A message to our Residents, Families and

Team members:

I would like to take a moment to introduce

myself to our Residents, Families and Team

members of Golden Years Long term care. My

name is Paul Rektor and I am happy to

announce that as of February 24, 2020, I will

be joining the Leadership team as the new

Executive Director of Golden Years Long Term

care. I have been a lifelong resident of

Kitchener Waterloo and I have been with

peopleCare since 2015.

I have previously supported Golden Years

during the year of 2018, in the role of Senior

Executive Director while working out of the

AR Goudie home in Kitchener.

It is a pleasure to take on this new role, and I

look forward to meeting our resident’s, family

members and the Golden years team over the

coming weeks.

Golden Years, Long term care

News and Notes:

Effective July 1, 2020, red and white health cards will no

longer be accepted as proof of eligibility for insured health

services under the Ontario Health Insurance Plan (OHIP).

Long Term Care Home (LTCH) residents must be insured to

continue to receive health services covered by OHIP.

The conversion to the more secure photo health card is

mandatory, and the ministry is asking LTCH administrators

to continue to assist with the conversion process whenever

possible.

* Please ensure all residents have their address

updated with ServiceOntario within 30 days of

admission. This will ensure all residents receive

important updates regarding their health cards.

LTC Residents with Red and White Health Cards

New and existing LTCH residents MUST convert from red and

white health cards to photo health cards before July 1, 2020.

To convert to photo health cards, proof of Canadian

citizenship/OHIP-eligible immigration status, proof of

residency in Ontario and confirmation of consent are

required. The process for conversion is documented in the

attached Fact Sheet.

LTCH Residents - Original Citizenship/OHIP-Eligible

Immigration Document not available

If a LTCH resident is missing the required document Canadian

citizenship/OHIP-eligible immigration document) to convert

to a photo health card, the family/Power of Attorney (POA)

should be asked to assist the resident to locate or obtain the

missing documents in order to complete the health card

conversion.

For residents who were born in Canada, a replacement

document can be requested from the province/territory

where the resident was born. For Ontario-born residents,

please contact the Ontario Registrar General. For residents

who immigrated to Canada, contact Immigration, Refugees

and Citizenship Canada for information about how to replace

documents.

If the proof of citizenship/immigration document is not

immediately available and there is an urgent need to obtain

the replacement card, please include a notation in the

registration request indicating this.

Specific questions can be directed to ServiceOntario at 1-800-

268-1153.
