1. INTRODUCTION

This report focuses on the South African medical device supply and manufacturing industry, which generates estimated annual revenue

of between R10bn and R11bn. Up to 95% of locally consumed products are imported into South Africa, mainly by large multinational

companies. Locally produced products are either niche products or are exported because local manufacturers cannot match the low

cost of imported products. A weak regulatory framework means that cheaper imported products are often of low quality, an area of

concern to the local industry.

2. DESCRIPTION OF THE INDUSTRY

The South African medical device industry comprises mainly agents and distributors of international manufacturers that import products

into South Africa. Domestic manufacturing is small and focused on the production of low-tech and low-value devices such as surgical

goods and disposable needles. More than 80% of the industry consists of small and medium sized companies competing with large

multi-billion-dollar healthcare companies like Johnson & Johnson, GE, Siemens and Medtronic.

The medical equipment manufacturing industry manufactures a large number and broad variety of devices, tools, instruments and

medical consumables used in hospitals and healthcare institutions. Classification of medical devices is generally based on function, as

shown below.

♦ Diagnostic equipment includes devices such as ultrasound, X-ray and MRI machines or equipment that can be used at home

for specific purposes such as for the control of diabetes.

♦ Therapeutic equipment consists of infusion pumps, medical lasers and radiation therapy machines.

♦ Life support equipment, such as medical ventilators, anaesthetic machines, heart-lung machines, ECMO and dialysis

machines, are used to maintain a patient's bodily functions.

♦ Medical monitors allow medical staff to measure a patient's medical state, including ECG, EEG, and blood pressure and

dissolved gases in the blood.

♦ Medical laboratory equipment automates or helps analyse blood, urine and genes.

♦ Disposable equipment includes sterile sutures, dressings, bandages, syringes and drip tubes to treat wounds.

The complexity of the industry has increased over the last decade because of the introduction of new hybrid devices such as drug-

eluting stents and other implantable devices that incorporate a pharmaceutical component. This applies to new drug delivery

mechanisms as well. Information technology has contributed to this increasing complexity, as monitoring and imaging systems

incorporate telecommunications and data processing capabilities.

2.1. Industry Supply Chain

The South African medical equipment industry consists of multinational manufacturers whose products are imported by satellite

branches, agents or distributors, local manufacturers, wholesalers and retailers. Agents order products on an ad-hoc basis and

generally work on a commission basis while distributors carry larger stock of the product and enter into a formal agreement with the

manufacturer. Some distributors carry only a specific brand, while others may carry a range of non-competing products from different

manufacturers. These distributors will then also have maintenance and after-sales repair capabilities.

End users vary from healthcare providers such as private hospital groups and the public health care sector to individual health

professionals such as general practitioners. The end users of some products are the patients themselves. Some local manufacturers

export their products.

Industries that operate alongside the medical device industry include IT companies, medical equipment maintenance companies, the

clinical engineering industry, the pharmaceutical industry and the chemical industry.

3. SIZE OF THE INDUSTRY

According to Western Cape Minister of Economic Development, Alan Winde, the South African medical device industry was worth

R11.43bn in 2011. However, a survey by the South African Medical Device Industry Association (SAMED) revealed that 47 members

surveyed generated revenue of R8.3bn in 2012 and SAMED estimates that the total South African medical device industry generates

around R10bn in revenue annually. The association represents approximately 160 manufacturers and suppliers of an estimated 300 000

different medical devices. Of the more than 700 local industry players, the association estimates that fewer than 5% actually

manufacture the devices. The industry employs approximately 4000 people.

In 2011, 8.3% of South African GDP was spent on healthcare and in 2012, medical schemes paid out R103.3bn in benefits to scheme

members. Medical devices and medical consumables accounted for 2.5% of total health care benefits incurred by medical schemes in

2012, as shown below. In 2011, medical devices accounted for 4.4% of total healthcare spending and are forecast to account for 4.1%

of total healthcare spending in 2016 and 3.8% in 2012. In a 2012 SAMED survey, 34 companies indicated that 70% of their business

comes from the private health industry.

