ICCE: intracapsular cataract extraction

• Indication• เหมาะในท่ีไม่มีกล้องขยาย

ส าหรับผ่าตัดตา• อาจท าในรายท่ี hypermature และ Subluxated cataract

• Absolute contraindication• ในเด็ก• ในรายท่ีมีอุบัติเหตุ capsule ของ

เลนส์ฉีกขาด

• Relative contraindication• สายตาสัน้มาก• Marfan’s syndrome• Morgagnian cataract• มีVitreous in Anterior cahmber

ข้อดี• ลดอุบัติการณ์ การเกิดผลแทรกซ้อนที่เน่ืองจากแผล (Wound related complication)

• แผลผ่าตัดหายเร็วกว่า• การมองเห็นหลังผ่าตัดชัดเร็วกว่า• การอักเสบน้อยกว่า• สายตาเอียงจากแผลน้อยกว่า

ข้อเสีย• เครื่องมือมีราคาแพง• เทคนิคการผ่าตัดยากกว่า ECCE ต้องการความช านาญในการผ่าตัด

Pars plana lensectomy

• Indication

• Congenital cataract

• Traumatic cataract ที่มี vitreous หลุดออกมาหน้าเลนส์• Complicated, subluxated,dislocate

hypermature cataract

• Malignant glaucoma

• Retinopathy of prematurity

• Persistent hyperplastic primary vitreous

Femtosecond Laser

• ท าหน้าที่แทนใบมีดในการเปิดแผลที่กระจกตาและเปิดถุงหุ้มเลนส์ตา

• แผลยังมีความเรียบและสมานตัวได้ดีกว่า• การผ่าตัดจะใช้เวลาประมาณ 20-30 นาที• ข้อจ ากัด

• ไม่เหมาะกับต้อกระจกที่สุกมากเกินไป• ผู้ที่มีความขุ่นของกระจกตาหรือการอักเสบ

ในตา• ค่าใช้จ่ายสูง
