• Provides a lightweight, modular option for covering openings
• Allows staff to see through the “floor” to the process below

• Walking-Working Surfaces shall ….

o support the maximum intended load
o be inspected, regularly and maintained in a safe condition
• Correction or repair requires a qualified person performs or supervises the work.

• Foam was overflowing from the RAS well.
• Overflow lifted the grating out of its place.
• Employee fell through the grating into the RAS well.
• Employee asphyxiated and was pronounced dead on the scene.

• Employee partially removed grating over a scum pit to install a water hose for defoaming.
• Employee fell into the opening by stepping onto an improperly seated grating.
• Employee asphyxiated and was pronounced dead on the scene.

• Employee fell through the grating and into the waste water.
• Employee was pronounced dead at the scene.

WWTP and Grating Related Incidents
• Employee was collecting a routine sample grab at a distribution box
• Grating shifted off its support and employee fell into the basin
• Employee held onto a cable in the basin for approximately 2 hours
• Threw his hardhat onto the ground above the box, where it was discovered by rescuers.
• Employee was hospitalized with strains, abrasions, and an infected lung –But recovered and lived to tell his story.