4. STATE OF THE INDUSTRY

4.1. Local

The need for medical devices is growing because of increased health problems like obesity and changing demographics such as the

increasing age of the world’s population. SAMED reported that approximately two new companies join the association monthly and the

association’s estimate of industry revenue of R10bn per annum shows a 19% increase from 2009.

Approximately 90% to 95% of locally consumed products, by value, are imported into South Africa with a significant portion from large

multinationals such as Johnson & Johnson and Smith & Nephew. Together with pharmaceuticals, medical devices are the 5th largest

contributor to South Africa’s trade deficit. Most imports are from Europe, the United States of America (USA), China, Japan and

increasingly from India. Importers are able to offer products at lower prices because of the economies of scale enjoyed by large huge

multinational manufacturers and the fact that import tariffs are only levied on a few medical devices. Cheaper products, often of a lower

quality, are also imported from China and because of the lack of regulation many of these products are of questionable quality which

may impact on patient health. Although these low quality products often come with little or no after-sales support, local distributors

and agents mentioned that the importers of these products are often awarded tenders to supply the local healthcare market.

Government procurement, often through tenders, accounts for 30% of the business of 34 companies surveyed by SAMED in 2012.

Devices such as implants are procured directly by health professionals or institutions. Tenders are usually awarded on the basis of price

so large multinationals are able to undercut the prices of locally manufactured products. However, a respondent mentioned that product

specification and practitioner preferences also play a large role in the procurement of products. Industry players are often invited to

tender review meetings where stakeholders can advise on the tender specifications. Inefficiencies in the public procurement and

payment system, which have resulted in cash flow problems for many companies, are discussed under Influencing factors.

Local manufacturers tend to focus on the export market. They find that locally manufactured devices are well accepted in some regions

because the quality is high and the prices are competitive, especially when the Rand is weak. Elite Surgical, a company that

manufactures orthopaedic and arthroplasty implants, has found that it can offer its devices in the USA for one-third of the price offered

by American competitors. However, focusing on the export market also has its challenges: the slow recovery of the European economy

and the cost and time involved in achieving international approval and registration of products. South African National Accreditation

System (SANAS) and South African Bureau of Standards (SABS) are not accredited by the USA’s Federal Drug Administration (FDA) and

European regulators. Local manufacturers who want international accreditation therefore have to bring international accreditors to

South Africa, which adds to the difficulty, cost and time involved.

4.1.1. Corporate Actions

The following corporate transactions have taken place since January 2012.

♦ Johnson and Johnson completed its US$19.7bn acquisition of Swiss company Synthes in June 2012. The transaction allowed

Johnson & Johnson to move into the orthopaedic surgery market in the emerging markets of Russia, China and India. To

receive approval from both the US and European regulators, Johnson and Johnson agreed to divest its DePuy Orthopaedics

subsidiary to Biomet for US$280m in cash. Synthes will be integrated with the DePuy franchise to establish the DePuy

Synthes Companies of Johnson & Johnson.

♦ Southern Implants Inc merged with Keystone Dental in June 2012. Southern Implants granted Keystone Dental the licence

to manufacture and distribute its products in North America, South America and Asia. Both entities will retain their original

names.

♦ Covidien plc agreed to acquire Given Imaging for approximately US$860m in December 2013. The transaction will add Given

Imaging’s minimally invasive gastrointestinal (GI) diagnostic and monitoring solutions to expand Covidien’s presence in the

US$3bn GI market. Covidien expects Given Imaging will add between US$40 and US$50 million per quarter in incremental

revenue to the Medical Devices segment. The transaction is expected to be completed on 31 March 2014 after necessary

regulatory approvals.

