What is The Community
Connection Program?
A Community Inclusive
“Transition Program”
The Community Connection Program
• A community based post high school program
• Located on the Howard Community College campus,
• Designed for students with mild to moderate disabilities
• Who are in their last years of public school education.
“Characteristics”of
potential students
Student Characteristics
• 19 years of age
• will receive a Maryland HS Certificate, not a Diploma
• minimum of four years in a High School setting demonstrating the ability to function independently with minimal supervision.
• IEP goals and Transition plans are appropriate
• demonstrates satisfactory attendance and behavior
Referral to The Community Connection Program is based on an
IEP Team recommendation
* All students will be reviewed on a case by case basis
