendations
All staff to consider where they are placing their hands and feet when moving equipment, and if the method of handling plant or equipment is suitable.

1. Incident Details
On 02/04/2018 at Liverpool an RRV with a brush box attachment moving through the site struck & damaged a cable.
Investigation
The investigation indicated that the RRV was positioned to pass through with the brush box on in the wrong direction, meaningthe MC could not see the cable until after they had passed it. The MC acknowledges he should have moved the RRV to anotherposition to allow it to turn and face the correct direction.
Recommendations
Machine Controller to be re-briefed on correct processes.
2. Incident Details
On 15/04/2018 on an ABC project an ISS staff member was MC for a MEWP which passed through points which were set againstthe machine movement causing a points run-through and some damage to the points.
Investigation
The investigation highlighted that the MC had walked through the points twice previously, and had instructed the Points Op toleave set in current position as they would move through shortly. The Points Op changed the setting of the points withoutinforming the MC, who was not in position to notice they had been changed. The MC acknowledges he should always check thepoints, but investigation showed the Points Op also did not work to correct the procedure.
Recommendations
Machine Controller is to undergo refresher training.
3. Incident Details
On 15/04/2018 at Perth Station a MC for RRV passed through points causing damage - although both MC and Operator claim to be unaware of any damage to points. Alleged damage was discovered towards end of the shift.
Investigation
The investigation is currently ongoing and the MC competence has been taken down on Sentinel pending investigation. Recommendations to be communicated following close-out of investigation.
4. Incident Details
On 27/04 2018 at Cadder Yard 2 x ISS staff members were involved in an argument which resulted in one individual striking the other in the face.

Safety Related Reminders:
Since the last brief we have had close calls raised on our staff for failing to sign in with SACS with adequate & correct PPE. Please ensure that when you attempt to sign in for site that you are wearing full orange PPE including cut level 5 gloves, safety glasses, cap lamps, safety boots & FFT mask (when required).
Please ensure that when you arrive on site that you have your sentinel card in order for the COSS & SAC to complete a Sentinel check.
All over the counter & prescribed medication must be checked by the chemist-on-call system to ensure that you are permitted to resume normal duties whilst taking the medication. You must inform your line manager of any new medication that you are taking.
Ensure that you adhere to the lifesaving rules at all times including the use of mobile phones whilst driving.
Good Practice – We also had positive feedback/good practice raised for B Shilling as he checked all poles and testers, including using Proving Unit as per protocol (All in date and in good condition). He was also praised for his COSS brief as he demonstrated excellent knowledge.

Axle Counter Damage at Abergele This bulletin is for the attention of: All staff and sub-contractors
The site was follow up works at Abergele following the completion of the core works at the weekend. The Down Main was being ballast brushed by RRV in readiness for planned tamping works later in the week.
The RRV was brushing LME to HME through the site and when it approached an axle counter head (in advance of signal FH6109) it lifted the brush to avoid hitting it.
The brush dropped ballast around and onto the head of the axle counter, and as the RRV then proceeded over the top of it the RRV crushed the ballast down into the axle counter head. This can be seen from the damage caused.
Actions Required
• RRV is to stop short of axle counter by minimum of 1m.
• RRV operator must raise the ballast brush enough so that the crane controller and operator can watch RRV wheels as they ride over the axle counter head.
• Crane controller to do a visual inspection before the wheels of the RRV come into contact with the axle counter head that no ballast has been dropped.

Safety, Health & WellbeingDrugs & Alcohol
To improve safety and welfare whilst at work we are committed to enforcing a strict Drug & Alcohol Policy and as part of this policy we commit to undertaking random drug & alcohol tests on at least 5% of our sponsored workforce.
The consumption of alcohol, even in small quantities, and the consumption or use of any drugs may adversely affect a workers safety, performance, conduct or efficiency as well as the safety and wellbeing of others.
Whilst working on track there can be huge implications if an individual is under the influence of drugs or alcohol if you were for example to be operating tools or machinery, acting as a lookout or making decisions that could impact the safety of others.
Network Rail alcohol limits are less than legal limits so although you could potentially drive to work and be under the legal limit but could get to work and be tested and found to be over the acceptable limit. As a consequence your PTS could be revoked for 5 years, your sentinel card would be cancelled and you may be subject to disciplinary procedures.
Every individual you work with impacts your ability to be safe – would you want to work with somebody who you believed could potentially be under the influence of drugs or alcohol and put you or your other colleagues in harms way? Some of the affects of drugs & alcohol include:
• Reduced coordination & slower reaction• Short term memory loss• Impaired concentration• Mood swings/paranoia• Irritability & tiredness
These can all lead to a decrease in performance and the potential for accidents to happen whilst at work.
If you think you may have a problem with drugs and alcohol, recognise or are experiencing any of the side effects listed above you can speak to your line manager to discuss support options available to you.

EnvironmentalEnergy Management
To try and reduce our carbon footprint and our carbon emissions we need to ensure that we are optimising opportunities to save energy & water whilst we work. That are lots of small measures that can be implemented to reduce energy, carbon emissions & save money:
Conserving Electricity
The biggest contributors to minimising electricity consumption can come from the following:
Maximising lighting efficiency by-
1) Turn off lights when not in use2) Using natural light when possible3) Switching to energy efficient lighting e.g LED bulbs
Minimising electricity consumption whilst using a computer by-
1) Switching them off when not in use2) Change settings on monitors to put them into sleep mode when not in use3) Reducing the brightness of your screen to a reasonable level4) Closing down programmes that are not in use5) Unplugging laptops from charge when the battery is full
Minimising electricity consumption whist using Heating/Cooling & Other Equipment by-
1) Turn off machinery, tools or other electrical equipment when not in use2) Switch off heating or air conditioning units when not in use (do not tamper with the settings on thermostats
or radiators)3) Wear layers of clothes – allowing you to add/remove layers before turning on heating or air conditioning4) Do not open windows or while heating or air conditioning is on
Conserving Water
Minimise water consumption by-
1) Tightly close & turn off taps when not in use2) Report leaks in pipes/hoses etc. as soon as possible3) Look out for water saving opportunities at all times4) Only use the small flush on toilets if available5) Only turn on dishwashers/washing facilities once they are full to maximise on potential energy efficiency