4.1.2. Regulations

The lack of global uniform regulations for medical devices can threaten the import and export of these devices. Therefore, despite a

device having gained approval within its country of origin, the manufacturer may have to initiate further approval processes in

destination countries. To tackle the issue, the Global Harmonisation Task Force (GHTF) was conceived in 1992 to harmonise medical

devise standards globally. The GHTF disbanded in November 2012, but was replaced by the International Medical Device Regulators

Forum (IMDRF). The World Health Organisation (WHO) is an official observer. In April 2013, new Unique Device Identification (UDI)

system draft recommendations were published by the IMDRF.

Medical devices have to be developed and manufactured under strict quality standards if they are to be exported to Western markets.

For example, for European markets the device must be manufactured according to the ISO13485 quality standard, certified according

to the medical devices directive and Conformité Européenne (CE) marked. Achieving these standards is one of the biggest hurdles that

South African medical device developers face. ISO certification presents two major problems:

♦ It has been suggested that local developers do not have a clear understanding of what is required to achieve ISO13485

standard qualification; and

♦ The cost of achieving ISO13485 and a product CE mark runs into the hundreds of thousands of Rand, a cost which local

manufacturers struggle to afford. CapeRay estimated that the cost for achieving accreditation for the Pandia digital X-ray

camera amounted to about R1m. Annual re-certification costs more than R100 000.

Currently, there are very few regulations applicable to the medical devices industry, with the exception of imaging and In vitro

diagnostics as shown in the table below. To rectify this situation, legislation to amend the Medicines and Related Substances Act 1965

(Act No 101 of 1965) known as Act 101 and the Amendment Act, known as Act 72 has been in progress since 2009. In 2011, a new

proposal to amend Act 101 surfaced, which left the medical device industry with many concerns, which are discussed under Influencing

Factors.

SAMED published the following table containing the three main types of medical devices with varying degrees of regulation in the

current regulatory framework in South Africa.

The draft Intellectual Property (IP) policy document was released in September 2013. One of the main purposes of the policy is to

reform South Africa’s IP laws to adopt an examination standard for all pharmaceutical patent applications to prevent evergreening. This

refers to making small changes to an existing, patented drug just before its patent expires and selling it at a cheaper price than the

originator drug. By doing this, the company is able to establish a market share for its generic before other companies can release their

generic equivalents. The implications of the draft policy are discussed in the Influencing Factors section.

The Consumer Protection Act provisions on product failure and liability are also problematic for the medical device manufacturing

industry. Non-compliance with the maintenance and servicing requirements of medical devices, especially in government institutions,

means that any fault may not lie with the manufacturer. Industry stakeholders have recommended that specific criteria for hospital

managers relating to medical devices should be included in health facility management documents.

The National Health Amendment Act, 12 of 2013 came into effect on 2 September 2013, introducing the Office of Health Standards

Compliance (OHSC). This body is empowered to monitor and enforce health standards in all private and public health establishments.

The OHSC will report directly to the Minister of Health and was formed with the NHI in mind. OHSC inspectors have started with the

audit of health facilities that voluntarily agreed to the inspection. This will be extended to audits on a mandatory basis. Currently audits

relate to infrastructure, but will be extended to include the auditing of medical equipment.

In 2011, SAMED became a member of the Marketing Code Authority (MCA), making them signatories to a single Code of Marketing

Practice, adopted by various Health product Associations in South Africa and intended to facilitate ethical behaviour in the industry.

Health care practitioners, customers and patients may lodge a complaint with the MCA regarding marketing and advertising of

healthcare products.

The new area of mobile technology in medical devices has led to the development of South Africa’s eHealth Strategy 2012/13-2016/17.

The strategy has 10 strategic priorities to leverage eHealth to strengthen healthcare transformation.

4.1.3. Black Economic Empowerment

The amended BBBEE codes which replaced the BEE codes of Good Practice were gazetted on 11 October 2013. Companies have a one

year transitional period in which to become compliant.

SAMED commented that the revised code penalises certain industries and would be unachievable or hard to achieve within the

stipulated one year. This would result in a significant drop in some BBBEE scores while others would become non-compliant.

♦ The revised codes place more emphasis on local procurement. Therefore companies and health facilities that rely on imports

will be negatively affected.

