GUIDELINE TO APPLICANT

1. SCOPE OF WORK:-

The empanelled chemist will supply the medicines/drugs at the “Govt.

Hospital Rate” in the stipulated time on the basis of „Limited Quotation‟

Issued time to time from central Hospital Jaipur.

The nominated chemist will respond to each & every call for quotation called by

Central Hospital, NWR, Jaipur.

2. ELIGIBLITY:-

2.1 The chemist must hold valid license as on date on specific form by local statutory body i.e. the drug control authority of state under provision of drugs & cosmetic act 1940.

2.2 The empanelled chemist must have not convicted by the state drugs authorities and no case should be pending under the drug & cosmetic act & Laws.

2.3 The annual turnover of the bidder must be 20 laces per annum during the last year. 2.4 The chemist shop/establishment should preferably be situated within the

municipal limitation of city of Jaipur or within yhe the state of Rajasthan. 2.5 VAT clearance certificate should be available with the applicant, attached copy with

application along with TIN certificate 3. Cost of application:-

The prospective applicant shall bear all the cost associated with the

preparation and submission on at their application. Railway in no case shall be responsible /

liable for their cost regard less at conduct on out come of the application process.

4. Pre Application Meeting:-

4.1 A pre application meeting open to all prospective applicants will be held as

per applicant schedule on 14/08/2012 in the office of the MD/CH/NWRJP. Where in the

prospective applicant will have an opportunity to obtain clarification regarding scope of work

and empanel conditions.

4.2 The prospective applicants are free to ask any additional information and

clarification concerning the works either written or orally. The reply shall be common to all

applicants.

6. Deposition of application:-

6.1 The applicant can submit the application/documents at the office of the

Medical Director, Central Hospital NWR, Jaipur on or before date…28/08/2012..at 14.00 hrs.

Application received after this date and time will not be entertained. Railway administration

will not be responsible for any postal delay.

6.2 Any conditional application will not be accepted.

The applicant should under take to quote “Minimum Hospital Supply Rate “,

as fixed by principle manufacturer.

8. Inspection of applicants premises If the technical application on the basis of the document mentioned in

this section appear to be fulfilling the eligibility conditions the applicants premises will be inspected by a team of officers for:-

A. Physical verification of site, B. Certification of original documents, C. Availability of proper cold chain which would be a refrigeration or proper back

up system, and D. Presence of outlet, E. Assess availability of stock of medicines both physically and financially etc. 9 Notification of acceptance of applications.-

The medical director CH/NWR will communicate acceptance to the application by letter.

10. Miscellaneous:- 11.1 The medical director CH/NWR reserves the right to enter into parallel

contracts simultaneously or at any time during the period of this contract, with one man chemist/stockiest.

11.2 Appointed chemist stockiest will as and when required to attend the meeting fixed by the medical director & Sr, DMO (Store0/CH/NWR/JP

11.3 The stockiest / chemist in whose case the NWR has terminated their contract on the basis of substitution of medicines overcharging in the bills, misbehavior with railway employees / official etc. will not be eligible for participation in the tender. (Affidavit to be submitted)

Minimum requirement for eligibility:

1. The firm should hold valid licenses on the date of application in specified form for various categories of allopathic drugs issued by the state under the provisions of The Drugs & cosmetics Act 1940. The successful empanelled applicants further ensure validity till the end of the contract period.

2. The firm must not have been convicted by the state drug authorities and no case should be pending under The Drugs & cosmetics Act 1940 and rules. Non-conviction certificate issued by state authorities must be enclosed.

3. The annual turn over of the firm must be 20 lacs per annum for last 03 years.

4. The firm establishment should be situated within municipal limitation of Jaipur/state of Rajasthan

5. Photocopy of latest VAT/CST clearance certificate (Mar-2012) should be available with the applicant a copy of PAN & TIN allotted to the firm.
