Scott Peck April 2014 On July 1st, 2014 Kansas will begin the process of implementing Health Homes to KanCare members who have certain chronic conditions such as diabetes, asthma or mental illness. Individuals who will receive these serves are those who are currently KanCare consumers. Labette Center for Mental Health Ser- vices is working towards a Health Home provider designation for individuals residing in Labette County who suffer from severe mental illness. According to the KanCare website (www.kancareks.gov), “. . . ‘Health home’ refers to a new Medicaid option to provide coordination of physical and behavioral health care with long term services and supports. . .” This is not a building of any kind and not a place where individuals live. It is the coordi- nation of those services to find ways to best serve individuals who fit the criteria and help them achieve better outcomes for their health. Health homes focus on a person as a whole and help them to man- age all their health care needs.     Mental Health First Aid Course April 29th, 2014

    Labette Center for Mental Health Services will be holding an Adult Mental Health First Aid course on Tuesday April 29th, 2014 from 8:00am to 5:30 pm. The course will be held at Labette Cen-ters main facility located at 1730 Belmont Par-sons KS in the multi-purpose room. To register for the April Mental Health First Aid Training, individuals can visit our website at http://www.lcmhs.com/Mental-Health-First-Aid-65456.asp or call 620-421-3770 800-303-3770.

    According to the Mental Health First Aid webi-ste, this program is intended for a variety of audiences: friends and family of individuals with mental illness or addiction, key profes-sionals (such as police officers, human resource directors and pri-mary care workers), school and college leadership, faith communi-ties, or anyone interested in learning more about mental illness and addiction. The training venues will also vary as Mental Health First Aid program sites reach out to Chambers of Commerce, professional associations, hospitals, nursing homes, Rotary Clubs, parent organi-zations, social clubs and other groups who make up the fabric of a community. The core program is tailored for response to the general adult population.

    able to provide to this community. Along with Wil-liams, the Center will employ two care coordinators and peer support specialists that will be focused on the Health Homes model. The Center will also be looking to expand staff to include a registered nurse who will assist in providing these services. This person will have the primary function of managing the full array of behavior-al health needs, physical health needs and support social services and family services.

    In the Health Home model there are six key services that the Center will provide. These include care coor-dination, comprehensive care management, health pro-motion, comprehensive transitional care, member and family services and referral to community services and supports. The Health Home will be for any member of the Labette county community who suffers from a severe mental illness, whether they are currently a client of La-bette Center or not.

    Labette Center will work with physicians, hospitals and public health agencies to best serve the needs of those in the health homes.   • The Labette Center for Mental Health Service, Inc. Foundation for Help and Hope is an entity governed by a Board of Trustees, which is jointly shared with Labette Center for Mental Health Services. The foundation creates opportunity for individuals in the community and surrounding areas to be able to give to a local non-profit organization whose mission is to create hope and opportunity for life change by providing education, support, consultation, and therapeutic behav-ioral health services to the people and communities of Labette County.

    The foundation enables us to grow and expand the hope and help we are able to provide. The Foundation for Help and Hope provides a tax deductible entity for

    individuals or organizations to be able to donate to.

    Labette Center for Mental Health Services offers Community Support Services for adults in Labette County. Community Support Services are designed to help adults with psychiatric issues to have mean-ingful and productive lives.Community Support Services help consumers move towards a sense of recovery and rehabilitation. To that end, case managers explore strengths and re-sources that assist consumers in identifying and

    meeting goals in regards to hous-ing, employment, education, suc-cessful relationships and inde-pendence.

    Community Support Services build meaningful and productive lives by using a multitude of resources. Included in the resources available are strength based case management ser-vices for adults, case planning and resource referral, peer support services, psychosocial groups, atten-dant care. Individuals receiving community support services also receive coordination with therapist, psychiatrist and other community agencies as well as Integrated Dual Diagnosis Treatment for those individuals who have co-occurring substance use disorders.

    Referrals for Community Support Services can be made by calling Labette Center for Mental Health Services and requesting an intake appointment. If an individual already receives therapeutic services through Labette Center, they can discuss Commu-nity Support Services with the Qualified Mental Health Professional that is assigned to them. If the criteria are met a case manager can be assigned and a treatment plan that is specific to that persons needs can be formulated.
