To say “protecting the right to health” could be interpreted erroneously as a right in force; I prefer to say “building the right to health”

![Page 9: J breilh 18 oct 2011](https://reader034.vdocuments.us/reader034/viewer/2022052601/559637bc1a28abfd148b478d/html5/thumbnails/9.jpg)
“La ciencia, como cualquier otra operación
simbólica es…una expresión
transformada, subordinada, transfigurada, y
algunas veces irreconocible de las relaciones
de poder de una sociedad.” (Bourdieau, 89)
“Science as any other symbolic operation
is ....a
transformed, subordinated, transfigured and
some times unrecognizable expression of the
social and power relations of a society”
(Bourdieau, 89)”
![Page 10: J breilh 18 oct 2011](https://reader034.vdocuments.us/reader034/viewer/2022052601/559637bc1a28abfd148b478d/html5/thumbnails/10.jpg)
Epidemiology, as “diagnostic” instrument of
public health experiences the
tensions, impulses and obstacles of all
knowledge that contributes to define societal
image and the degree of political success.
![Page 11: J breilh 18 oct 2011](https://reader034.vdocuments.us/reader034/viewer/2022052601/559637bc1a28abfd148b478d/html5/thumbnails/11.jpg)
Por ese motivo es indispensable debatir en
esta asamblea la determinación social de la
salud –DSS-.
For that reason a debate on social dtermination
of heralth –SDH- is indispensable in this
assembly.
![Page 12: J breilh 18 oct 2011](https://reader034.vdocuments.us/reader034/viewer/2022052601/559637bc1a28abfd148b478d/html5/thumbnails/12.jpg)
varios idiomas.
A debate that we started in the middle 70s in
Latin America has led to a vital scientific and
intellectual production that has been published
in numerous books and peer review articles in
Spanish, Portuguese and some even in English.
![Page 13: J breilh 18 oct 2011](https://reader034.vdocuments.us/reader034/viewer/2022052601/559637bc1a28abfd148b478d/html5/thumbnails/13.jpg)
It is surprising and paradoxical that these
contributions coming from the South have
been clearly and systematically
understated, not only in the discussion paper
of this conference, but also in many related
forums.
![Page 14: J breilh 18 oct 2011](https://reader034.vdocuments.us/reader034/viewer/2022052601/559637bc1a28abfd148b478d/html5/thumbnails/14.jpg)

In all periods of history you find clashing epidemiological
perspectives that explain social determination from
opposed views and reflect conflicting interests. The main
interpretative contradiction operates between empirical
multi-causation versus the paradigms of social
epidemiology and of critical epidemiology. But among these
last two, we can also ascertain important interpretative and
practical differences that this conference must debate.

• While it applies a more comprehensive causality -“the causes of the causes”-, it is still imprisoned in the uni-culturality of Eurocentric reasoning and the linearity of causal thinking and empirical associative thinking.

• This holds practical political implications because social epidemiology limits its action scope and political reasoning to redistributive governance, geared to the correction of isolated causes or health determinants.

Critical epidemiology relies on complex
dialectical process analysis and is geared
towards the transformation of the transnational
market production system and of the living
civilization models of modern
capitalism, which are intrinsically unhealthy.
![Page 25: J breilh 18 oct 2011](https://reader034.vdocuments.us/reader034/viewer/2022052601/559637bc1a28abfd148b478d/html5/thumbnails/25.jpg)
Buscamos superar la lógica no sustentable, no
solidaria, individualista, competitiva, de
desperdicio, y ecológicamente agresiva de la
reproducción social bajo el capitalismo y no
actuar sobre fragmentos que llamamos
“determinantes”.
We seek to overcome the unsustainable, non
solidary, individualist, competitive, wasteful
and environmentally antagonistic logic of
social reproduction of capitalism and not to act
cosmetically on fragments that some called
“determinants”.

abstractas, descontextualizadas, mientras la
economía política de la determinación social
ha sido apenas definida en términos vagos.
From our perspective, it seems illusory to start

empirical socio-environmental impact
variables, when the political economy of social
determination has been only defined in vague
terms.

It wouldn't be an overstatement to affirm that de
marginalization of this production by the WCSDH
is a sort of cultural inequity, that has become
one more of the health inequalities that we must
study and confront; a discrimination that should
be corrected in this conference’s declaration and
in future WHO documents.

• Ocupan enormes extensiones de territorio agrícola y desplazan cultivos alimentarios. (big extentions)
• Promueven modelo de monopolios y empresas transnacionales. (monopoly model)

To speak, for example, of food security, equitable nutrition and health in territories with rampant global loss of food sovereignty, of massive irresponsible application hazardous but profitable procedures, is at least an unforgivable naivety and in fact can become a lethal complicity.

algunas personas cercanas a nosotros. Nuestra

“Human beings are part of a complete totality which
we call Universe; we are a limited part in time and
space. We experience ourselves, our thoughts and
sentiments like something separated from the
rest…an optical ilusion of our concience. This
ilusion becomes like a a prison to us, restricting our
personal desires and even afinities and warmth in
front of people sorrounding us. Our challenge
should be to liberate ourselves of that prison by
expanding our circle of compassion to embrace all
living creatures and Nature in its beauty.” (trad. J.B.)

cariño hacia algunas personas cercanas a nosotros.

buen vivir. Nuestra lucha junto a múltiples

A PERSPECTIVE FOR EPIDEMIOLOGICAL
INNOVATION
(21st Century Health Research) (J.B.)
1) Incorporate multidimensional analysis (macro & micro orders) recognizing the dialectics of the generative potential of the individual, local, order, and the reproductive conditioning of the general, macro social order determined by the logic of capital accumulation.
2) Work in the simple/complex; simplicity/complexitydialectics.
3) Incorporate diversity (social, ethnic, gender, generational) without loosing sight of the communalities, codeterminations and the movement of diversity and structural determination.
4) Understand in our interpretative models the irregular, fussy, uncertain processes , without loosing sight of moments and expressions of regularity and historical, changing structural determinations.
5) Overcome the nomothetic and causal determinism logics, which have converted the search for constant conjunctions and causal explanations -with their linearity and reductionism-, and quantitative analysis, as the keys of scientific work.
