1. The undersigned midshipman would like to ask from your office an approval to have an official business leave for the purpose of joining the Regional Higher Education Press Conference to be held at at Villa Alfredo’s Resort, Barangay Baliti, San Fernando City, Pampanga on December 3-5, 2014.

2. If approved, the said leave will commence at 0730H of 03 December 2015 and will terminate until 1900H of 05 December 2015.