♦ Medical device importers will not be able to comply with local content requirements and because of the lack of skills and

technology, it is also unlikely that local manufacturing plants will be established.

♦ Companies will not be able to comply with the training aspect of the BBBEE code. Medical device companies engage

extensively in product training in the health sector. However, the training is device specific which does not comply with the

scorecard requirements that skills development is implemented within accredited courses.

The Preferential Procurement Policy Framework Act (PPPFA) stipulates that when government assesses contracts, it must take into

account a preference point system which prescribes functionality, price and reconstruction development programme (RDP) goals. This

practice is believed to have created an intermediary sector of empowered importers, often bringing in goods cheaply from China. Under

these circumstances the added margin still undercuts the price of locally produced goods. Thus import companies, who employ a

fraction of the staff that local manufacturers do, are able to secure government contracts on the basis of price.

The 2011 SAMED Industry Survey showed that approximately 63% of respondents surveyed had been rated by a BEE verification

agency and the majority (22%) of these had a level 4 rating. The current BEE ratings of some of the industry players are shown below.

4.2. Regional

The African market with a population of over a billion people represents a vast potential market for health care products. The market

for medical devices has grown in recent years due to increased income, urbanisation and a growing middle class. In countries such as

Mozambique and Angola where years of civil war mean there are many amputees, there is high demand for prosthetic limbs. Currently,

African countries rely mainly on imports and the fastest growth in the medical device market has been in western and northern Africa.

According to SAMED, South Africa-based companies are focused on expansion in Africa and have established distributors and agents.

More international companies are also looking to enter the South African market, often exporting their products or product components

from South Africa to other African countries.

The total health expenditure in Africa for 2012 was estimated at US$117bn with South Africa (29.1%) and Egypt (10.9%) as the largest

health markets on the continent. Public health expenditure at US$55bn in 2012 is highest in South Africa, Algeria, Egypt and Nigeria,

but in most African countries, public health expenditure is very low. This results in high out-of-pocket expenditure on health services.

The regulation of medical devices in most African countries is neglected and this is compounded by the lack of technical human

capacity, infrastructure and financial resources. The Pan-African Harmonisation Working Party (PAHWP) was conceived in 2012 and

stakeholders include regulatory authorities, the medical device and diagnostics industry representatives, RECs, WHO, NEPAD, London

School of Hygiene & Tropical Medicine and the African Society for Laboratory Medicine. The first meeting of the East African Community

(EAC) Task Force on the regulation of medical devices and diagnostics was held in April 2013 in Dar-Es-Salaam where the proposed

structure of PAHWP was approved. The structure was presented to the African Union/NEPAD Agency African Medicine Harmonisation

Advisory Committee meeting in June 2013 in Nairobi, Kenya. The harmonisation of IVDs is the priority of the Task Force.

4.3. International

The global medical device industry is highly fragmented with about 10 000 general device groups and approximately 85 000 different

products available. US-based companies dominate the US$350bn global medical device industry and 32 of the 46 medical device

manufacturers with annual revenue higher than US$1bn are based in the USA. The largest markets are the USA (40%), China and

Japan. Growth in established markets in the European Union, the USA and Japan has been moderate in recent years, while there has

been substantial uptake of medical devices in emerging economies. China, in particular, has shown an annual growth rate of 15% to

20% depending on the product. As a result, emerging economies are being targeted by medical device manufacturers. The USA is now

the largest exporter of medical devices to China, accounting for 30% of medical devices imported in 2011. It is forecast that the

Chinese medical device market alone will reach US$30.6bn of the expected global total of US$302bn by 2017. By 2020 the Chinese

market is expected to grow by 15% annually to reach about US$43bn.

