A Sworn Affidavit of the Secretary of the NGO/PO that none of its incorporators, organizers, directors or officials is an agent of or related by consanguinity or affinity up to the fourth civil degree to the officials of the DOLE authorized to process and/or approve the proposal, the MOA and the release of funds.

the fund utilization report/s (liquidation report/s) on the previous fund release/s.

For ACPs who have not submitted the fund utilization report/s (liquidation report/s) despite follow-ups made by the DOLE:

Include in the blacklist/s thereby prohibiting or disqualifying them to apply for any DOLE grants;

Enforce refund of grants made by the DOLE; and File the necessary lawsuit/s against the ACP.
