Keep your keiki’s health care on track.

It’s important that they see their primary care provider (PCP) regularly, not just when they’re sick. The Early and Periodic Screening, Diagnosis, and Treatment (EPSDT) program can help keep them healthy and fit. The program is a benefit of their HMSA QUEST Integration plan, so you pay nothing!

With EPSDT exams, your child’s PCP can:

• Monitor your child’s growth and development.

• Discuss nutrition, physical activity, and safety.

• Provide help for any medical or behavioral problems.

• Provide immunizations.

How much does it cost?There’s no cost for EPSDT exams for QUEST Integration members up to age 21.

How often should my child get an exam?

Schedule EPSDT exams for the following ages:• 14 days• 1, 2, 4, 6, 9, 12, 15, and 18 months• 2, 3, 4, 5, 6,

and 8 years• 10, 12, 14, 16, 18, and 20 years

If you have diabetes, it’s important that you get your eyes checked once a year or as often as your PCP recommends. Even if you aren’t having eyesight problems now, vision loss can go unnoticed until it becomes serious and more difficult to treat.

Ask your PCP if you should see an eye specialist who can help:

• Test your vision with a retinal eye exam.

• Correct any problems.

• Prevent new problems.

Beat diabetes before it beats youPrevention is the key to controlling diabetes.

Three things you need to check:

Your PCP is your go-to person when you’re sick, for preventive screenings, and to answer general questions about your health. But there may be situations when your PCP refers you to a specialist who can treat a specific health condition.

When do I need a referral?Specialists your PCP may refer you to:

• Allergist for allergies.

• Dermatologist for acne.

• Cardiologist for heart disease.

Do I always need a referral to see a specialist?No, not always. Here are some services that don’t need a referral:

• Behavioral health services from a licensed psychiatrist, clinical social worker, or family therapist.

• Family planning.

• Vision care, including eye exams and fittings for eyeglasses.

• Women’s health for screenings such as breast exams, mammograms, Pap smears, and pelvic exams.

Find a providerIf you need a specialist, make sure you choose a specialist in the HMSA QUEST Integration provider network. To find a specialist, go to HMSA’s website at hmsa.com and click Find a Doctor on the home page.