The Affordable Care Act in the United States is expected to have a large impact on the industry. At least 30 million people are likely to

receive medical insurance by the end of 2014, and this could have a positive impact on the demand for devices. To help fund the new

act in 2013 the government introduced a new 2.3% excise tax levied on the total revenue of medical device companies, regardless of

whether the company generated a profit. In response to these developments some medical device companies have adopted new

business models. Medtronic, for example, formed Medtronic Hospital Solutions which works with hospitals to improve operational

efficiency and patient outcomes. Medtonic is managing and modernising cath-labs for two European hospitals while Royal Philips

formed a US$300m, 15-year alliance with Georgia Regents Medical Centre, ensuring that the Philips’ imaging systems, patient

monitoring and clinical informatics solutions are effectively used.

5. INFLUENCING FACTORS

5.1. Regulatory Environment

As noted previously, in South Africa there is no comprehensive regulation on the registration of medical devices so any person or entity

is able to import or manufacture and sell any device without the imposition of safety and quality controls. However, the government is

in the process of drafting regulations to monitor the quality of products used locally. In 2009, legislation to amend shortcomings in Act

101 introducing a comprehensive system for the regulation of medical devices was introduced. Unexpectedly, however, in July 2011,

the Department of Health (DoH) published new draft regulations relating to medical devices which require the licensing of all medical

devices. In December 2011, cabinet decided to return Act 72 to parliament for an amendment to remove the definition of ‘health

product’. A long-standing concern about Act 101 and Act 72 is that it applies regulations on medical devices and IVDs in the same

manner as medicines. For example, it assumes the supply chains of medicine and medical devices and IVDs are similar. According to

respondents, the lack of government participation in international harmonisation initiatives is also an area of concern.

Role players believe the National Health Act needs to be amended before Act 101 and Act 72 are finalised. SAHPRA will replace the MCC

which registers new medicines and medical devices and although it was expected that the body would be operational by April 2013, the

draft amendments were only submitted to a Cabinet committee in June 2013. Currently, the MCC’s policies are not adequate to include

the complexities of medical devices. For example, combination devices are currently erroneously regulated as pharmaceuticals. The

long registration process at the MCC also means that by the time the MCC registers the process, the product is outdated. Thus the slow

adoption of new processes and legislation hamper the growth of the local industry.

Proposed changes South Africa’s intellectual property (IP) laws will also have an effect on the medical device industry, which is being

treated as part of the pharmaceutical industry. SAMED mentioned the following concerns regarding the draft policy.

♦ Medical devices have very short product life cycle and investment recovery periods at approximately 18-24 months on the

market compared to a long development and investment recovery period of pharmaceuticals.

♦ The IP policy should take into account that medical devices undergo rapid incremental development and innovation. For

example, in 2012, more than 10,000 patent applications were filed with the European Patent Office in the field of medical

devices compared to 5,400 for pharmaceuticals.

♦ Medical devices may contain more than one component and each may be subject to different types of IP protection.

♦ Medical devices are intrinsically related to operator/user training and skill and maintenance requirements. Both training and

maintenance have distinct IP properties in the form of copyright, design and trademark protections so any change in the IP

protection of any component of a medical device can affect the whole device.

♦ Any extra delays in IP registration could result in the reluctance on companies to import devices or to manufacture them

locally. This would negatively affect investment in research and development and the rate of innovation in the local industry

and has the potential to negatively affect patient care.

5.2. Government Procurement

South African medical device suppliers often are in debt with their international suppliers because they have not been paid by provincial

health departments. Gauteng’s Health Department in particular, has been cited as a department with a poor payment record and

problems have also been experienced in KwaZulu-Natal and the Free State. The Gauteng Health Turnaround Strategy 2012 – 2014 was

launched in the 2nd half of 2012 in response to the near collapse of health services at the more than 300 health facilities in Gauteng,

after service providers refused to supply the department until about R4bn in unpaid invoices dating as far back as 2007 had been

settled. The payment of suppliers within 30 days is one of the goals of the strategy and was partly achieved by September 2013 when

76% of suppliers were paid, compared to 66% in the first quarter of 2013. However, at 30 September 2013, 1009 companies were

owed R420.9m for more than 30 days. This includes 592 companies which were owed R179.2m for more than 90 days and 498

companies owed R105m for more than 180 days.

