Do you have diabetes? Diabetes Self-management, a six-week self-management education workshop, can teach you how to take charge of your ongoing health condition.

You will learn skills like goal-setting, problem-solving and overcoming barriers to help you manage and cope with symptoms from diabetes.

Active involvement in self-manage-ment can help you improve your overall quality of life.

The Diabetes Self-Management Program was created by the Stanford Patient Education Research Center at the Stanford University School of Medicine. It is designed to help people gain self- confidence in their ability to control their symptoms, better manage their diabetes and lead fuller lives.

At the workshops you will learn about:• Healthy eating• Techniques to deal with the symptoms

of diabetes• Appropriate exercise for maintaining

and improving strength and endurance• Appropriate use

of medication• Working more

effectively with health care providers

TAKE CONTROL OF YOUR HEALTH

Participants who completed the workshop have demonstrated significant improvements in:• Dealing with the symptoms of diabetes,

fatigue, pain, hyper/hypoglycemia, stress, and emotional problems such as depression, anger, fear and frustration

• Patient activation and communication with health care providers

• Confidence in their ability to manage their health

• Well-being
