Assessment Tools
Assessment tools are resources that are used in
the support planning process to help build up a
holistic picture of an individual’s needs and
relatedcircumstances. Once all the details are recorded,
an assessment can be made, and suitable care
andsupport can be identified. (Stretch and Whitehouse,
2010)

Behavioural Task
Approachidentifying behaviour thatneeds to be changed.

Task CentredThis approach involves identifying
particular tasks orskills that the individual needs to
achieve. The taskcentred approach centres on supporting the individual
to achieve the task for themselves and therefore
develop independence in that particular area.
Therapeutic Groups These are often used when a number of individuals
require similar support in a particular area.
Groupscan be for general support (e.g. to
improve selfconfi dence) or for more specifi c areas such as
overcoming alcohol addiction or learning anger
management skills.
