e-Prescription – How does it work?
1 The doctor issues an e-prescription
2 You receive
SMSwith a 4-digit code
or an e-mail with PDF informationabout the e-prescription
The doctor can also give youa printable version
3 At the pharmacyYour pharmacist will give you medicine based on paper document
the pharmacist scans the code from the e-prescription
you enter the code and PESEL (Personal Identification Number)
e-referral – How does it work?
1 Doctor electronically issues and signs e-referral.
2 You get an e-referral electronically – via an e-mail or SMS with a 4-digit code. You can also request a printable version.
3 You can schedule a visit by phone, website or personally.
4 To register for a visit you should provide 4-digit code and your PESEL (Personal Identification Number).
5 You receive date of your appointment.
