OCCMHA: Funding

OCCMHA receives Federal funding from the Centers for Medicare and Medicaid Services (CMS)

OCCMHA also receives State funding from the Michigan Department of Community Health (DCH)

OCCMHA receives local funding from Oakland County

What does OCCMHA do with their funding?OCCMHA uses the funds to contract with different

Core Provider Agencies to provide services and supports.

These agencies form a large Provider Network. This network is composed of the facilities that actually provide the services to persons with developmental disabilities or persons with mental illness.

The Core Provider Agencies also contract with different organizations to provide various services.

Common Ground AccessCommon Ground Access is the central screening

center for eligibility for person’s living in Oakland County.

Ways to be screened at Common Ground Access:

Criteria for Eligibility for Developmental Disability

The disability must be manifested before the individual reaches 22 years of age.

The disability is most likely to continue indefinitely.

The disability results in substantial functional limitations in 3 or more of the following areas of major life activity:

Self-care Receptive and expressive language Learning Mobility Self-direction Capacity for independent living Economic self-sufficiency

Eligibility Determination Screening ProcessCall Common

Ground at

(800) 231-1127.

If a screener is available, they will

take your call.

If a screener is unavailable, they will return your call within 24 hours.

General information (name and phone number) will be taken.

Screener will ask a variety of questions pertaining to the caller’s developmental disability and/or related mental health.

If found eligible, Common Ground will link the person who will receive services to a Core Provider Agency of their choice.

If not found eligible, that person can appeal the decision by asking for a Second Opinion through OCCMHA Customer Services at (800) 341-2003.

Intake Appointments1. Person’s who have been found eligible will

have an intake appointment at their Core Provider Agency of choice. Your son/daughter will be attending this appointment with you. They will have a specific date and time arranged by Common Ground.

2. There will be paperwork that needs to be filled out at the initial intake appointment.

3. More detailed information will be gathered by the Intake Coordinator in regards to their developmental disability and/or mental health matter(s).

4. A preliminary Person Centered Plan is completed.
