Probably something with unadorned walls, cold tile floors, beeping and blinking machines, and fluorescent overhead lights. Rooms that are sterile and hermetically sealed, with hard surfaces and neutral colors that remove patients from natural surroundings and make them and their visitors feel vulnerable and exposed. A place where the emphasis is on cure, not care. Hospices, however, are about caring. Take that comfortless mental image and put color and beautiful paintings on the walls. Add to it a window that allows natural light to stream in, and a door that leads to a carefully crafted garden filled with greenery and flowers. Soft music, recorded or live. Books nearby with inspirational thoughts and stories. What a tremendous difference it makes! These and other elements, when appropriately managed, create welcoming, reassuring environments that support and maintain the patient’s dignity, reduce stress and encourage loved ones’ participation in the delivery of care. “Art, in all of its forms, helps treat the patient’s body, mind and spirit,” says Hospice Chief Operating Officer Carol Brainerd. “An art-filled environment can offer sustenance to the soul and give meaning to experience. It can help individuals who are suffering transcend their pain through its ability to help the viewer or listener connect with the universal and eternal.” Citrus Valley Hospice is a compassionate place, and art is integral to its approach. Intended from its inception as a free-standing facility for end-of-life care rather than adapted from an existing building or hospital floor, it provides the best possible palliative care in a warm and soothing environment. Among the features of its inpatient facility are private patios with flowering plants; meditative and inspiring paintings and photographs in both the patient and family rooms; and recorded music as well as that which is performed live by volunteers, mostly on the piano, to provide quiet, pleasing background sounds for patients and their visitors. Art and the Hospice Environment This stained glass window in the Chapel was dedicated in memory of former patient Julia McGlothlin. An Artful Memorial Tribute One of the Hospice’s most striking and inspirational works of art is in the Chapel, which is dominated by three brilliantly colored stained glass windows. The scene they create is not religious, yet is life affirming. The windows are a memorial to Julia McGlothlin, a Hospice patient and long-time pediatric nurse at both the Inter- Community and Queen of the Valley campuses of Citrus Valley Medical Center, who died of breast cancer in 1993. The memorial was paid for by a memorial fund established by her husband, Keith McGlothlin, through the Inter- Community Hospital Foundation (now CitrusValley Health Foundation). The 4-foot by 6-foot panels, dedicated about 10 years ago, capture the things McGlothlin loved most: the mountains of her youth; the ocean; her children, represented by three sailboats on the water; and her seven grandchildren, represented by seven seagulls wheeling overhead. McGlothlin herself, who enjoyed windsurfing with her family, is seen as a windsurfer plying the waves near the shore.

Dear Hospice Friends,One of the things I am most proud of about our Hospice is

its end-of-life model program. To have a nationally knownPalliative Care Program, due in large part to David Kessler, aswell as a free-standing, in-patient hospice building, a homehospice program, and a home health agency that providespalliative home care to those who are housebound, is unique tothis community and to our program.

While we have a remarkable base of support from familieswho have experienced what our own Hospice has to offer, arecent study has revealed that a large percentage of patients and families who would benefitfrom hospice do not use it, or use it much later than is fully beneficial for the patient. Thestudy, headed by David Casarett, MD, assistant professor of Medicine at the University ofPennsylania and a senior fellow at the Leonard Davis Institute of Health Economics, revealedthat while one-third of all patients who die in this country receive hospice care, their lengthof stay is not optimal—less than three weeks, and sometimes even less than one day. Thedecision to enroll in hospice is complicated by the reluctance to accept a terminal diagnosis,the study notes.

This and other related studies show that although families recognize the value ofhospice in the patient’s last few days of life, earlier referrals and longer stays producegreater perceived benefits. The studies suggest that the optimal stay is between three weeksand three months. Also, patients and families considering hospice care were shown to havethe same types of questions, such as frequency of visits, practical support and paymentoptions. And finally, these studies show that a simple, five- to 10-minute intervention todetermine the appropriateness of hospice care can improve the quality of end-of-life care anddecrease resource hospital stays by promoting earlier access to hospice care in nursing homes.

Always striving to enhance the hospiceexperience, Friends of Hospice recentlyearmarked funds for, among other items,benches that will enable visitors to enjoythe facility’s leafy entryway, recordedmusic, inspirational literature, and akeyboard for the chapel for use in services.

One example of how art can helppatients is a study reported in theMay 2006 issue of The Journal ofAdvanced Nursing, which showed thatregularly listening to music helped providerelief to patients with back, neck or jointpain beyond that brought on by standardpain management techniques. Groups ofstudy participants who received standardcare and listened to music for an hour aday for a week experienced 20 percent lesspain than those who received standard carealone.

In addition to the comfort that artprovides to Hospice patients, it can alsocomfort those whom they leave behind.Citrus Valley Hospice offers a program forgrieving children called “I Count, Too”,which incorporates the use of art therapy,

Ahandbell choir, a profusion of pointsettias and old-fashioned globe-shaped “street lights” greeted LIGHT UP A LIFE attendees. The

annual event was held in the Hospice parking lot next to the towering pinetree on the seasonably cool and overcast night of Dec. 8, 2005. Speakersincluded new Chaplain Amy Arnold as well as Jack and Connie Watson, whothrew the switch that set the towering tree ablaze with lights. Linda Geleris,daughter-in-law of Citrus Valley Health Foundation Trustee Dr. I. PaulGeleris, who passed away during 2005, accompanied herself on guitar whileshe serenaded guests with holiday carols. The event featured the traditionalcandle lighting in memory of the “honor roll” of Hospice patients during theyear that was drawing to a close. The event raised $23,000.