Non-payment by government departments means that the contracts of some local suppliers with international partners were cancelled

until their debts were settled. One supplier interviewed mentioned that the company was forced to sell shares while others have had to

retrench staff, institute a ‘hire-freeze’ and arrange loans and overdrafts to stay solvent. Smaller companies, especially, are threatened

by non-payment. Many companies do sue the department for outstanding debts because of a lack of financial muscle and the fear of

victimisation. There have been reports that some companies who have sued the department have lost further business as a result of

their actions. There are also concerns about the integrity of tender adjudication and execution in certain provinces and as a result,

many device suppliers have decided to target private instead of public hospitals.

5.3. Reimbursement Practices of Medical Aid Schemes

Although less than 20% of South Africa’s population belongs to a medical aid scheme, the schemes paid R2.5bn for medical devices and

medical consumables in 2012. Medical aid schemes have a great deal of influence when it comes to the payment of medical services

that involve medical devices and equipment and if the scheme will not cover the cost of the medical device, it is unlikely that the device

will be bought. As medical aid schemes are continually trying to reduce costs, they are more likely to approve cheaper medical devices.

SAMED argues that lower cost products do not always equate to value for the end-user. While some devices appear to be more

expensive, their use could result in savings: for example, reduced operating time; fewer patient post-operative hospital visits; and less

need for operations to be repeated.

Medical device manufacturers find it difficult to convince medical insurance companies to accept new and innovative technologies. As

there is no overseeing body such as the USA’s Federal Drug Administration (FDA) that gives funding guidelines when new medical

technology is made available, medical schemes do not always receive impartial guidance regarding the effectiveness of new technology.

Another problem mentioned was that different medical aid schemes use different methods to determine whether a device will be

reimbursed or not. This causes confusion, especially when the standards and methods used are not made available to the company

concerned.

5.4. National Health Insurance

The NHI, which is currently being debated, is expected to provide opportunities for the medical device industry. The Green Paper on the

NHI was published in August 2011 and the White Paper, which is the next step in policy development before the enabling legislation can

be drafted, still has to be published. The NHI aims to provide healthcare services through accredited and contracted public and private

providers which will require extensive public-private partnerships. It also requires the upgrade of healthcare facilities, which includes

many types of medical devices. The Green Paper does not specify how, for example, procurement under the NHI will take place, which

has led to uncertainty in the medical device industry, especially in light of the preferential procurement requirement of government and

the amended BEE compliance requirements discussed previously. The Green Paper also suggests that profits in the private healthcare

industry would be capped, which has met with opposition from private healthcare providers and their suppliers.

5.5. Economic Environment

The demand for some types of medical devices, particularly those classified as cosmetic by medical aid schemes and therefore not

covered, is lower when economic conditions are tough and disposable income is negatively affected. Most medical devices are imported

and are influenced by the exchange rate, making it more expensive to import when the Rand is weak. This, on the other hand, favours

the exporters of medical devices.

5.6. Innovation

Innovation in the medical device industry is a key success factor. In the current economic climate manufacturers have to prove their

innovations create value and save money for medical aid schemes. Investment in innovation in the local medical device industry is very

low when compared to international figures. For example, CapeRay has only been able to find investment of R20m to develop its

mammography device. The developer of a similar product in the United States has the backing of a large multinational which has

provided US$10m annually for the past 10 years.

The lack of investment can be attributed to a number of reasons.

♦ There are relatively few venture capitalists and angel funders in South Africa. Local innovators and manufacturers mentioned

that because of the lack of competition among venture capitalists, they have to supply a large percentage of equity needed.

South African venture capitalists are also risk adverse and want fast returns on their investment. The development and

registration cycle of some medical devices, however, may take years.

♦ Up to 95% of medical devices by value used in South Africa are manufactured by multinational companies and imported into

South Africa. Multi-national companies therefore do not have an incentive to invest in the development of products in South

Africa.

