Sheri Sangji

• On the 16th of January 2009, a 23 year old research assistant, Sheri Sangji, lost her life to severe burns suffered in a UCLA chemistry laboratory fire. On December 29, 2008, she suffered second and third degree burns over 40% of her body from exposure to tert-Butyl lithium, a chemical that combusts on contact with air.

The Laboratory Safety Manual (LSM) has been reviewed with all laboratory personnel. This is documented annually in the lab safety binder. A link to the LSM is located on the lab computer. The LSM can be viewed at (http://www.ehs.iastate.edu/publications/manuals/labsm.pdf )

Every person working in the laboratory has completed a safety check-in form and a copy has been placed in the lab safety binder. Safety check-out forms are completed for appropriate laboratory personnel & copies are located in the lab safety binder. Templates of the lab check-in and check-out forms to be used for this laboratory are on file in the FSHN office, 2312 FSB. Only new or updated templates must be sent to Joe Przybyszewski, 2312 FSB. Example check-in and check-out forms are located on the FSHN Safety Forms & Policies website at the top of this page.

Lab-Specific Safety Training is performed every year with all lab personnel and has been documented in the lab safety binder. The template only for the Lab-Specific Safety Training Form is on file in the FSHN office, 2312 FSB. Only new or updated templates must be sent to Joe Przybyszewski, 2312 FSB. An example form is located at (http://www.fshn.hs.iastate.edu/faculty/safetyforms/SiteSpecifTraining-2008.pdf).

Hazard Inventory Forms (http://www.ehs.iastate.edu/forms/hazardinventory.pdf) have been filled out for each new lab member (or updated for new methods that each lab member is using) and have been sent to EH&S.

Each lab member has performed a “Needs Assessment” of safety training courses as recommended by EH&S. [Login to the EH&S Learning Center (https://www.ehs.iastate.edu/cms/default.asp?action=article&ID=244&page=LC/login.asp) and go to “My Training Needs” under Topics; then answer the questions and complete the required courses.] The required safety training has been completed for all laboratory personnel, reviewed with the PI and documented in the Lab Safety Training History form (http://www.ehs.iastate.edu/forms/lstraining.pdf) and individual training course certificates are placed in the lab safety binder.

The Standard Operating Procedures (SOP) forms (for template from EH&S go to: http://www.ehs.iastate.edu/forms/soptemplate.pdf) for each similar group of methods performed in the lab have been completed and are placed in the lab safety binder. Reminder: laboratories using gas cylinders need to have SOP for handling gas cylinders (available at FSHN Safety Forms & Policies website).

The chemical inventory has been updated this calendar year and one electronic copy has been e-mailed to the FSHN safety officer, Joe Przybyszewski ([email protected]) and to Jim Gunning at EH&S ([email protected]) and a printed copy has been filed in the lab safety binder. Reminder: laboratories that use gas cylinders must include the gas cylinders on the laboratory chemical inventory. A chemical inventory template in Excel is located on the FSHN Safety Forms & Policies website (above).

Material Safety Data Sheets (MSDS) are available and up-to-date for all chemicals in individual lab groups.

Quarterly and Yearly lab safety survey inspections and Radiation Safety Lab Audit have been performed and completed forms are placed in the LSM or lab safety binder. The FSHN Quarterly Laboratory Inspection Sheet is located on the FSHN Safety Forms & Policies website (above). Safety checklist for a new GRADUATE STUDENT to begin work in a lab:

Obtain ISU identification card (ISUCard) from 0530 Beardshear Hall.

Safety checklist for new FACULTY to begin work in a lab:

Obtain ISU identification card (ISUCard) from 0530 Beardshear Hall.

Read then place a Laboratory Safety Manual (LSM) in the lab preferably on a computer or else it must be printed out. Keep a lab safety binder for safety records in the lab or in a central, accessible location with proper signs.

Write Standard Operating Procedures (SOP) for all lab methods using EH&S template and place into lab safety binder (http://www.ehs.iastate.edu/forms/soptemplate.pdf).

Obtain safety equipment as required for SOPs (at least nitrile gloves, labcoats, safety goggles/glasses, First Aid kit, spill kit and secondary containment for unwanted materials)

As you acquire chemicals, compile a chemical inventory using FSHN template (EXCEL) and obtain Material Safety Data Sheets (MSDSs) for each chemical (ie from company website).

Write a Lab Specific Training form using the FSHN template (Word).

Write and complete Lab Check-in (new link) and Lab Check-out (Word) forms using the FSHN templates.

Perform lab safety survey using EH&S form (http://www.ehs.iastate.edu/forms/LabSafetySurvey.pdf).

Develop an emergency action plan using the template from EH&S (http://www.ehs.iastate.edu/forms/LEPWTemplateFI.pdf).

Fill out a Hazard Inventory form (http://www.ehs.iastate.edu/forms/hazardinventory.pdf) and send to EH&S.

Establish notebook/record keeping practices for your students.

Perform a safety training Needs Assessment through EH&S (http://www.ehs.iastate.edu/cms/default.asp?action=article&ID=424).

Alternate Principal-Investigator Policy

updated 8/11/09 The Food Science and Human Nutrition Safety Committee recommends that each principal investigator (PI) in the FSHN department designate an alternate PI to be responsible for the safety of the PI’s laboratory in the PI’s absence. The alternate PI should be a faculty member who would provide guidance to the Environmental Protection Agency, Environmental Health & Safety, Iowa Departments of Health and Natural Resources, OSHA and/or the State Fire Marshall in the event of an inspection or accident. Any fines, penalties, or liabilities resulting from noncompliance to the safety guidelines are the responsibility of the PI. Every year and/or before their absence, the PI will review special safety precautions for the lab with the alternate PI using the PI’s Lab-Specific Safety Training form and the FSHN Quarterly Safety Inspection Checklist as guidelines. This training and the designation of the alternate PI will be documented and updated annually in the below form, signed by the PI and alternate PI and copies kept with the safety records in the lab and in the FSHN 2312 FSB office. Before the absence, the PI will notify all lab personnel of the alternate PI’s contact information and ensure that the temporary emergency contact information is posted on the outside of all lab doors designating the alternate PI and the time period of the PI’s absence. For absences due to personal or business travel, the PI will put the alternate PI’s name on the travel authorization form. PIs with animal protocols or labs authorized to use biohazards, bloodborne pathogens and/or radioactivity must designate an alternate PI whose lab has the same authorization.
