This change will be caused by the enactment of the Medicare Access and CHIP Reauthorization Act of 2015 (MACRA). MACRA will link physician payments to quality, value measurements, and participation through Merit-Based Incentive Payment Systems (MIPS) and Alternative Payment Models (APMs). Work in this project will enable you to be successful in these new payment models. Why is the Transforming Clinical Practice Initiative (TCPI) Important? This PTN is being led by HQI, one of 29 healthcare collaborative networks recently selected by the Centers for Medicare & Medicaid Services (CMS) to be part of the Transforming Clinical Practice Initiative (TCPI). TCPI is aiming to help clinicians achieve large-scale healthcare transformation. The goals of this four-year engagement with clinicians include: 1. Support for over 140,000 clinicians in their practice transformation work. 2. Improve health outcomes for millions of Medicare, Medicaid, and CHIP beneficiaries and other patients. 3. HQI's role in the PTN will include providing free technical assistance support to help equip clinicians with tools, information, and network support needed to improve quality of care, increase patients’ access to information, help people live healthier lives, and spend healthcare dollars more wisely. Clinicians will benefit from this initiative by being able to:

What is Practice Transformation?

There are five phases of practice transformation and each phase is more advanced. The resources you find in HQI's PTN will help your practice work through each one. To get your practice started, HQI will do an assessment to determine which phase you are in, then we will tailor our support to assist you in moving through the phases.