♦ The healthcare market is complex. The decision to use medical devices has moved from the patient and/or user to the

medical aid scheme, which means that a manufacturer not only has to prove the effectiveness of the innovation but also has

to prove the cost-effectiveness of the use of the device.

♦ Devices have to be registered in overseas markets to be used there, which is a complicated, expensive and lengthy process.

♦ Government is not active in funding innovation in the medical device industry. More information is provided under Research,

Development and Innovation.

5.7. Labour Resources

There is a major global shortage of technical skills to maintain medical devices and this shortage also negatively affects the number of

innovations in the medical device field. The outcome of the use of medical devices often depends directly on the skill or the experience

of the user. Medical knowledge and technical skill are thus necessary to operate increasingly complex medical equipment during

medical procedures.

To improve and expand the skills base in sub-Saharan Africa, Medtronic Africa opened a simulator medical training facility in

Johannesburg. Medical practitioners will be able to practise procedures before attempting them on patients. The facility has facilities to

train cardiac-, spine- and neurosurgeons and two full-time virtual catheterisation labs to practise the implantation of cardiac devices,

such as pacemakers and coronary stents. Various modules are taught by medical professionals at no charge and trainers also

accompany doctors and surgeons to real-life operations, where they offer their expertise and advice. The facility has had an

overwhelming response from doctors from Mauritius, Ghana, Kenya, Uganda and Nigeria. SAMED also offers SETA-accredited training

courses for sales professionals in the clinical environment called the Company Representative in a Clinical Environment (CRICE).

A number of tertiary institutions offer qualifications specifically tailored for the medical device industry. For example:

♦ The University of Pretoria’s Graduate School of Technology Management offers internationally recognised management

development programmes up to a PhD in the fields of Management of Technology, Engineering Management and Project

Management. Certificates programmes and short courses are also offered. These courses build capacity in the areas of

Assessment, Innovation, Management of Healthcare Infrastructure and Technologies and areas of specialisation in Clinical

Engineering and Hospital Engineering; and

♦ The University of Cape Town’s Programme in Healthcare Technology Management offers undergraduate qualifications in the

manufacturing, maintenance and application of the electronic medical equipment in hospitals.

SAMED noted that there has been an increased need for administrative and legal skills to ensure the industry complies with the

regulatory framework. There is for example, a need for staff with expertise in dealing with the provisions of the Consumer Protection

Act as well as for reimbursement managers to deal with medical aid schemes.

6. COMPETITION

The medical devices industry is highly competitive due to the large number of participants and the presence of many substitute

products. Due to challenging economic conditions in Europe and the USA, international players are looking towards increasing their

business in emerging economies, which drives competition. As noted previously, about 90% to 95% of the South African medical

equipment industry is dominated by multinational manufacturers whose products are imported to South Africa. As any entity is able to

import, manufacture or sell any medical device without any regulatory control, manufacturers of high quality, internationally approved

medical devices therefore have to compete with medical devices of inferior quality.

Due to concerns about the rapid rise in healthcare costs, the Competition Commission will undertake a broad market inquiry into the

private healthcare sector to identify factors that are driving cost increases. The first round of public hearings is expected early in 2014

and the final market report is expected in November 2015. Hospital groups and specialists doctors will be the main focus of the inquiry,

but it will also look at the influence of government’s tender processes on product prices in the private healthcare sector. SAMED noted

with concern that the public sector has been excluded from this investigation and requested the Commission to expand the scope of the

inquiry to include government health services. Some industry analysts have suggested that there could be vertical collusion between

hospitals and medical product suppliers in the form of an agreement between hospital and the medical device supplier.

6.1. Barriers to Entry

The following are barriers to entry into the medical device market.

♦ Innovative technology is one of the greatest barriers to entry as high investment is necessary to fund product development.

The new innovation must also be a significant improvement over existing technology to justify the expense of product

development, regulatory approval and marketing.

♦ Convincing medical aid schemes to reimburse the use of medical devices can be a barrier with new or expensive technology.

