Information for 4th grade students and parents: As we begin to prepare for the end of the 2011-12 school year, we also begin preparing for
the beginning of the 2012-13 school year! As our thoughts turn to next year, we would like
to remind you of our policy and procedure for requesting teachers. We utilize a Parent In-
put Form to gather information pertaining to your child. This form is attached to this news-
letter (for our current fourth graders only) and can also be found on our school website and
in our school office. The form is the only communication channel that will be accepted.
This form focuses on the strengths and needs of your child. You may also describe the char-
acteristics you desire in a teacher as well. If you choose to write the name of a specific
teacher on the form, please list the reasons for your request. There are never any guarantees
for a teacher request, however, because of the many variables we take into account when
placing students in classrooms for the following year. We ask that you trust our profession-
al judgment on what is best for ALL children and want you to know that we take this pro-
cess very seriously. Class lists will be created during the summer by Mrs. Dyer, Mrs.
Raycroft, and Mrs. Cripe. When creating class lists for our incoming fourth graders, we
also include the elementary principals and guidance counselors in this process. We do en-
courage you to fill out the Parent Input Form to provide us with information about your
child. If you choose to fill out this form, please return it to school NO LATER than Fri-
day, May 25, 2012. If you have specific questions, please call our school office to speak
with Mrs. Dyer or Mrs. Raycroft. Questions related to sixth grade placements should be
directed to Northridge Middle School at 825-9531.
ISTEP+ (Round 2)
During the time period of April 30 – May 11, 2012 all of our 4th and 5th graders will be tak-
ing the multiple choice section of the ISTEP+ test. As you may recall, we took the Applied
Skills section in early March. Students will be taking ISTEP+ on-line as a new approach
which is mandated by the state of Indiana. Again, we would encourage your child to eat a
healthy breakfast, get plenty of rest, and to be present at school each of these days. Try to
avoid scheduling doctor’s appointments during this time, if at all possible. Thank you for
providing the support necessary to insure your child feels the most success on these tests.

The mission of Heritage Intermediate School is to equip students to achieve
their potential and positively impact their community.
Heritage Helps Out Although it seems Spring has come early, Heritage will be having a spring clean-up
on Thursday, April 26th. 4th and 5th grade students and Staff will be cleaning up
trash from around the campus from 2:30 to 3:30 after school. Following the clean-
up, the volunteers will be treated with a snack for their good deeds. Keep an eye
out for more information and permission slips following Spring Break.
Notes from our School Nurse…. Just a few reminders for everyone about the Middlebury Community Schools medication policy: 1. Medication must come to school in the original container with a note from the parent that specifies the time the medica-tion is to be given and the dose of the medication. If your child is to receive prescription medication for more than 5 days, a note to the school from the doctor is required. 2. All prescription and non-prescription medications must be kept in the nurses' office (includes tylenol, tums, throat sprays). The student must bring the medicine to the clinic right away in the morning. 3. In some cases, emergency medications (i.e. inhalers) may be carried on the student's person IF the parent provides a note from the doctor stating that the student may carry and self-administer the inhaler. It is recommended, however, that all inhalers are kept in the nurse's office. 4. Any medication that is sent to school may be released only to the parent or a designated adult over the age of 18 with the parent's written permission to pick up the medicine. Please do not send medicine to school with your child and ex-pect the school to send it back home with your child. Sixth Grade Immunizations
The state of Indiana (IC 4-22-2) REQUIRES the following immunizations for students in grades 6-12 for the 2012-2013 school year. Students not in compliance will be subject to exclusion from school.
Two doses of varicella vaccine, or a history of chicken pox disease. One dose of tetanus-diptheria-acellular pertussis vaccine (Tdap) given on or after the 10
th birthday.
One dose of meningococcal conjugate vaccine (MCV4).
Fifth Grade Parents: Please remember to make your child’s appointment for his or her sixth grade immunizations ASAP, if you haven’t done so already! If you’d like to schedule them with the Elkhart County Health Department, the phone num-ber is (574)523-2127.

Hoops For Heart Update Recently at HIS, all students took part in Hoops for
Heart during PE. It was a fundraiser for the Amer-
ican Heart Association. The students collected
donations from the community which were sent to the Ameri-
can Heart Association. In all, the students raised $3660.18!!
The students also learned about the importance of
taking care of their bodies, especially the heart, and learned
some ways to help themselves and others stay healthy. The
students also participated in healthy fitness activities during
the Hoop for Heart event. Some of the events they took part in
were basketball shooting contests and spinning the hula hoop.

Mobile Dentist The Mobile Dentist will once again be coming to our school on April 27, 2012. The team from Mobile
Dentist will set up a mini-dental office in our school and provide dental examinations, cleanings, fluoride
treatments, x-rays and sealants (if necessary) for children who return completed forms. Mobile Dentist
accepts many types of insurance and provide reduced fees and full financial assistance when need-
ed. They have been coming to Middlebury Schools for a number of years now and provide excellent care to our stu-
dents. Forms for Mobile Dentist are available in the school office.
Boys & Girls Club Spring Break Hours
The Middlebury Boys and Girls Club is ready for Spring Break! The Regular Club hours for Spring Break week (April 2-6) will be from 10am – 6pm. They do offer an Optional EARLY BIRD special (fee based) program for those that might be interested as well. The EARLY BIRD hours for that week will be 6:30am – 10am. during this session. The program will last
approximately 45 minutes and there will be
information handed out to assist in the transi-
tion for your child to the Intermediate school.
There will also be a 3rd grade transition trip to
Heritage later in May for your student. This
will include an orientation trip to Heritage and
activities that will blend students together from
all four elementary schools. More information
will be coming home with your child regarding
this trip.
One Thing Constant in the World...is Change
As the end of the school year approaches, we begin to mentally prepare children for the changes coming up in their lives. In 5th grade, we start introducing the students to what life will be like in the middle school. More independence, more responsibility, and of course, more opportunities. To help children with change, be sure to keep some schedules con-stant. Maybe the same times of getting up and going to bed, maybe eating together as a family, are ideas for keeping consistency for children. Children thrive on knowing what to expect from adults. You are one thing that they can depend upon!

lon will be held at the Northridge High School and will provide both youth and adult
courses.
