The right care in the right place at the right time.RUNNYMEDE HEALTHCARE CENTRE is a state-of-the-art, 200-bed hospital located in Toronto’s west-end that provides safe, leading-edge care. In 2011, we responded to the changing needs of the community by introducing our Low Tolerance Long Duration Rehabilitation program (LTLD Rehab). The program is designed to provide care for adults who are in need of slower-paced, less intensive rehabilitation therapy, (e.g. after surgery or treatment in an acute care hospital).
LTLD Rehab helps patients regain the functional skills, mobility and independence they need for day-to-day living. Our top priority is to get patients home sooner so they can get back to their lives.
PARTNERS IN HEALTH
Runnymede collaborates with acute care hospitals to achieve a common goal: to help patients recovering from treatment or surgery reintegrate into the community. Patients transferred to Runnymede’s LTLD Rehab program can expect safe, high-quality care in a supportive environment.
While at Runnymede, patients receive around-the-clock clinical care. When this level of healthcare is no longer needed, we work closely with patients and relevant community partners, e.g. Community Care Access Centres (CCACs) to facilitate access to community supports that will meet each patient’s unique needs upon discharge.
A STEP CLOSER TO HOME
Since Runnymede’s LTLD Rehab program launched, we have seen tremendous success:
• Over 500 patients have been successfully treated and transitioned back into the community.*
• The average length of stay of a rehab patient at Runnymede has been reduced to just 55 days.*
ADMISSION AND ACCOMMODATION
We want our patients to be comfortable during their stay at Runnymede. We make this possible by providing warm and welcoming rooms that include a bed, bedside table, chair and a closet for clothes and belongings.
Patients are contacted by a member of our patient flow team to advise when a bed at Runnymede is available for their admission. At this time, a patient will be informed of his/her physician, patient care unit, room number and bed number.

CUSTOMIZED CARE FOR EVERY PATIENT
Upon admission, new patients are assessed individually by our interprofessional team so that a customized plan can be developed for their recovery. We ensure each patient benefits from our wide range of clinical programs and services. Our patients receive highly personalized treatment based on their unique needs to enhance health, mobility and quality of life.

Our interprofessional team takes a collaborative approach to delivering safe, high-quality care, promoting patient-centredness and encouraging wellness to improve quality of life and enhance the patient experience at Runnymede.
• Activation therapy: provide creative, therapeutic programs to foster social well-being.
• Clinical nutrition: assess nutritional status to develop personalized nutrition plans.
• Medicine: assess and treat medical conditions.• Nursing: deliver high-quality, around-the-clock
patient care to meet daily needs.• Occupational therapy: restore independence and
ability to perform daily activities, like feeding and dressing.
• Pharmacy: supply medication and act as a valuable drug information resource to ensure patient needs are safely met.
• Physiotherapy: restore independence, mobility, stability, comfort and physical function.
• Social work: support mental health and emotional well-being through individual and group therapy sessions.