The manufacturer has to be able to prove that the new device does offer long-term savings, while clinicians and users have to

be convinced that the product offer benefits like faster healing or less pain.

♦ Regulatory compliance in overseas markets is expensive and may take a long time to obtain.

♦ There are many large multinational companies, often manufacturing devices for the same purpose.

♦ The domestic market is small so new entrants would need to develop and manufacture products with global appeal.

6.2. Research and Development and Innovation

Products in the industry can become redundant very quickly so players need to invest in research and development. Access to funding

is therefore vital. Covidien, for example, dedicated more than US$500m to R&D in 2013. As mentioned earlier, South African R&D has

suffered from under-funding the past two decades, but there have been a number of government initiatives to drive R&D in the medical

device industry.

♦ The Industrial Development Corporation (IDC) has a Healthcare Strategic Business Unit that offers loans assisting new and

BBEEE companies that want to enter the sector, including manufacturers of medical devices and technology, medical and

surgical equipment, orthopaedic appliances and dental supplies. The IDC has already invested in a number of local medical

device manufacturers. One such venture is CapeRay that received R15m from the IDC in 2010.

♦ The Medical Research Council has an eHealth Research and Innovation Platform (eHRIP) which includes a Strategic Health

Innovative Partnerships (SHIP) unit which aims to enhance the capacity of South African science in the research and

development of novel or improved drugs, vaccines and other biologicals, diagnostics and medical devices in the identified

priority diseases. SHIP also supports existing DST strategic health innovation initiatives such as the Medical Device

Innovation Platform (MDIP).

♦ The Cape Health Technology Park project is a joint project between the Western Cape Government and DST, academia and

business. It will be a facility where pharmaceutical companies, research institutes, clinical trial facilities and health-related

academic and government programmes are housed in one location. It will provide support to local manufacturers and will be

supported by Western Cape trade promotion agency, Wesgro.

The local medical device manufacturing industry, although small, has developed a number of products.

♦ CapeRay’s PantoScanner products are currently undergoing clinical trials and the company hopes to get FDA approval in the

first half of 2014. The Panida digital X-Ray camera contained in the scanner has already obtained CE compliance and ISO

certification. CapeRay won the National Science and Technology Forum award in June 2012 in the small-to-medium sized

category and plans to release its products in South Africa during 2014.

♦ Elite Surgical is in the process of developing new products in the orthopaedic and arthroplastic field. The company funds its

own product developments working in close collaboration with surgeons. Elite Surgical also exports products to the USA and

Europe.

♦ DISA Vascular received CE approval for its newly developed Cape Cross PTCA catheter in the field of cardiology. As part of

the product development process, the company concluded a licence agreement with American company SurModics for the use

of its surface modification technology, which DISA Vascular has trademarked as Contour® coating. The company's

ChromoFlex and SolarFlex coronary stents are sold in numerous countries in Europe, Africa, Asia, the Middle East and South

America.

♦ In 2013, locally designed and manufactured Lodox Xmplar-dr, which takes a high-speed, full-body x-ray image, was

revealed. Locally, the R4m scanner can be found at 15 institutions. Xmplar-dr was named the top innovation concept at

Accenture’s 2013 Innovation Index and was featured in an episode of Grey’s Anatomy. The IDC became a 95% majority

shareholder of Lodox in 2010 and funds the company’s on-going market research, developments and product improvements.

8. FUTURE OUTLOOK

According to SAMED, the medical device manufacturing industry is set to grow at a steady rate, although not at the rate experienced

five years ago, mainly because of the slow recovery of the European economy and the fluctuating Rand. Western Cape Minister of

Economic Development, Alan Winde, believes the South African medical device industry will be worth an estimated R16bn by 2016.

Tanya Vogt, SAMED Executive Officer, emphasised that the industry is “in a positive period” as “we are starting to see more South

Africa-based multinational and distributors looking to enter other markets in the rest of Africa.” Growth in the local industry is expected

to be driven by government support of technological innovation and the introduction of the NHI.
