Preparations

Please have the following available before you get

started on your claim:

• Black or blue ink pen

• Piece of paper to write notes on

Part 1: Claimant Information

#1: Be sure that your first and last name is spelt correctly. If it is not then draw a

single line through the misspelled word and rewrite it. If there is more than one

person on the title of the home then add the second homeowner to line B if

they are not already listed on line A.

Part 2: Property Identification

# 2 & 3: Be sure that your address is all spelt correctly. If it is not then draw a single line

through the misspelled word and rewrite it. Also if they have not filled in your county,

you need to do that as well.

# 4: List one telephone number that is best to contact you at.

Part 3: Verification of Ownership

#5: The property Identification number is located on your paid tax receipt. It is also known

as your parcel number. If you do not receive your paid tax receipt because your

mortgage company takes care of it then refer to slide #17 to show you how to look it

up. For the structure of square footage you can estimate how much you have if you

don’t know the exact amount.

#6: You must pick either A,B, or C to provide with your claim. You CAN NOT provide option

D because you do not live in a mobile home. If u choose either a mortgage coupon or

statement make sure it is current within 30 days.

YOU MUST COMPLETELY DARKEN IN THE CIRCLE FOR THE OPTION THAT YOU CHOOSE; ALONG

WITH THE REST OF THE CLAIM

#7: Go ahead and fill in no because even if you are associated with a home owners

association they are not responsible for the outside of your home; and if you were to

put yes you would have to provide the bylaws for the association along with mailing

address and contact name.

Part 4: Product Identification

#8: You are going to need to provide a 6inch by 6inch sample cut of your siding. If you do

not want to cut your sample piece yourself then you can call our office and set up an

appointment for one of our contractors to come out to your home. So you need to fill in

option A along with the two photograph options right below it.

Part 5: Prior Claims

#9: If you have never filed a class action or warranty claim before then go ahead and fill in

no and move on to the next slide which is slide #9.

If you have filed a previous claim then fill in yes. If you do not know your previous

claim number then you can leave it blank, but you need to fill in the year that you last

filed.

Part 6: Property Information

#10: Put the year that you bought your home. If you do not know the month then you can

leave that blank.

#11: If you are the first homeowner of your property fill in yes; if you are not fill in no but you

DO NOT have to list the previous homeowners and the date they purchased the home.

Part 7: Product Description

#17: The Masonite product was installed on your home the year your house was built. You

also need to provide proof of installation date. The easiest for you to obtain if you do

not have permit paperwork is option E which is assessor record. Refer to slide #17 for

further information on how to get this.

#18: This is the same answer as number 17; the year your home was built

#19: I’m going to give you some information and it is your choice weather to use it or not. If

you put that you painted your home 5 or 6 years ago then you will be fine. If you put

that you have painted your home longer than 6 years ago then you will get hit with

paint deductions, meaning they will take money from your claim. They do not ask for

any proof of when you painted either so it is up to you what you want to put as your

answer.

#20: Write down the name of your subdivision if you have one.

#21: How to get the square footage of Masonite is to add about 500 square feet to how

much square feet you have inside your home.

#22: Put down that you have at least 75% of damage unless you think you have more.

#23: There is no right or wrong answer here, just put a year you started to notice the

damage.

#24: You need to copy this down: edge checking, thickness swell, swelling around nail

heads, waxing, bleeding, and buckling

Part 8: Property Floor Plan

#25: Fill in the direction that

the front of your home

faces.

Part 9: Inspection Information

#27: Fill in yes if you have a dog or no if you do not have a dog.

#28: Fill in yes or no depending on your property.

#29: Fill in yes or no depending on your property.

#30: You can sketch out directions from the nearest highway to your property or if you are

familiar with google maps or mapquest you can print out a map with directions.

Part 10: Photos

#31: Here is a detailed list of all the photos that you will

need. You need to fill in letters A through I. Do not fill in J,

K, or L. If you do not want to take the photos yourself we

can do it for you for $8.

Part 11: Agreement

#32: Now all you have to do is have all homeowners print, sign, and date the

form and it is finished!!!

Paid Tax Receipt

Jackson County

– Go to www.jacksgov.org

– Scroll to the middle of the paid and on the left hand side you will see a link that says

Taxes, under that link you will see another one that says Print Tax Receipt. Click on it.

– You will use the second search box that says “Owner Name Search”. Be sure to put

your Last name first followed by your first name with an asterisk next to it.

– Your parcel number will be the longest number with all the dashes in between. (xx-

xxx-xx-xx-xx-x-xx-xxx)

– Once you click on your parcel number it will bring up your paid tax receipt; you

can print this off and use it as #6 (page 1) for option A ; copy of property tax bill

Johnson County

- Go to http://treasurer.jocogov.org

- Click on the top menu bar that says “Real and Personal Property Taxes”

- On the left hand side click “View and Pay Tax Bill”

- A disclaimer page will pop up and you need to click yes.

- Search under Real Estate on the Left hand side

- Go to http://collector.claycogov.com

- You will use the second search box that says “Owner Name Search”. Be sure

to put your Last name first followed by your first name with an asterisk next to

it.

- Go to http://lang.jocogov.com

- You can print your year built card off here

- Or you can call the Assessor’s office: 913-715-9000

Clay County

- Claycogov.com

- Click enter to go into site

- Click County Government and there will be a drop down menu and click

County Offices

- Click Assessor’s Office

- Click Real Estate on top menu bar

- Click Online Parcel Search

- Click I agree

- Search under property address and fill in Street number and name
