Special enrollment opportunity — October 23 - November 1, 2019You have a special one-time enrollment opportunity to elect the following coverage options without answering health questions, also known as evidence of insurability (EOI):
• Employee optional term life and accidental death and dismemberment (AD&D): Elect or increase existing coverage to a new total of $150,000
• Voluntary employee AD&D: All guaranteed at each annual enrollment
• Voluntary spouse AD&D: All guaranteed at each annual enrollment

exceed 50% of employee’s voluntary AD&D coverage amount)
If your spouse or child is eligible for coverage as an employee, they cannot be covered as a dependent. A child may only be covered by one parent.
