1.  I understand that I am eligible to receive the Co-op services as set out in “Services Provided” (attached)

2.  I must meet the Co-op eligibility requirements for my program as set out in Co-op Eligibility (attached)

3.  I understand that the Co-operative Education Consultant conducts mandatory classes to prepare me for

resume writing, job searching, interview skills, etc. and these topics are necessary for success as a Co-op

student

4.  I will prepare a personal resume to be submitted and reviewed by the Co-operative Education Consultant

and utilized by myself for making applications to potential employers. I will update my resume at

appropriate intervals

5.  I will conduct an independent Co-op job search with guidance and advice from my Co-operative Education

Consultant. I specifically understand that it is my responsibility to obtain a Co-operative Education work

term

6.  I understand that once I have accepted a Co-op position with an employer that I am committed to fulfillthat Co-op work term. I will then adhere to the appropriate co-op time line as available on the Co-op

website

7.  I have read and fully understand the Co-operative Education Policy and agree to comply with those terms

of the policy. (attached) I understand that non-compliance with these terms will cause me to be ineligible

for future participation in the Co-operative Education program. The consequences of this ineligibility are:

a.  My transcript will indicate an “F” grade for Co-op

b.  I will NOT be eligible to receive a Co-op endorsed diploma

8.  I understand that I must complete an online Health and Safety Workshop (currently WorkSmart) and

achieve a minimum grade of 80%. I must provide a copy of the certificate upon completion to my Co-op

consultant (in the Co-op FOL Dropbox “WorkSmart”). This is a mandatory requirement prior to

acceptance of a Co-op position

9.  I understand that I can decide NOT to participate in Co-op (with the exception of #6) and I will inform my

Co-op consultant immediately of my decision. I understand that if I do not participate in Co-op I will NOT

receive a Co-op endorsed diploma.
