Finally, may we draw your attention to some upcoming training events, which we would like full attendance! There will be some lovely hot food and excellent specialist speakers. This includes creating a public facing website to promote all that community pharmacy has to offer. We want to create an up to date picture of the services available in your pharmacy. Staffordshire and Stoke Medicines Management have noticed that there are significant differences in prices of specials between different pharmacies. For example for the same item one pharmacy has claimed £40 where as another has claimed £150. We understand that you may be only authorised to use one supplier. Can we ask you to please inform your prescriber if an item is a special item over £100. Please advise the prescriber of a cheaper drug tariff alternative. Do NOT Prescribe Ibuprofen in Chickenpox We have come across this story from the Manchester Evening News which highlights that ibuprofen should NOT be used to treat fever in children with chickenpox (varicella): http://www.manchestereveningnews.co.uk/whats-on/family-kids-news/chickenpox-ibuprofen-picture-facebook-septicaemia-11158669 As a reminder, NICE Clinical Knowledge Summary for treating chickenpox states:

There are concerns that use of NSAIDs in children with varicella is associated with an increased risk of necrotizing soft-tissue infections and infections with invasive group A beta-haemolytic streptococci [Heininger and Seward, 2006].

Evidence suggests that there are elevated risks of skin complications in people with varicella when exposed to NSAIDs. These findings have been replicated in several studies [Bijl, 2010; Mikaeloff et al, 2008]. For this reason it is recommended that NSAIDs are avoided in children with varicella.

Paracetamol has no notable adverse effects when used at the correct dosage [BNF 64, 2012].

NHS England to recommission flu vaccination service May 9, 2016 NHS England has announced it will recommission the Community Pharmacy Seasonal Influenza Vaccination programme in 2016/17. The terms and fees for the recommissioned service will remain the same as in 2015/16. Further guidance will be issued to contractors once legislation and associated documents have been updated to take into account the recommissioning of this service. Alastair Buxton, Director of NHS Services, PSNC, commented: “This early announcement of recommissioning will help pharmacy contractors prepare for provision of this important service and is to be welcomed.” PSNC and Pharmacy Voice publish briefing on Accessible Information Standard PSNC and Pharmacy Voice have worked together to produce a briefing updating those working in community pharmacy on the Accessible Information Standard. NHS England are introducing a new Accessible Information Standard, which will become a legal requirement for all providers of NHS and adult social care services in England, including community pharmacies, on 31st July 2016. The purpose of the Standard is to ensure that people who have accessible information and/or communication support needs because of a disability, impairment or sensory loss receive information in a suitable format, ideally of their choice. The purpose of the joint briefing is to update community pharmacy contractors on the requirements of this Standard, as well as explaining what steps they need to take next and outlining how the pharmacy organisations will be supporting them. REMINDER Re: Supply issues with Diamorphine Injection End of Life Network advice on alternative drug treatment in this patient group: Where diamorphine is unavailable, we would recommend using morphine sulphate for injection first line as the alternative (Palliative Care Service Pharmacies have been asked to stock 10 ampoules of the 10mg injection). Morphine is available in the following strengths: 10mg/ml, 15mg/ml, 20mg/ml and 30mg/ml. It can be diluted in water for injection. The oral to subcutaneous potency ratio of morphine is 1:2 (i.e. the subcutaneous dose is approximately twice as potent as an oral dose) If you are prescribing anticipatory/end of life medicines in a patient who isn’t currently prescribed any strong opioids, we would recommend: Morphine 2.5-5mg subcutaneous, as required, maximum hourly. For patients who have previously been established on regular modified release morphine who need a continuous subcutaneous infusion of morphine by syringe driver, divide the total 24 hour oral dose by 2. For example - patient taking Zomorph 30mg twice a day orally = 60mg/24hrs oral Morphine 60mg ÷ 2 = 30mg/24hrs by continuous subcutaneous infusion. The usual as required “breakthrough” dose remains 1/6th of the 24 hr dose, so in this case 30mg÷6 = 5mg as required subcutaneous, maximum hourly. If you have a patient who requires large doses of subcutaneous morphine (more than 100mg subcutaneous morphine in 24 hours), or patients who cannot tolerate morphine, or if you are uncertain about the dose of opioid to prescribe, please seek advice from the Douglas Macmillan Hospice 24/7 advice line (01782 344300). Please note this information is also being shared with relevant community teams. Actions:

Please consult the attached list for participating Palliative Care Pharmacies and signpost patients/carer accordingly. Ideally provide a copy of the list of pharmacies. It may be advisable to telephone the pharmacy first for stock availability/ordering.

When prescribing End of Life palliative medications please prescribe on a separate prescription so that patients/carers have the option of trying an alternative pharmacy. (We understand the template does separate anyway for the 4 main EoL drugs).

If prescribing for syringe driver use, seek advice for individual patients to ensure compatibility with other drugs is checked.

The PSNC’s LPC Leadership Academy programme has begun. Twelve future leaders from various LPCs across the country were selected from a huge number of applicants. Elissa Pateman from North Staffs and Stoke LPC was one of the candidates that was successful in her application to join the academy. “I was thrilled when North Staffs and Stoke LPC supported my application to join the PSNC Leadership Academy and was even more excited when I was successful. I feel that the leadership programme will give me the confidence and skills to help in my role as HLP Facilitator and to make a positive impact on pharmacy teams.” Elissa Pateman.

PricewaterhouseCoopers (PwC) is conducting an independent assessment of the social value of community pharmacy for PSNC and we are calling on community pharmacy teams to get involved to make sure this work provides an accurate picture of the services that pharmacy teams provide up and down the country.

The work being undertaken by PwC will help the ongoing community pharmacy campaign by providing a robust evidence base to use in discussions with stakeholders and in future negotiations. We therefore strongly encourage all pharmacy teams to get involved in this work to help highlight the social value that your pharmacy brings to your community.

Pharmacy teams in England can get involved by assisting with two easy to complete activities:

Activity 1: Survey on additional activities relating to dispensing prescriptions

Participation in a short survey on the number of additional activities that your pharmacy team has provided in relation to dispensing prescriptions, for example, delivery of medicines and provision of monitored dosage systems. A survey form is available to collate this information, and your answers can then be submitted to PSNC and PwC via an online form on the PSNC website. Please submit your response to the survey by 5th June 2016.

Activity 2: Data collection on ‘advice on self-care or OTC medicines’

Completion of a simple data collection form over a seven-day period (from Wednesday 1st June to Tuesday 7th June) to record how many times advice on self-care or on the use of over-the-counter medicines has been provided. Once this data has been collected, we are asking that it is submitted to PSNC and PwC via an online form on the PSNC website.

CPPE Upcoming Events

Polypharmacy Focal Point.

Acute Kidney Injury (AKI) Event. Keele Hall, Keele University. September 29th

CPPE Leadership School 4 day course, comprised Day 1 & 2 with overnight stay, then two separate days spaced approx 6 weeks apart. The course is heavily subsidised by CPPE but there is a £300 attendance fee which goes towards the MBTI feedback and overnight stay between days 1 & 2. This still represents excellent value for money compared to other leadership courses. Open to all pharmacists and pharmacy technicians, we have attendees from all branches of the profession - it is a great course and a great way to network. Venues available: April 20th and June 9th 2016 Manchester: September 16th and 17th, October 20th and November 24th 2016. Bristol: October 14th and 15th, November 24th 2016 and January 10th 2017.
