1. Please return this request in a sealed envelope to Jennifer Salwei theParish Accountant.

2. Using the confidential TADs website at www.tuitionaid.com, submityour financial information by the deadline of March 18, 2011.

3. Once you have submitted your information, TADs will process it and

send a report to GA with the amount of assistance they recommend.

4. The Scholarship Committee will meet on April 15, 2011, and willdetermine scholarship amounts at that time.

5. All applying families will be notified of scholarship amounts byApril 22, 2011.
